Artificial intelligence for outcome prediction in burn care: A scoping review.

INTRODUCTION: Machine learning (ML) is increasingly applied across burn care, but its clinical utility remains unclear. This scoping review mapped current evidence on ML models developed to predict key outcomes in burn patients, including mortality, sepsis, inhalation injury, delirium, surgical decision making, intraoperative blood loss and acute kidney injury (AKI). METHODS: Following the Preferred Reporting Items for Systematic Reviews and Meta Analyses extension for Scoping Reviews (PRISMA-ScR) guidelines, two databases were searched from 2020 to 2025. Studies using ML to predict diagnosti

A new nomogram to predict the need for tracheostomy in burned patients.

PURPOSE: To evaluate the impact of tracheostomy on complications, dysphagia and outcome in second and third degree burned patients. METHODS: Inpatient mortality, dysphagia, severity of burn injury (ABSI, TBSA) and complications in tracheotomized burn patients were compared to (I) non-tracheotomized burn patients and (II) matched tracheotomized non-burn patients. RESULTS: 134 (30.9%) out of 433 patients who underwent tracheostomy, had a significantly higher percentage of inhalation injury (26.1% vs. 7.0%; p < 0.001), higher ABSI (8.9 ± 2.1 vs. 6.0 ± 2.7; p < 0.001) and TBSA score (41.4 ± 19.7

Surfactant administration in severe inhalation injury: case report.

Inhalation injury is one of the main causes of death in patients with severe burns. Administration of exogenous surfactant appears promising for the treatment of acute respiratory failure. We report our clinical experience with this approach. A 35 yr-old man was admitted to our burns unit after an industrial accident. He had sustained a 60% total body surface area full-thickness burn combined with severe inhalation injury. Fiberoptic bronchoscopy confirmed the diagnosis, demonstrating severe blisters and ulcers of the bronchial mucosa. Refractory hypoxaemia (PaO (2)/FiO (2)56 mm Hg, where PaO

Pulmonary complications in burn patients resuscitated with a low-volume colloid solution.

The occurrence of the adult respiratory distress syndrome (ARDS) and pneumonia was assessed in 482 patients with severe burns, but without evidence of inhalation injury. The patients were resuscitated during the burn shock period with a low-volume formula consisting mainly of plasma. The incidence of ARDS was 2.5 per cent for the entire burn population, and 9.4 per cent for 65 patients with burns covering more than 50 per cent of the body surface area. The general incidence of pneumonia was 4.4 per cent but was 12.5 per cent for the 65 patients with the extensive burns. These incidences were c

Lisbon Burn Centre experience with intentional burn injuries.

Burn injury as a form of hetero or auto-aggression accounts for a significant amount of admissions to a Burn Care Unit, with epidemiologic and clinical specificities. To investigate the differences in risk factors, psychiatric comorbidities, injury severity and mortality among adult patients with accidental or intentional burns, we analyzed routinely collected data from a Central Hospital Burn Unit over a period of 6 years (January 1st, 2010 to December 31st, 2015). We identified 22 intentional burn patients (5%) among all the admissions to our Burn Unit. When compared to the accidental burns,

Evaluation of human brain damage in fire fatality by quantification of basic fibroblast growth factor (bFGF), glial fibrillary acidic protein (GFAP) and single-stranded DNA (ssDNA) immunoreactivities.

Burns and inhalation of toxic gases, including carbon monoxide (CO) and cyanide, which are produced by combustion, are major factors involved in fire death. The present study immunohistochemically investigated basic fibroblast growth factor (bFGF), glial fibrillary acidic protein (GFAP) and single-stranded DNA (ssDNA) in the brains of fire fatalities (n=49) to examine the differences between fatal burns and CO intoxication, compared with those in cardiac deaths (n=24) and mechanical asphyxiation cases (n=23). In acute fire fatality, neuronal ssDNA immunopositivity in the cerebral cortex of the

Short-Term Outcomes of Burn Patients: a Tertiary Hospital's Four-Year Experience.

Introduction: Burns are the fourth cause of trauma worldwide, with a yearly death toll of 180,000. Multiple studies have investigated prognostic factors for the short-term and long-term evolution of burns, commonly citing age, percentage of total body surface area affected (BSA), percentage of third-degree burn, inhalation injury, surgery and overall depth of burn. The aim of our study is to review all cases managed in a tertiary emergency center and find factors associated with duration of hospital stay. Material and methods: The official reporting of burns from 2020-2023 "Bagdasar-Arseni" Em

Mortality and causes of death in a burn centre.

Mortality rates are important outcome parameters after burn, and can serve as objective end points for quality control. Causes of death after severe burn have changed over time; in the international literature, multisystem organ failure is seen as the most important cause, but the exact distribution of causes of death remains unknown. Insight into underlying agents of mortality can be directive in research and prevention programmes. This comparison between results from the Rotterdam Burn Centre (RBC) and the American National Burn Repository (NBR) examines the most important predictive paramet

Smoke, Scars, and Survival: A Six-Year Analysis of Burn Mortality in a Resource-Limited Appalachian ICU.

BACKGROUND: Despite recent advances in burn management, burn injuries remain a major global cause of morbidity and mortality, with rural and underserved populations, such as those in Appalachia, being disproportionately affected. Contributing factors include limited access to specialized care and a high prevalence of comorbidities. Understanding the prognostic factors associated with mortality in adult burn patients is critical for guiding clinical care and resource allocation, particularly in resource-limited settings. METHODS: This retrospective study analyzed data from adult burn patients

Nonmedical Factors Influencing Early Deaths in Burns: A Study of the National Burn Repository.

It is well-established that survival in burn injury is primarily dependent on three factors: age, percent total-body surface area burned (%TBSA), and inhalation injury. However, it is clear that in other (nonburn) conditions, nonmedical factors may influence mortality. Even in severe burns, patients undergoing resuscitation may survive for a period of time before succumbing to infection or other complications. In some cases, though, families in conjunction with caregivers may choose to withdraw care and not resuscitate patients with large burns. We wanted to investigate whether any nonmedical

Microalbuminuria in acute burn injury.

Microalbuminuria is a known finding in inflammatory states. We hypothesized that urinary albumin/creatinine ratio (ACR) would correlate with injury severity and resuscitation demands after acute burns. This pilot study evaluated 30 adults admitted within 12 hours of injury with burns > or =10% total body surface area burn injury (TBSA). The urinary ACR was calculated for each patient at 7 to 12 hours, 19 to 24 hours, and 43 to 48 hours following burn injury. Microalbuminuria was defined as a urinary ACR > or =20 mg/g. Study patients (23 males, 7 females) had a mean age of 42.9 + 14.0 years and

The measured effect magnitude of co-morbidities on burn injury mortality.

INTRODUCTION: The ability to better prognosticate burn injury outcome is challenging and historically, most center use the Baux or revised Baux score to help prognosticate burn outcome, however, the weighted contribution of comorbidity on burn mortality has traditionally not been accounted for nor adequately studied. We therefore sought to determine the effect of comorbidities, using the Charlson comorbidity index (CCI) on burn mortality. METHODS: The purpose of this study was to determine the effect of comorbidities on burn injury mortality as determined by the LA50 (lethal TBSA burn at whic

[Systemic complications of extended burns].

Burns covering more than 10% of the total body surface area (TBSA) are responsible for systemic perturbations which, in very severe cases, can represent a vital risk and, in all cases, affect the wound evolution. Among these general perturbations, fluid volume and electrolyte changes, leading eventually to burn shock, have the most dramatic consequences. Burn shock is, still to day, a vital risk and can also, in case of inadequate early fluid resuscitation, results in secondary morbidity and mortality. Fluid replacement during the very first hours after injury represents certainly a key point

Burns, Aging, and Appalachia: The Untold Story of Hospital Stays.

Background Burn injuries are among the top 10 causes of trauma and premature death globally. While burns are more prevalent in low-income countries, geriatric burns are disproportionately observed in developed nations, where longer life expectancies result in a larger elderly population. Geriatric burn patients face unique challenges, including slower healing, higher risk of complications, and increased hospital length of stay (LOS), which may be further exacerbated in resource-limited regions such as Appalachia. This study investigates factors influencing hospital LOS in geriatric burn patien

Management of acute burns and burn shock resuscitation.

Initial management of minor and moderate, uncomplicated burn injury focuses on wound management and patient comfort. Initial management of patients with major burn injury requires airway support, fluid resuscitation for burn shock, treatment for associated trauma and preexisting medical conditions, management of adynamic ileus, and initial wound treatment. Fluid resuscitation, based on assessment of the extent and depth of burn injury, requires administration of intravenous fluids using resuscitation formula guidelines for the initial 24 hours after injury. Inhalation injury complicates flame

The prognostic relevance of full-thickness burns on ABSI.

INTRODUCTION: The Abbreviated Burn Severity Index (ABSI) by Tobiasen, which is commonly used to estimate the mortality risk of severely burned patients, calculates an additional point for the existence of full-thickness (third-degree) burns. [1] However, the score does not consider the extent of the body surface affected by third-degree burns. To understand whether there is a way to improve ABSI prediction power, this study aims to determine the influence of full-thickness burns on survival rates and how it affects the predictive precision of the ABSI. MATERIAL AND METHODS: In this study, the
