Real-world Study of Three-gas Breath Testing Nationwide and the Association With Symptoms.

GOALS AND BACKGROUND: Hydrogen (H 2 ) breath testing (BT) is used to diagnose small intestinal bacterial overgrowth (SIBO), and methane (CH 4 ) to diagnose constipation. Recently, a third microbially derived gas, hydrogen sulfide (H 2 S), was added to BT and associated with diarrhea. Here, we assess a 3-gas BT nationwide, comparing the results to gastrointestinal (GI) symptoms. STUDY: Consecutive subjects (N=6000) undergoing 3-gas at-home BT were asked to complete a symptom questionnaire. Substrate choice (glucose or lactulose) was at physician's discretion. H 2 , CH 4, and H 2 S levels, and

Hydrogen Production Dynamics During Lactulose Breath Testing in Patients with Suspected SIBO.

Background: Small intestinal bacterial overgrowth (SIBO) is characterized by excessive microbial colonization of the small intestine and is commonly diagnosed using hydrogen breath tests. However, most studies focus primarily on diagnostic thresholds rather than the overall dynamics of hydrogen production during the test. Methods: This cross-sectional study included 162 adults with chronic gastrointestinal symptoms who underwent a lactulose hydrogen breath test. Hydrogen concentrations were measured every 20 min over a 180 min period. Total hydrogen production was quantified using the area und

Application of the CO breath test to explore the reversibility of hyperglycemia's impact on erythrocyte lifespan.

Hyperglycemia can shorten red blood cell (RBC) lifespan, leading to incorrectly measured glycated hemoglobin (HbA1c) values. Correcting for the impact of the RBC lifespan on HbA1c is a critical issue in clinical practice. Before establishing a generally accepted correction formula to account for the impact of RBC lifespan on HbA1c, it is necessary to investigate the duration necessary to emake the hyperglycemia-induced RBC lifespan shortening reverse to normal. This longitudinal clinical trial examined the RBC lifespan in 31 hospitalized patients with type 2 diabetes mellitus by measuring the

The limitations of mouth alcohol detection systems in breath alcohol testing: Case reports.

Contamination of a person's breath alcohol with alcohol from sources other than the lungs can falsely elevate the results of a breath alcohol test. This is known as "mouth alcohol" contamination in breath alcohol testing. To combat the issue of mouth alcohol contamination, manufacturers of breath alcohol analyzers have incorporated slope detection algorithms into their devices known as "slope detectors." Despite this effort, breath alcohol analyzers sometimes fail to detect mouth alcohol contamination. Three case reports presented in this paper strongly suggest undetected mouth alcohol contami

Effort During Ethanol Breath Testing Impacts Correlation with Serum Ethanol Concentration.

INTRODUCTION: The gold standard for quantifying ethanol intoxication in patients is serum testing. However, breath testing is faster, less expensive, and less invasive. It is unknown whether perceived effort during a breath ethanol test impacts the accuracy of the test and the correlation with serum concentration. In this study we analyzed whether perceived "poor" effort during breath ethanol testing would result in worse correlation than perceived "normal" breath-testing effort with respect to serum ethanol concentration. METHODS: Subjects were identified retrospectively over a 49-month peri

Can Symptoms and Medical History Predict Outcomes for Hydrogen and Methane Breath Testing?

INTRODUCTION: Hydrogen and methane breath testing (HMBT) is used to diagnose small intestinal bacterial overgrowth (SIBO) and intestinal methanogen overgrowth (IMO). There are limited data on whether symptoms and medical history can predict outcomes on lactulose HMBT. METHODS: A large survey-based study was conducted in 731 patients who completed lactulose HMBT at a single tertiary center in the United Kingdom. The 32-item survey included questions relating to symptoms and medical history. Survey items were statistically interrogated to determine associations with SIBO, IMO, or breath gas pro

Methane gas in breath test is associated with non-alcoholic fatty liver disease.

Although the associations between a patient's body mass index (BMI) and metabolic diseases, as well as their breath test results, have been studied, the relationship between breath hydrogen/methane levels and metabolic diseases needs to be further clarified. We aimed to investigate how the composition of exhaled breath gases relates to metabolic disorders, such as diabetes mellitus, dyslipidemia, hypertension, and nonalcoholic fatty liver disease (NAFLD), and their key risk factors. An analysis was performed using the medical records, including the lactulose breath test (LBT) data of patients

Sleep-disordered breathing and cardiovascular disease: who and why to test and how to intervene?

Sleep-disordered breathing (SDB) is common in individuals with established cardiovascular disease (CVD), particularly those with heart failure (HF). There are two main types of SDB, central sleep apnoea (CSA) and obstructive sleep apnoea (OSA) which frequently overlap as mixed SDB. Investigating for SDB could be considered in patients with excessive daytime sleepiness, male sex, high body mass index, low ejection fraction, atrial fibrillation (AF), in patients with no dipping blood pressure pattern, recurrent paroxysms of nocturnal dyspnoea or when an apnoea is witnessed. Excessive daytime sle

Hydrogen and Methane-Based Breath Testing in Gastrointestinal Disorders: The North American Consensus.

OBJECTIVES: Breath tests (BTs) are important for the diagnosis of carbohydrate maldigestion syndromes and small intestinal bacterial overgrowth (SIBO). However, standardization is lacking regarding indications for testing, test methodology and interpretation of results. A consensus meeting of experts was convened to develop guidelines for clinicians and research. METHODS: Pre-meeting survey questions encompassing five domains; indications, preparation, performance, interpretation of results, and knowledge gaps, were sent to 17 clinician-scientists, and 10 attended a live meeting. Diagnosing lactose malabsorption in children: difficulties in interpreting hydrogen breath test results.

Lactose malabsorption (LM) is caused by insufficient enzymatic degradation of the disaccharide by intestinal lactase. Although hydrogen (H2) breath tests (HBTs) are routinely applied to diagnose LM, false-negative results are not uncommon. Thirty-two pediatric patients (19 females, 13 males) were included in this prospective study. After oral lactose administration (1 g kg(-1) bodyweight to a maximum of 25 g), breath H2 was measured by electrochemical detection. HBT was considered positive if H2 concentration exceeded an increase of  ⩾20 ppm from baseline. In addition to H2, exhaled methane (C

Rapid point-of-care breath test for biomarkers of breast cancer and abnormal mammograms.

BACKGROUND: Previous studies have reported volatile organic compounds (VOCs) in breath as biomarkers of breast cancer and abnormal mammograms, apparently resulting from increased oxidative stress and cytochrome p450 induction. We evaluated a six-minute point-of-care breath test for VOC biomarkers in women screened for breast cancer at centers in the USA and the Netherlands. METHODS: 244 women had a screening mammogram (93/37 normal/abnormal) or a breast biopsy (cancer/no cancer 35/79). A mobile point-of-care system collected and concentrated breath and air VOCs for analysis with gas chromatog

Variability of a closed, rebreathing setup for multiple breath wash-out testing in children.

OBJECTIVE: The multiple breath wash-out technique (MBW) that measures lung clearance index (LCI) and functional residual capacity (FRC) may be more sensitive than spirometry for identification of early obstructive airways disease. The open MBW setup using mass spectrometry referenced in previous publications is not readily available in the U.S. Our objective was to assess validity and sensitivity of a commercially available device that uses a closed (rebreathing) setup with photoacoustic spectroscopy for MBW testing. STUDY DESIGN AND METHODS: Subjects aged 5-21 who were either healthy or had
