Capsular block syndrome: A case series.

Capsular block syndrome (CBS) or capsular bag distension syndrome has been described after cataract removal with in-the-bag placement of a posterior chamber intraocular lens in the presence of an anterior continuous curvilinear capsulorhexis. Features of CBS include shallowing of the anterior chamber and an unexpected myopic overrefraction; occasionally, there is a persistent uveitis. The patient may be mistakenly diagnosed with pupil block glaucoma or endophthalmitis. We report 9 cases of CBS and their initial diagnoses and management. In 1 case, the capsular bag distention and anterior chamb…

The lens was correctly implanted with the anterior and posterior capsulorhexis edges placed into the interhaptic groove of the BIL IOL in seven of 13 eyes; none of these eyes developed VAO. In the remaining six eyes, the IOL was only fixated to the anterior capsulorhexis edge, and in these cases, an anatomical anomaly of the p …

Management of pediatric subluxated cataract and different methods to face the challenges.

BACKGROUND: Traumatic cataracts present in multiple and different forms. Based on the nature of injury, it necessitates a customized approach for individual case. Capsulorhexis and in the bag intraocular lens (IOL) placement are vital for long term centration of pediatric IOLs. Paediatric lenses having elastic capsule provide more challenges to perform optimum rhexis and the surgery becomes further complicated, when it is subluxated or fibrosed. Here is a pediatric case with such challenges managed using capsular tension ring (CTR) and capsular tension segment (CTS). PURPOSE: To demonstrate t…

Effect of surgical simulation training on the complication rate of resident-performed phacoemulsification.

OBJECTIVE: To study the effect of additional training with ophthalmic surgical simulation on the intraoperative complication rates of phacoemulsification performed by residents. METHODS AND MATERIALS: This was a retrospective study of phacoemulsification surgeries performed by third-year residents at Siriraj Hospital. The operations were classified into two groups according to the experience of the surgeon in simulation training, that is, trained vs untrained. The main outcome was the total rate of complications. Other outcomes, including posterior capsule rupture, anterior capsulorhexis tear…

[Phacoemulsification in white cataract].

PURPOSE: The study presents the technical difficulties of the capsulorhexis and phacoemulsification in the white, intumescent cataract and the incidences occurred during the foldable IOL implantation with the Monarch II injector. METHODS: The clinical study comprises a number of 70 eyes with white intumescent cataract operated during 15 January-15 December 2003 by the same surgeon. The surgical intervention consisted of lens extraction by phacoemulsification with clear corneal incision, using the Monarch II injector for the foldable IOL implantation. In 56 eyes, the anterior capsule was stain…

Piggyback miLOOP-assisted phacoemulsification combined with intraocular lens scaffold in hypermature cataracts.

Hypermature cataracts with liquified cortex pose a higher risk for capsular complications due to a distended capsular bag, floppy posterior … ere is no epinuclear shell. Evacuating the liquefied cortex and prolapsing one nuclear pole through capsulorhexis with ophthalmic viscosurgical devices create enough space to implant an intraocular lens (IOL) into the capsular bag beneath the undivided nucleus. This IOL scaffold shields the floppy posterior capsule and stabilizes the nucleus and zonular fibers …

The soft-shell technique in eyes with a shallow anterior chamber - a randomized trial on the influence of the ophthalmic viscosurgical device.

PURPOSE: Randomized, prospective single-blinded study to examine the properties of different ophthalmic viscosurgical devices (OVDs) regarding their ability to establish and maintain stable intraocular pressure (IOP) while performing continuous curvilinear capsulorhexis (CCC). As smaller anterior chambers (AC) pose a greater challenge in aspects of endothelial protection as well as space maintenance, this study related the results to AC dimensions as anterior chamber volume (ACV) and anterior chamber depth (ACD). MATERIALS AND METHODS: IOP was measured using rebound tonometry with sterilized p…

Effect of Anterior Capsule Polishing on the Capsule-IOL Complex in Patients With Normal Axial Length: A Prospective Randomized Controlled Trial.

PURPOSE: To evaluate the effect of anterior capsule polishing (ACP) on postoperative formation of the capsule-intraocular lens (C-IOL) complex in patients with cataract who have normal axial length. METHODS: In this prospective randomized controlled trial, patients were assigned to either 360° ACP or no polishing. Primary outcomes (capsular bend index [CBI], adhesion index [AI], and continuous curvilinear capsulorhexis diameter [CCCD]) and secondary outcomes (IOL tilt and decentration) were quantitatively assessed at 1 week, 1 month, and 3 months postoperatively using 14-meridian swept-source…

In addition, long AL, thicker lens, and less capsulorhexis-IOL overlap were positively correlated with decentration (P < .001, P = .029, and P = .026). Preventing the Argentinian flag sign and managing anterior capsular tears: A review.

The Argentinian flag sign (AFS) is a feared complication during cataract extraction. Intralenticular pressures, especially excessive posterior pressure, have been identified as potential mechanisms for capsular stress and tearing associated with AFS. Capsular tension is created by positive intralenticular pressures, which cause the irido-lens diaphragm to move anteriorly once the manual capsulorhexis has been initiated. This tension can cause inadvertent tears that self-propagate to the lens equator, causing an AFS, among other intraoperative complications. Thus, this review highlights the imp…

Capsular opacification after cataract surgery.

Posterior-capsule opacification, by far the most common complication of primary cataract surgery, continues to stimulate important work toward understanding its causes, preventing it, and effectively treating it. Of special note here are a report by Koch and Kohnen that a combination of vitrectomy and posterior capsulorhexis is required to inhibit posterior-capsule opacification in pediatric patients; work by Nishi et al. toward the dream of replacing the cataractous lens with a flexible artificial lens, supported by the natural capsular bag; and methods by Tetz et al. and Pande et al. for pre…

"Equator ring" for maintenance of the completely circular contour of the capsular bag equator after cataract removal.

We designed an "equator ring," a flexible silicone rod with a groove on its inner surface, to maintain the circular contour of the capsular bag equator after cataract removal. After cataract extraction, the ring is inserted in the bag through a 5.0-mm circular capsulorhexis or 4.5-mm linear "dumbbell" opening. Then a conventional posterior chamber intraocular lens (PC-IOL) is inserted, with the IOL loops in the groove. The ring not only allows complete maintenance of the circular contour of the capsular bag equator for most conventional PC-IOLs, but also may prevent invasion of metamorphosed l…

Chandelier retroillumination-assisted torsional oscillation for cataract surgery in patients with severe corneal opacity.

We describe a technique that uses 25-gauge transconjunctival chandelier endoillumination in combination with a torsional oscillation system for cataract surgery in cases with severe bullous keratopathy. Because of the hands-free and self-retaining nature of the chandelier fiber, continuous curvilinear capsulorhexis and subsequent bimanual intraocular manipulation can be performed easily. Torsional oscillation efficiently emulsifies lens particles along with the fluidics, preventing posterior capsule rupture and endothelial cell damage. After the chandelier fiber is removed at the end of surger…
