Handheld ultrasound in cardiology: Current perspective.

The advantages of ultrasonography do not need to be discussed. It is suitable for use in diverse clinical settings and environments by operators with different backgrounds. Recent technological advances have led not only to the enhancement of the diagnostic capabilities of stationary ultrasound systems but also to miniaturization, which in turn led to the introduction of smartphone-sized handheld ultrasound devices (HUDs), designed to be used at bedside to improve and extend the scope of physical examination. Although diagnostic capabilities of HUDs are expanding, according to guidelines, they

Preventive Medicine in Pediatric Cardiology Practice.

OBJECTIVES: The objective of the study was to assess contemporary practice patterns of pediatric cardiologists with respect to cholesterol disorders and smoking-related illness. STUDY DESIGN: We sent 2 anonymous surveys to the members of the American Academy of Pediatrics Section on Cardiology and Cardiac Surgery and the Pediheart online community. The surveys addressed training in and management of cholesterol disorders and smoking-related illness. RESULTS: There were 97 responses to the cholesterol disorders survey. A total of 51.6% reported little or no formal training on cholesterol diso…

EAPC Core Curriculum for Preventive Cardiology.

Preventive cardiology encompasses the whole spectrum of cardiovascular disease (CVD) prevention, at individual and population level, through all stages of life. This includes promotion of cardiovascular (CV) health, management of individuals at risk of developing CVD, and management of patients with established CVD, through interdisciplinary care in different settings. Preventive cardiology addresses all aspects of CV health in the context of the social determinants of health, including physical activity, exercise, sports, nutrition, weight management, smoking cessation, psychosocial factors a…

Interventional Cardiology for Congenital Heart Disease.

Congenital heart interventions are now replacing surgical palliation and correction in an evolving number of congenital heart defects. Right ventricular outflow tract and ductus arteriosus stenting have demonstrated favorable outcomes compared to surgical systemic to pulmonary artery shunting, and it is likely surgical pulmonary valve replacement will become an uncommon procedure within the next decade, mirroring current practices in the treatment of atrial septal defects. Challenges remain, including the lack of device design focused on smaller infants and the inevitable consequences of somat

Paediatric cardiology: the last 50 years.

In the last half-century, the outlook for children with heart disease has changed dramatically. Morphologists have transformed our understan … . Advances in anaesthesia and intensive care have been integral. New sub-disciplines, such as fetal cardiology and electrophysiology, have emerged. The sum of these numerous developments is enormous. Lesions that were previously considered lethal are now routinely treated. Consequently, the great majority of children with major heart disease now reach adulth …

Advances in cardiology: clinical trial update.

Multiple key cardiology trials have been presented or published over recent months, several with the potential to change clinical practice. In this article, we summarize and place in clinical context new trial findings regarding anticoagulation in the cardiac catheterization laboratory (enoxaparin, fondaparinux and unfractionated heparin), the implications of genetic polymorphisms and functional testing for antiplatelet therapy (clopidogrel and ticagrelor), new oral anticoagulants for use in atrial fibrillation (apixiban and rivaroxaban), optimal pacing strategies and pharmacological agents in…

Managing patient dose in interventional cardiology.

This paper introduces a methodology for managing the radiation delivered to individual patients, incidental to their fluoroscopically guided interventional procedures. Because rare, but devastating skin injuries have occurred, radiation should be monitored and managed as well as drugs or radiographic contrast media. Dose metrics such as the total dose delivered to a reference point (reference point dose) should be used for this purpose. Fluoroscopy time is a poor independent predictor of skin dose in an individual patient. The concept of a "significant dose" is introduced. This is a predetermi

Monitoring device safety in interventional cardiology.

OBJECTIVE: A variety of postmarketing surveillance strategies to monitor the safety of medical devices have been supported by the U.S. Food and Drug Administration, but there are few systems to automate surveillance. Our objective was to develop a system to perform real-time monitoring of safety data using a variety of process control techniques. DESIGN: The Web-based Data Extraction and Longitudinal Time Analysis (DELTA) system imports clinical data in real-time from an electronic database and generates alerts for potentially unsafe devices or procedures. The statistical techniques used are

New frontiers in molecular pediatric cardiology.

Numerous advances in understanding the molecular basis of congenital heart disease have been published in the past year. Highlights are reviewed, focusing on two major topics: genetic syndromes and cardiac organogenesis. Genetic syndromes are discussed in the context of complementary data from targeted mutations in animals and genetic mapping studies in humans. These include the DiGeorge, Holt-Oram, Alagille, familial primary pulmonary hypertension, and Noonan syndromes. Novel concepts in cardiac organogenesis are discussed, including the existence and contribution of an anterior heart field t

Anaesthetic experience with paediatric interventional cardiology.

Anaesthetic and sedation techniques, complications and outcomes were reviewed in 176 children undergoing 184 interventional cardiologic procedures. Techniques included sedation only, and ketamine, inhalational or narcotic anaesthesia. Ketamine infusion was the technique most frequently used. Ketamine was associated with a higher incidence of respiratory complications (P less than 0.05) than the other techniques. The higher incidence of hypercarbia (15.6 per cent), which did not affect outcome, may be attributable to the use of supplemental sedatives. The incidence of upper airway obstruction (

Positron emission tomography in clinical cardiology.

Positron emission tomography of the heart is a physiologic imaging technique that enables both qualitative and quantitative assessment of regional myocardial blood flow and substrate utilization. Exercise or dipyridamole perfusion imaging with PET is both sensitive and specific for detecting coronary disease and may prove clinically useful in assessing the physiologic significance of anatomically defined stenoses and for noninvasively following stenosis progression or regression. PET can be used to localize and quantitate the extent of antecedent myocardial infarction, and frequently identifie

[Magnetic resonance tomography in pediatric cardiology].

Magnetic resonance imaging (MR) is a relatively new non-invasive imaging method that has not been used as a routine procedure in pediatric cardiology. To evaluate MR in pediatric cardiology we performed 110 examinations in 91 children during the period from December, 1984, to November, 1987. The children were divided into the following groups: 1) Diseases of the great vessels (54 examinations in 39 children); 2) Cardiac diseases (42 examinations in 39 children); 3) Diseases of the pericardium (four examinations in three children); 4) Ten children without cardiovascular diseases (imaging of the…
