Maintaining tricuspid valve competence in double discordance: a challenge for the paediatric cardiologist.

OBJECTIVES: To establish the prevalence of tricuspid valve abnormalities in children with a double discordant heart (or congenitally corrected transposition of the great arteries); to study the influence of the loading conditions induced by various surgical interventions on the right and left ventricle in patients with double discordance and an abnormal tricuspid valve; and to propose a rational surgical approach. METHODS: Case notes were reviewed of 141 consecutive patients admitted in the first year of life with various types of double discordance (intact ventricular septum (group 1), ventr

Investigating Medical Radiation Awareness of Saudi Arabian Cardiologists in Catheterization Labs: A Survey Study.

OBJECTIVES: To assess the knowledge, attitudes, and practices of interventional cardiologists in Kingdom of Saudi Arabia (KSA) regarding occupational radiation safety. Occupational radiation exposure poses significant long-term health risks for interventional cardiologists. Understanding current safety knowledge and practices is essential for guiding improvement strategies. METHODS: A multicenter, cross-sectional survey was administered across several cardiac centers in KSA. The survey evaluated radiation safety awareness, use of dosimetry monitoring, and adherence to protective measures amon…

Supply of interventional cardiologists and the provision of lower-value Percutaneous Coronary Interventions (PCI).

BACKGROUND: Percutaneous coronary interventions (PCI) can be lifesaving for patients with acute coronary syndromes but of lower value for patients with stable coronary artery disease (CAD). Previous studies suggest that larger provider supply drives higher healthcare utilization. Whether this is true for lower-value PCI is unknown. OBJECTIVE: To examine the association between the regional supply of PCI-performing physicians and PCI use and lower-value use. DESIGN: Cross-sectional. SETTING: 100% Medicare fee-for-service (FFS) claims and Medicare Advantage encounter data, linked to publicly

Clinical Management Strategies of Cardiologists in Heart Failure with Reduced Ejection Fraction in Türkiye.

OBJECTIVE: Integrating heart failure (HF) guideline recommendations into clinical practice takes time and is often suboptimal in real-life settings. Physician-related factors may be significant barriers to the adoption of these guidelines. This survey aims to assess the current opinions of cardiologists practicing in Türkiye regarding the management of heart failure with reduced ejection fraction (HFrEF). METHODS: The survey comprised 22 questions and was published on the SurveyMonkey platform. RESULTS: A total of 177 cardiologists (mean age: 39.5 years; 73.3% male) participated in the surve…

Aortopulmonary Collaterals in Single Ventricle Physiology: Variation in Understanding Occlusion Practice Among Interventional Cardiologists.

Although aortopulmonary collaterals (APCs) frequently develop in patients with single ventricle palliation, there is a lack of understanding of pathophysiology, natural history, and outcomes with no universal guidelines for management and interventional practice. We conducted a study to assess the views held by interventional congenital cardiologists regarding the hemodynamic impact of APCs in patients with single ventricle palliation, and their embolization practice. An electronic survey using the Pediatric Interventional Cardiology Symposium (PICS) mailing list was conducted between February…

Reduction in exposure of interventional cardiologists to ionising radiation over a 10-year period.

Exposure of operators to ionising radiation in interventional cardiology has likely diminished, but data confirming the magnitude of the reduction are lacking. The aim of this study was to compare the dose of radiation received by interventional cardiology operators at 11 years interval (2006 vs 2017). The study population comprised all interventional coronary procedures performed by a single operator in one catheterization laboratory (cathlab) of a large university hospital in north-eastern France. Exposure was compared between two periods, namely period 1 (from October 2005 to March 2006) an

Intra-cavitary course of right coronary artery: what the cardiologists should be aware of!

We report an unusual case of an anomalous intra-cavitary course of the right coronary artery (RCA) that was detected on multislice computed tomography (CT) angiography. Albeit rare, this anomaly is being picked up with increasing frequency owing to the widespread use of coronary CT angiography (CCTA). Although this entity does not produce symptoms per se, it can result in potentially catastrophic complications during interventional procedures or bypass surgeries if not recognized in time.

Exercise Counseling in Congenital Heart Disease: A Guide for the Pediatric and Congenital Cardiologist.

Exercise is a critical determinant of both immediate and long-term health for individuals with congenital heart disease (CHD). While pediatric cardiologists increasingly recognize the importance of promoting physical activity, formal training in exercise counseling remains limited. Exercise cardiology is an evolving subspecialty, with growing educational and training opportunities for congenital cardiologists to develop diagnostic and therapeutic approaches and expand access to exercise resources for patients with CHD. As part of this effort, the Global Coalition for Fitness and CHD (GloCo) co…

Cardiac amyloidosis red flags: What all the cardiologist have to know.

Cardiac amyloidosis is becoming increasingly important among cardiologist and an early diagnosis is very important. Amyloidosis is a systemic disease and many cardiac and extracardiac elements (red flags) should raise the suspicion of the disease. Electrocardiographic and imaging techniques (such as echocardiography, cardiac magnetic resonance and scintigraphy) are useful tools to make a diagnosis together with the presence of orthopedic issues, peripheral neuropathy or plasma cell dyscrasia. Cardiac amyloidosis is also often associated with valvular disorder, heart failure or cardiomyopathy.

[What a cardiologist needs to know about managing a patient with multiple myeloma].

Multiple myeloma is one of the most frequent hematological malignancies in the elderly and corresponds to the presence of a plasma cell clone. Antitumor treatment combines different therapeutics, including alkylating agents, high-dose corticosteroids, immunomodulators and proteasome inhibitors. These treatments can have cardiovascular side effects that are important to be aware of. The role of the cardiologist is essential in preventing, detecting and managing these effects properly in order to improve the cardiological and oncological prognosis of patients.

Acute coronary syndrome as the first manifestation of essential thrombocythaemia: A challenge for cardiologists.

We present the case is presented of an acute coronary syndrome in a 30-year-old patient with essential thrombocythaemia. Acute coronary syndromes occur in 9% of cases in these patients, and their management constitutes a challenge for the cardiologist, specifically in terms of choosing the most appropriate antiplatelet therapy and its duration, taking into account that these patients have a high thrombotic risk, as well as a considerable haemorrhagic risk.

Practice variability in management of infectious issues in heterotaxy: A survey of pediatric cardiologists.

BACKGROUND: Splenic dysfunction is common in heterotaxy syndrome, and increases the risk of bacteremia and bacteremia related mortality. Despite the risks associated with bacteremia in this setting, best practice guidelines for management of infectious concerns are lacking. We conducted a survey of pediatric cardiologists to characterize practice regarding the diagnosis of splenic dysfunction, approach to antibiotic prophylaxis, and management of possible bacterial infection. METHODS: A 22-item web-based survey was distributed via email to pediatric cardiologists in North America. RESULTS: W…
