Bulimia nervosa is characterized by episodes of compulsive eating of large amounts of food that are followed by measures undertaken to avoid weight gain, and an exaggerated concern about one's figure and weight. Bulimic symptomatology comprises a complex of various interacting components such as poor self-esteem, a negative attitude towards one's own body, eating behavior and current conflicts. Treatment must take account of all of these factors, and involves both internistic and psychotherapeutic elements. The severity of the condition, possible comorbidities, and psychosocial criteria are cr…

The eating disorder bulimia nervosa has been associated with impaired satiety, decreased resting metabolic rate, and abnormal neuroendocrine regulation. Preclinical studies suggest that such alterations could be associated with impaired leptin function. Thus, the goal of this study was to assess whether leptin function is decreased in bulimia nervosa. Serum leptin levels measured in women with bulimia nervosa (n = 18) and in women who had maintained stable recovery from bulimia nervosa (n = 15) were compared with values in healthy female controls (n = 20). Subjects were studied during the foll…

BACKGROUND: Since bulimia nervosa's introduction to the psychiatric nomenclature in 1979, data concerning long-term outcome have been largely unavailable.
METHODS: Women with the diagnosis of bulimia nervosa between 1981 and 1987 who participated in 1 of 2 studies were located and invited to participate in follow-up assessments.
RESULTS: More than 80% of the women from these studies participated in follow-up assessments and the results represent findings for 173 women. More than 10 years following presentation (mean+/-SD length of follow-up, 11.5+/-1.9 years), 11% of this sample met full cri…

Bulimia nervosa is an eating disorder that is growing in prevalence in Western societies, and affecting more people at a younger age. Requests for treatment are creating serious problems for mental health services, making it necessary to establish appropriate treatment guidelines aimed at increasing the effectiveness of clinical interventions and guaranteeing a rational therapeutic approach. For this reason, research on predictive factors of the outcome of bulimia nervosa has greatly increased in recent years. This article reviews this recent research and discusses the most prevalent clinical …

BACKGROUND: Evidence that serotonin-active antidepressant medications decrease binge eating in patients with bulimia nervosa has fueled interest in the hypothesis that abnormal serotonergic neurotransmitter function contributes to symptoms of the disorder. To evaluate this hypothesis, we employed pharmacological challenge testing to compare serotonin function in patients with bulimia nervosa and healthy controls.
METHODS: Neuroendocrine response patterns were compared for 15 nonhospitalized, medication-free, normal-weight women with bulimia nervosa and 14 age-matched healthy female controls. …

There is a long tradition of psychoanalytically oriented treatments for in-patients in Germany and a large proportion of bulimics is treated in this way, but there are very few reported controlled trials for this form of treatment. The treatment rationale for psychoanalytically orientated psychotherapy is discussed against the background of the specific psychopathology of bulimia and the principal elements of this form of treatment are presented. Recent results of a controlled study involving n = 32 Patients with a follow-up of 38 months are reported with regard to comparable evaluations. The …

The present study examined daughter and parent variables associated with bulimia nervosa, including attitudes about body image, eating and dieting histories, and self-esteem. Subjects were female undergraduates (bulimic, subclinical bulimic, and normal) their parents. Daughters differed in bodymass, with bulimics largest, followed by subclinicals, and finally controls. After daughter's bodymass was covaried, daughters differed in the expected direction on eating and body measures and perceived pressure from mothers to lose weight. Bulimic and subclinical daughters also perceived their mothers,…

The purpose of the present study was to examine the relationship between parental factors and bulimia nervosa. Subjects were female undergraduate students (classified as bulimic, subclinical bulimic, or normal) and their parents. Daughters and their parents were administered a questionnaire which assesses attitudes about being overweight, radical dieting, and parental pressure to lose weight. The bulimic group, and in some instances the subclinical group, were significantly different in that they reported perceiving themselves to be overweight, engaging in radical means of weight control, and …

Bulimia nervosa is a prevalent disorder of unknown cause, characterized by recurrent episodes of uncontrollable eating. In the light of recent evidence that the gastrointestinal hormone cholecystokinin induces satiety and reduces food intake in laboratory animals and humans, we investigated the hypothesis that abnormalities in cholecystokinin secretion and satiety may occur in patients with bulimia and contribute to their disturbed eating patterns. Blood levels of cholecystokinin and subjective satiety were measured in 14 women with bulimia and 10 normal women before and after a mixed-liquid m…

Bulimia nervosa is a health problem of increasing magnitude that is estimated to affect 2-5% of the American adolescent and young adult female population. Because of the magnitude of this clinical problem and because of the importance of the upper gastrointestinal tract in its expression, a intradepartmental program of health care for patients affected with the disease was initiated. Eleven consecutive symptomatic bulimic individuals have been evaluated jointly by the gastroenterology and the psychiatry departments of the University of Pittsburgh. Five of these 11 individuals were found to hav…

Recent psychotherapy studies for bulimia nervosa are reviewed and it is concluded that many interventions lead to statistically and clinically significant reductions in the frequency of bingeing and vomiting and other associated symptoms; however, the results are more variable if success is defined as abstinence from symptomatic behavior. Treatment and patient variables which seem to be associated with positive response to treatment are reviewed and serious methodological concerns are raised. Carefully controlled dismantling trials are recommended in order to isolate factors which are critical…

Bulimia is characterized by recurrent episodes of binge eating and severe self-deprecation, often accompanied by self-induced vomiting and/or laxative abuse. It is most often found among young women in their late teens to mid-30s. Estimates of the disorder's prevalence vary widely, depending on the diagnostic criteria used, but usually range from 5% to 20% of college age women. Binge eating typically begins in late adolescence, frequently after a period of dieting to lose weight. Self-induced vomiting usually follows the onset of binge eating by about a year. To date, theories of the disorder'…

Body image perception was measured in 50 women with bulima nervosa and 19 age and weight matched female controls, using a visual size estimation apparatus. Both groups overestimated body widths, but not the width of a neutral object, and whilst there was a trend for bulimics to overestimate more than controls this did not reach significance. The part of the body most overestimated corresponded to the part most disliked in only a third of both groups. The bulimics without a previous history of anorexia nervosa overestimated body width more than those with such a history; this may be related to
