[Unilateral congenital aplasia of the scaphoid and dysplasia of the trapezium (a case report)].

Aplasia of the carpal scaphoid bone in association with dysplasia of the trapezium is a rare disorder. A right-handed, 66 years old male, suffering from a trapeziometacarpal arthritis on the right side is reported. X rays, MRI and CT-scan reveal an absence of scaphoid and a dysplasia of the trapezium. The lunate remains in place on the radius but is oriented in dorsi flexion. The capitate is dorsally and radially subluxated. Trapezium inclination of the joint at the base of the first metacarpal (trapezial tilt) is 32 degrees greater than on the opposite side which is normal (48 degrees). Radia

Apophyseal fracture of the greater trochanter.

Apophyseal fractures about the pelvis and proximal femur are well-described; however, these injuries rarely involve the greater trochanter. We report the case of a 15-year-old boy of large build who appeared to have all signs and symptoms of a left slipped capital femoral epiphysis. No specific inciting event had occurred before the hip pain. Radiographs and bone scan of the capital femoral epiphysis appeared normal, and follow-up radiographs confirmed an apophyseal fracture of the greater trochanter. This case represented a rare occurrence, and its interesting manifestation was similar to tha

Midcarpal fusion: Complications and outcomes of endomedullary screws technique with three techniques: Four-corner, luno-capitate and bicolumnar fusion. Case series.

INTRODUCTION: We report the clinical and radiographic outcomes, and complications of midcarpal fusions, four corner fusion (4CF), bicolumnar (BC) and luno-capitate (LC), with a focus on endomedullary screw fixation. METHODS: Between 2018 and 2024, 38 patients underwent three distinct types of midcarpal fusions: 20 underwent 4CF, 6 underwent BC and 12 underwent LC. Postoperative evaluation was conducted using X-ray and computed tomography scans to assess the degree of fusion, correction of the lunate and capitate bones, presence of possible screw prominences and secondary radio-carpal degenera…

The vascularity of the wrist. Identification of arterial patterns at risk.

The intraosseous and extraosseous vascularity of carpal bones was studied in 75 cadaver limbs. Vascular patterns were correlated with the clinical incidence of avascular necrosis, and at-risk patterns of vascularity were identified. The carpal bones were found to fall into three groups. Group 1, which consisted of the scaphoid, the capitate, and eight percent of the lunates, had either vessels entering only one surface or large areas of bone that were dependent on a single vessel. This group was the most vulnerable to posttraumatic avascular necrosis. The bones of Group 2, the hamate and the t

Dairy products and bone health.

Bone mineral mass, geometry and microstructure, hence determinants of fracture risk, result bone accrual during growth and bone loss later in life. Peak bone mass, which is reached by the end of the second decade of life, is mainly determined by genetic factors. Among other factors influencing bone capital, dietary intakes, particularly calcium and protein, play a significant role in peak bone mass attainment. Both nutrients are provided in dairy products, which accounts for 50-60% and 20-30% of the daily calcium and protein intakes, respectively. Children avoiding dairy products are at higher

Proximal Row Carpectomy with and without Capitate Resurfacing: A Preliminary Systematic Review and Meta-Analysis.

BACKGROUND: In wrist salvage, proximal row carpectomy (PRC) has increasingly shown superior outcomes to four-corner fusion (4CF). Furthermore, PRC with resurfacing capitate pyrocarbon implants (PRC + RCPIs) provides a treatment option that may allow patients to avoid 4CF or wrist arthrodesis and help restore natural joint function and distribute loads evenly across the implant, though RCPI has yet to be evaluated on a large scale. We aimed to compare outcomes between PRC and PRC + RCPI for the treatment of scapholunate advanced collapse (SLAC) and scaphoid nonunion advanced collapse (SNAC) wri

Proximal femoral changes related to obesity: an analysis of slipped capital femoral epiphysis pathoanatomy.

Recent research has revealed the importance of the femoral epiphyseal tubercle and cupping height in the stability of the physis and its association with capital femoral slippage. To better understand the connection between the pathogenesis of slipped capital femoral epiphysis and obesity, we performed a retrospective analysis of proximal femur and acetabular anatomies using computed tomography (CT) scans in the hips of normal weight and obese pediatric patients. We measured morphologic characteristics of the proximal femur and acetabulum in developing hips of 31 obese adolescent patients and

Tuberculosis of Distal Ulna: A Case Report.

BACKGROUND: The occurrence of tuberculosis in the wrist constitutes a rare manifestation. Predominant sites for primary osseous tuberculosis involvement are typically the capitate bone or the distal end of the radius. The presentation of wrist tuberculosis as an isolated lytic lesion in the distal ulna represents an exceptionally infrequent phenomenon, frequently resulting in misdiagnosis as malignancy and consequent delays in therapeutic intervention. CASE DESCRIPTION: Herein, we report the case of a 23-year-old healthy man with persistent ulnar-sided pain in his left wrist over the past 1 y…

Fracture of an aberrant os styloideum: a unique case report.

Several accessory bones can be found around the wrist, which may pose a diagnostic challenge. Hereby, we report a unique case of a traumatic fracture of an aberrant os styloideum in a 66-year-old male. The patient presented with a swollen dominant right hand and a hematoma on its anterior and posterior aspects, as a result of a fall on an outstretched hand. The diagnosis was based on a CT scan, which showed a fractured accessory bone between the distal portion of the capitate bone and the base of the third metacarpus. Due to its typical location, the ossicle was identified as os styloideum. Th…

Four-corner bone arthrodesis with dorsal rectangular plate: series and personal technique.

Controversy exists about the best method to achieve bone fusion in four-corner arthrodesis. Thirty-five patients who underwent this procedure by our technique were included in the study. Surgical indications were stage II-III SLAC wrist, stage II SNAC wrist and severe traumatic midcarpal joint injury. Mean follow-up was 4.6 years. Mean active flexion and extension were 34 degrees and 30 degrees respectively; grip strength recovery was 79%. Radiological consolidation was achieved in all cases. The mean DASH score was 23 and the postoperative pain improvement by visual analogue scale was statist

Bone-tissue-bone repairs for scapholunate dissociation.

Scapholunate dissociation is a commonly seen and treated form of carpal instability. Several surgical options have been used for the repair of scapholunate instability over the past 50 years. These have included benign neglect, reduction and percutaneous pinning, primary repair, partial fusions, tendon weaves, and combinations of these. Recent advancements in scapholunate repair and anatomy have been aimed at a more physiologic repair. Composite replacement of the entire scapholunate interval, similar to other tendon repairs seen in orthopedic surgery, has become popular. Bone-tissue-bone (BTB

Natural history of early onset and late-onset Legg-Calve-Perthes disease.

Legg-Calve-Perthes disease develops after interruption of the blood supply to the capital femoral epiphysis. This results in various changes in the femoral epiphysis and metaphysis, the capital femoral epiphysis, growth plate, and the acetabulum. The necrotic bone of the epiphysis is gradually replaced by new bone, and over 2 to 4 years complete healing of the epiphysis occurs. The evolution of this process can be clearly seen on radiographs and the disease can be divided into distinct stages. In the early stages of the disease, the epiphysis may extrude outside the confines of the acetabulum

Isolated Osseous Excision in the Adult Carpus: A Narrative Review.

Various pathologies of the adult carpus result in clinical scenarios where excision can be considered and even recommended. In the appropriate patient population, isolated carpal excision can alleviate pain and improve mobility. Excisions of the pisiform, trapezium, and trapezoid have abundant literature evidence to support positive long-term functional outcomes. In contrast, isolated excision of the capitate, hamate, and triquetrum has limited support in the literature secondary to compromise of carpal mechanics and lead to recurrent pain. Additionally, isolated scaphoid and lunate excision a

Remodelling of the femoral head after proximal femoral osteotomy for avascular necrosis associated with slipped capital femoral epiphysis.

Avascular necrosis is the most devastating complication associated with slipped capital femoral epiphysis. In this case report, we present the clinical and radiological outcome of two patients after proximal femoral osteotomy for avascular necrosis associated with slipped capital femoral epiphysis. The Iowa hip score was used to assess the function. Periodic radiographic observations showed no progression of the collapse, and bone remodelling of the necrotic femoral head. Our evaluation of patients who underwent proximal femoral osteotomy for avascular necrosis prior to collapse of the femoral

[Therapy of acute and subacute epiphysiolysis of the femur head].

Therapy and prognosis of the slipped capital femoral epiphysis are discussed in connection with the results obtained in the authors' own patients. Reduction of a dislocated femoral head is always desirable. If closed reduction fails, it will be necessary to reduce in full view. In smaller children, fixation is effected by means of several Kirschner wires, in older ones via screw osteosynthesis. Bone pegging or osteotomy are not suited for treatment of an acute or subacute slipped capital epiphysis.

Further investigations of the intraosseous pressure characteristics in necrotic lunates (Kienböck's disease).

To confirm the hypothesis that venous congestion plays a role in idiopathic lunate necrosis (Kienböck's disease), intraosseous pressure in 12 normal and 12 necrotic lunates was measured. Intraosseous pressure in the capitate of 12 healthy subjects served as reference measurement. The intraosseous pressure was recorded in neutral position and 60 degrees extension of the wrist under normotensive conditions and during venous stasis. In all groups, the mean intraosseous pressure rose significantly with extension of the wrist, with the largest increase being 92.3 mmHg for necrotic lunates, followed

MDCT and radiography of wrist fractures: radiographic sensitivity and fracture patterns.

OBJECTIVE: The purpose of our study was to determine which wrist fractures are not prospectively diagnosed at radiography using CT as a gold standard and to identify specific fracture patterns. MATERIALS AND METHODS: Through a search of radiology records from January 1 to December 31, 2005, 103 consecutive patients were identified as having radiographic and CT examinations of the wrist. After excluding incomplete or nondiagnostic examinations and those with a greater than 6-week interval between imaging studies, the final study group consisted of 61 wrist examinations in 60 patients. Lunate excision, capitate osteotomy, and intercarpal arthrodesis for advanced Kienböck disease. Long-term follow-up.

BACKGROUND: Kienböck disease is caused by aseptic necrosis of the lunate. In the advanced stages of the disease, carpal collapse, joint incongruity, and osteoarthritis develop. We performed lunate excision, capitate osteotomy, and intercarpal arthrodesis (the modified procedure of Graner et al.) on fifteen patients with stage-IIIB or IV Kienböck disease. This report is a review of the findings in these patients. METHODS: The subjects ranged in age from twenty-six to fifty-four years (mean, 39.2 years) at the time of surgery. We evaluated the results more than five years postoperatively (range
