The existence of branchiogenic carcinomas is questioned in this review, which also discusses the different possibilities in an adult patient who presents with a cystic swelling in the upper lateral aspect of the neck. Most cases of branchiogenic carcinomas reported in the literature may actually have been secondary metastatic lesions from an occult tonsillar carcinoma. We will also discuss the different features of cystic metastasis that distinguish them from solid metastasis, and explain why oropharyngeal carcinomas with cystic neck metastasis should be in a separate category from other head

Multiple abnormal pharyngeal and pharyngolaryngeal bands were present in a child presenting with diffuse interstitial pulmonary fibrosis. The soft palate was fused to the posterior pharyngeal wall. Instead of there being facial pillars, a muscular band on either side extended from the base of the tongue to the lateral pharyngeal walls. There was neither tonsil nor adenoid tissue present. The epiglottis was tethered to the posterior pharyngeal wall by bands from its lateral aspects. The right arytenoid mound was tethered to the posterior wall of the hypopharynx. It is postulated that these aros

The brainstem is classically divided into functional columns including special and general subdivisions for somatic and visceral components. The term 'special visceral motor' is applied to branchiomotor nuclei, while 'special visceral sensory' refers to nuclei devoted to incoming gustatory and olfactory senses. The use of the term 'special visceral motor' is questioned in that the branchiomotor neurons function more like general somatic than general visceral motoneurons. The designation of taste and smell as 'special visceral sensory' systems seems inconsistent on several bases. First, taste a

First branchial cleft sinuses account for less than 1% of all branchial cleft anomalies. A thorough understanding of the embryology and developmental anatomy is essential for successful management. Nine cases of first branchial cleft sinuses treated at The Hospital for Sick Children by the Department of Otolaryngology between 1984 and 1990 were reviewed. A large proportion of these lesions were initially misdiagnosed despite significant symptomatology. This resulted in a high rate of infectious complications. Early diagnosis, prompt control of infection, and early surgical excision are recomme

Three sibs were affected by numerous accessory tragi. On clinical examination, no other associated developmental defects, such as the Goldenhar syndrome, were detected.

Hemifacial microsomia is the second most common congenital facial anomaly. It occurs as a spontaneous event during embryogenesis and involves structures derived from the first and second branchial arch. In the past treatment of the skeletal defect was delayed until the child ceased growing and end-stage of the deformity was reached. More recent approach to analysis and treatment of this anomaly, emphasizing early treatment, to give the patient the maximum benefit of a combined surgical and orthodontic therapy, is described.

We present an HRP study of the sensory tracts and motor nuclei associated with feeding (especially use of the tongue) in plethodontid salamanders (mainly Batrachoseps attenuatus, Bolitoglossa subpalmata, Desmognathus ochrophaeus, Eurycea bislineata, and Plethodon jordani). The nerves studied are VII (ramus hyomandibularis only), IX, X, XI, the first spinal nerve (hypoglossus), and the second spinal nerve. Two types of sensory projections are universally found in the brainstem: superficial somatosensory projections of VII, IX, and X, and deeper visceral sensory projections of IX and X to the fa

On the basis of two cases, one a fistula and the other a cyst of the first brachial cleft with chiefly parotid manifestations, a review of the embryology is given which explains the anatomical features. The elements in their clinical diagnosis, complemented by fistulography and sialography are compared with operative findings, the essential steps in which are as follows: 1) Identification of the facial nerve which by necessity overlies the lesions. 2) Partial resection of the superficial lobe of the parotid. 3) The essential resection of the external auditory meatus, the keystone of radical cu

The recurrent and severe infections of the ENT region during childhood are frequently related to cranio-facial malformations or/and deficiency of the immune system. The cranio-facial abnormalities are at risk to be complicated by transmission deafness either primary or secondary through recurrent middle ear infections. In our pediatric out-patient clinic, most of the patients suffering severe recurrent ENT problems show variable malformations: abnormal implantation or shape of the external ear, a microretrognathism, cervical or facial branchial fistulae, high or ogival palate with anomalies of

Branchial cleft anomalies are the second most common congenital neck lesion. They present as cysts, sinuses, or fistulas arising from remnants of the pharyngeal apparatus; second arch anomalies predominate. The embryologic origin determines their anatomical relationships to nerves, vessels, and the pharynx and guides diagnosis and surgical planning. Typical findings are cervical swelling, cutaneous discharge, or recurrent infection. Third and fourth branchial anomalies may cause recurrent neck abscesses or acute suppurative thyroiditis, particularly in children. Ultrasonography is the primary

To discuss the clinical manifestations and difficulties in the diagnosis and treatment of Type Ⅱ first branchial cleft anomaly with parotid fistula,to summarise experience,to reduce misdiagnosis and missed diagnosis, and to select surgical methods accurately. A comprehensive literature review was performed, and the management of one patient with this rare combination was retrospectively analyzed. Particular attention was paid to presenting symptoms, ancillary imaging findings, and surgical pearls. Complete pre-operative evaluation, accurate anatomical delineation, and individualized surgical pl

Third and fourth branchial arch anomalies belong to congenital lesions which are extremely rare accounting for <1-4% of branchial arch anomalies. In our case, a 4-year-old boy was admitted to our hospital with a painful mass on the left side of his neck. The imaging and clinical findings are oriented to the diagnosis of suppurative thyroiditis with underlying third/fourth branchial arch anomaly. The diagnosis was confirmed with laryngoscopy and the child was treated with endoscopic cauterization. The presence of specific clinical and imaging findings is not definitive, so the diagnosis relies

Branchial cleft anomalies are congenital, arising from the first to the fourth pharyngeal clefts. The most common is a second arch anomaly. As it is congenital, it presents at birth though may become symptomatic later. The spectrum of anomalies includes sinus, cyst, or fistula formation or a combination of these. Here we present a case series based on first cleft anomalies. The principles of management include early diagnosis, excision of any fistulous tract, and prevention of injury to the facial nerve.

The origin of the mammalian middle ear ossicles from mandibular and hyoid pharyngeal arches remains controversial and discussed. Two adverse theories are proposed. The first claims that malleus and incus derive from the Meckel's cartilage of the mandibular arch, and stapes from Reichert's cartilage of the hyoid arch. The second postulates that handle of malleus and long process of the incus are derived from the second arch as well as the stapes. Contradictory analyses support alternatively each theory without any experimental evidence. In order to bring new data, we analyzed by immunohistochem
