Oesophageal adenocarcinoma: a paradigm of mechanical carcinogenesis?

Incidence of adenocarcinoma of the oesophagus and gastric cardia is increasing in most developed countries and strongly associated with obesity and male gender. An underlying increase in the prevalence of gastro-oesophageal reflux has generally been postulated. We suggest that the increase in frequency of reflux and the 2 associated forms of cancer can be explained by growing abdominal pressure brought about by increasing central obesity, most common among men, and sedentary lifestyle, including car use. Abdominal pressure is further accentuated mainly in men by the shift in Western male dress

Role of Helicobacter pylori in gastric carcinogenesis.

Peptic ulcers and gastric malignancies are the two major complication of the course of Helicobacter pylori-associated chronic gastritis. Both gastric adenocarcinomas and MALT lymphomas occur in association with H. pylori infection, and studies support an etiological association. This article discusses the natural history of H. pylori-related gastric carcinogenesis and criteria to identify people susceptible to H. pylori infection-associated gastric cancer. It then reviews the molecular and genetic mechanisms underlying the malignant transformation of the gastric mucosa associated with H. pylor…

Factors of biliary carcinogenesis in choledochal cysts.

The long-term complications of choledochal cysts include malignant degeneration of the epithelial lining of the biliary tract. Possible factors leading to malignant degeneration were examined by measuring amylase levels in the biliary tract, intraoperative biliary manometry and a histopathological study. 27 patients, 1 to 13 years of age, with choledochal cysts, were studied over a 13-year period. Amylase levels in the bile of choledochal cysts and the gallbladder were measured. The pressure profile of the biliary tract was recorded as the probe was withdrawn from the sphincter of Oddi (SO) to

[Human papillomavirus as a risk factor in carcinogenesis].

BACKGROUND: The causal link between Human papillomavirus (HPV) and cervical neoplasia has been established through clinical, epidemiological … carcinomas and precursor lesions are HPV-positive. HPV is a necessary, but not sufficient factor in carcinogenesis. Several risk factors such as other sexually transmitted diseases, sexual activity, smoking, immunosuppression and diet may be important. High prevalences of HPV-DNA have also been found in carcinomas of other anogenital regions; however, HPV does n …

Toxicology and carcinogenesis studies of pentachlorophenol in rats.

Pentachlorophenol (PCP) has been used as an herbicide, algaecide, defoliant, wood preservative, germicide, fungicide, and molluscicide. A 28-day toxicity study of PCP in F344/N rats of both sexes was conducted to select dose levels for a carcinogenicity study. Groups of 10 male and 10 female rats were given 0, 200, 400, 800, 1600, or 3200 ppm PCP in feed for 28 days. The incidences of minimal to mild hepatocyte degeneration in males and females exposed to 400 ppm or greater and the incidences of centrilobular hepatocyte hypertrophy in the 3200-ppm groups were increased. For carcinogenicity stu

[Epidemiologic considerations on the significance of hormones in carcinogenesis].

The incidence of gynecologic malignancies shows considerable regional differences which suggest a decisive role of environmental and endocrine factors in tumor genesis. The risk of developing breast cancer increases with increasing age, a positive family history, prolonged exposure to estrogens (early menarche, late menopause), nulliparity, alcohol consumption, and obesity. A relationship between a long exposure to estrogens and an increased risk of cancer may also be assumed in the case of endometrial cancer. Whether estrogens or their metabolites promote the initiation of cancer remains to b

Molecular piracy: the viral link to carcinogenesis.

The vast majority of the human experience with viral infections is associated with acute symptoms, such as malaise, fever, chills, rhinitis and diarrhea. With this acute or lytic phase, the immune system mounts a response and eliminates the viral agent while acquiring antibodies to that specific viral subtype. With latent or chronic infections, the viral agent becomes incorporated into the human genome. Viral agents capable of integration into the host's genetic material are particularly dangerous and may commandeer the host's ability to regulate normal cell growth and proliferation. The oncog

Enhancement of biliary carcinogenesis in hamsters by cholecystokinin.

We recently developed a new model for rapid and reproducible induction of biliary carcinoma in hamsters. In the present study, we evaluated the effects of cholecystokinin (CCK), which has a trophic action on the gastrointestinal tract and on the pancreaticobiliary system, on biliary carcinogenesis in this hamster model. Hamsters treated with N-nitrosobis (2-oxopropyl) amine (BOP) were divided into four groups: In Group I, hydrolyzed gelatin, a solvent of CCK, was injected subcutaneously. In Groups II and III, CCK 2.5 and 25 microgram/kg were administered, respectively. In Group IV loxiglumide,…

Phenotypic and genotypic events in gastric carcinogenesis.

Two main histological variants of gastric carcinoma have been identified: intestinal and diffuse types. The former is preceded by a sequential chain of events characterized as chronic gastritis, atrophy, intestinal metaplasia, dysplasia, intramucosal carcinoma, and invasive neoplasia. The second type (diffuse) lacks well-recognized precursor changes. Genotypic events in the gastric precancerous process are described, but a clear model of their sequence and relevance is lacking. Cadherins may play a role in determining which type of carcinoma develops. Translocated promoter region-MET rearrange

p53 expression in the carcinogenesis in the oral mucosa.

Hyperplastic lesions of the oral mucosa such as leukoplakia and oral lichen planus can eventually develop into squamous cell carcinomas. In the clinical treatment of these lesions it would be very important to be able to predict the biological behaviour of an individual lesion. In 64 hyperplastic lesions and 85 squamous cell carcinomas of the oral mucosa, the expression of the mutant tumor suppressor gene p53 was investigated. A positive correlation was seen between the expression of the mutant tumor suppressor gene p53 and the grade of dysplasia of the lesions.

Gastritis and its relation to gastric carcinogenesis.

There have been rapid changes in understanding of chronic gastritis recently, fueled largely by the recognition of gastric infection by Helicobacter pylori. This report reviews the newer classifications of chronic gastritis, and describes the characteristics of H pylori-positive and -negative patterns of gastritis. The relevance of H pylori infection to gastric carcinogenesis is also discussed.

[Experimental study on carcinogenesis in the vagotomized stomach].

The susceptibility to cancer in the vagotomized stomach which was assigned either to selective vagotomy (SV) or selective proximal vagotomy (SPV) was studied by gastric carcinoma model of male Wistar rat on N-methyl-N'-nitro-N-nitrosoguanidine (MNNG) administration. Predilection for cancer of the remnant stomach was also studied. Four groups of the rats submitted to either one of the SV, SPV, antrectomy (Billroth-I reconstruction) or control (simple laparotomy) group, were prepared. The techniques of SV and SPV in rat was originally developed. MNNG in drinking water was given as carcinogen. Ga

Multistage prostate carcinogenesis: the role of hormones.

Prostate cancer is the most frequently occurring non-skin cancer in men in the U.S.A. and other Western countries, but its etiology is poorly understood. Human prostate carcinogenesis has been viewed as a multi-step process involving progression from low histologic grade, small latent carcinoma, to large, higher grade, metastasizing carcinoma. However, recent data suggest that a variety of pathogenetic pathways exist. The precise role of hormones in the genesis of human prostate cancer remains largely undefined. It is difficult to investigate stages in the development of human prostate cancer,…

The effect of hyperthermia on radiation-induced carcinogenesis.

Ten groups of mice were exposed to either a single (30 Gy) or multiple (six fractions of 6 Gy) X-ray doses to the leg. Eight of these groups had the irradiated leg made hyperthermic for 45 min immediately following the X irradiation to temperatures of 37 to 43 degrees C. Eight control groups had their legs made hyperthermic with a single exposure or six exposures to heat as the only treatment. In mice exposed to radiation only, the postexposure subcutaneous temperature was 36.0 +/- 1.1 degrees C. Hyperthermia alone was not carcinogenic. At none of the hyperthermic temperatures was the incidenc

Molecular analysis of chemical carcinogenesis in the skin.

The goal of understanding the molecular basis of human tumour development has been greatly facilitated by the use of animal model systems in which the aetiology of tumour development can be carefully controlled. Environmental chemicals, either naturally occurring or artificially produced, are thought to make a major contribution to the human tumour burden. The process of carcinogenesis can be divided operationally into the stages of initiation, promotion and progression and many different classes of chemical agents can act at one or more of these stages. Many of the concepts of multistage carc…

The role of calcium, magnesium, and zinc in carcinogenesis.

The effects of calcium, magnesium, and zinc supplementation and of magnesium depletion on carcinogenesis are comprehensively reviewed, including epidemiologic and experimental investigations. Some data on the effects of neoplasia on the homeostasis of these metals are also presented. Despite many conflicting results, this review reveals that calcium supplementation is more likely to enhance than inhibit chemical carcinogenesis; magnesium or zinc supplementation tends to inhibit carcinogenesis; magnesium deficiency increases the incidence of neoplasia in humans and animals; parenteral administr…

Modulation of carcinogenesis in the urinary bladder by retinoids.

Bladder cancer has a 70% recurrence rate within five years and a high associated mortality. It commonly occurs in one or both of two predominant growth/behaviour patterns: either well-differentiated, relatively benign exophytic papillary lesions, or flat, poorly differentiated invasive carcinoma usually arising from carcinoma-in-situ. We have used the F344 rat treated with N-butyl-N-(4-hydroxybutyl)nitrosamine (BBN) as a model for the papillary disease, and the BBN-treated B6D2F1 mouse for flat, invasive bladder carcinoma. In the rat, carcinogenesis is a multistage process and several retinoid…
