Using legal epidemiological and social determinants of health frameworks, this study examines how racial and spatial inequalities shape cardiometabolic health outcomes in Greenville County, South Carolina. We used Census tract-level data from the CDC PLACES project, and the U.S. Census Bureau to assess associations between racial/ethnic composition and cardiometabolic indicators, including coronary heart disease, hypertension, and obesity. Our regression-based mediation analysis examined whether socioeconomic factors (household income and education attainment) explained these associations. Tra

OBJECTIVE: Evaluate associations of neighborhood social capital and mobility of older adults.
METHOD: A community-based survey (Philadelphia, 2010) assessed mobility (Life-Space Assessment [LSA]; range = 0-104) of older adults (n = 675, census tracts = 256). Social capital was assessed for all adults interviewed from 2002-2010 (n = 13,822, census tracts = 374). Generalized estimating equations adjusted for individual- and neighborhood-level characteristics estimated mean differences and 95% confidence intervals (CIs) in mobility by social capital tertiles. OBJECTIVES: This study examines whether the Social Vulnerability Index (SVI), a location-based composite measure of social vulnerability, is associated with the risk of SARS-CoV-2 or influenza infection within households. STUDY DESIGN: Prospective cohort case-ascertained household transmission study. METHODS: We analyzed data from a case-ascertained household transmission study conducted across multiple U.S. sites (September 2021–May 2023). Household contacts of index cases with confirmed infections self-collected nasal swabs daily for ten days, tested via RT-PCR for SARS-CoV-2 or influenza. A

BACKGROUND: The concentration of under-5 child morbidity and mortality due to pneumonia in developing countries reflects the social inequities. This study aimed to map and assess the spatial risk for hospitalization due to Community-Acquired Pneumonia in children under 5 years of age and its association with vulnerable areas.
METHODS: Ecological study in the city of Ribeirão Preto, state of São Paulo, Brazil. The study population consisted of hospitalized under-5 children, diagnosed with community-acquired pneumonia, in Ribeirão Preto-São Paulo-Brazil, from 2012 to 2013. Data were collected i

BACKGROUND: Little is known about the role of wildfire smoke PM2.5 in cancer burden, despite ambient PM2.5 being classified as a human carcinogen.
METHODS: Daily wildfire smoke PM2.5 estimates were aggregated into cumulative (2006-2020) and recent (2018-2020) averages, and linked to 2022 tract-level cancer history prevalence estimates for all tracts in the contiguous US. Quasi-Poisson generalized additive models with random effects and spatial splines estimated prevalence ratios and 95% CIs for each exposure window, modeling smoke PM2.5 continuously (per IQR) and across tertiles. In Denver area census tracts during the years 2016-2019, the rate of carbapenem-resistant enterobacterales (CRE) infection increased by 10% for every 0.1 unit increase in Social Vulnerability Index (RR 1.10, 95% CI 1.06, 1.14). This finding suggests that community factors may influence risk of healthcare-associated infections such as CRE.

OBJECTIVES: To determine the association between neighborhood domestic violence and small-for-gestational-age (SGA) birth and to examine if there is a differential impact of neighborhood domestic violence on SGA births by race in a high crime community.
METHODS: This analysis includes all birth records issued in New Orleans, Louisiana from 2011 to 2012 geocoded by census tract (N=177 census tracts, N=8322 women). Hierarchical modeling and ecologic spatial analysis were used to examine the area-effect of neighborhood domestic violence on SGA births, independent of individual-level predictors a

Childhood lead exposure causes irreversible neurodevelopmental harm, and no safe blood lead level in children has been identified. Measured childhood blood lead is collected unevenly across the United States, and many areas publish no neighborhood-level surveillance, so the children at highest risk are often the hardest to find by measurement. Building on the U.S. Environmental Protection Agency finding that housing age and poverty predict where childhood lead exposure concentrates, we computed a lead-exposure risk score for all 83,388 U.S. census tracts and 3,222 counties from public American

INTRODUCTION: Texas has the highest rates of hepatocellular carcinoma (HCC) in the United States. Exposure to toxicants may play a role in liver disease. Several mechanisms of arsenic carcinogenesis have been proposed, however, the evidence in human populations is limited to associations between HCC and ingestion of arsenic-contaminated drinking water. Through an ecological study, we examined associations between ambient arsenic and HCC incidence rates.
METHODS: Primary outcome was HCC incidence rates based on Texas Cancer Registry (TCR) data. Primary exposure of interest was the simulated ce

OBJECTIVES: To assess the relationship between neighborhood effects and the severity of dental caries among low-income African-Americans.
METHODS: A multistage probability sample of African-American families living in the poorest 39 census tracts in Detroit was drawn. During 2002-03, cross-sectional data of a cohort that includes 1021 caregivers were collected in the first of three waves of interviews and examinations. Multilevel analyses focused on 27 neighborhood clusters and involved a combination of individual (Level-1) and neighborhood (Level-2) data including census and geocoded (addres

IMPORTANCE: In the United States, Black and Hispanic children have higher rates of asthma and asthma-related morbidity compared with White children and disproportionately reside in communities with economic deprivation.
OBJECTIVE: To determine the extent to which neighborhood-level socioeconomic indicators explain racial and ethnic disparities in childhood wheezing and asthma.
DESIGN, SETTING, AND PARTICIPANTS: The study population comprised children in birth cohorts located throughout the United States that are part of the Children's Respiratory and Environmental Workgroup consortium. Cox p

To a great extent, research on geographic accessibility to mammography facilities has focused on urban-rural differences. Spatial accessibility within urban areas can nonetheless pose a challenge, especially for minorities and low-income urban residents who are more likely to depend on public transportation. To examine spatial and temporal accessibility to mammography facilities in the Atlanta metropolitan area by public and private transportation, we built a multimodal transportation network model including bus and rail routes, bus and rail stops, transfers, walk times, and wait times. BACKGROUND: African-Americans are more likely than Caucasians to be diagnosed at an advanced stage of colorectal, lung, breast, cervical, and prostate cancers. This study explores if racial differences in stage at diagnosis can be explained by socioeconomic status (SES) differences. Previous studies investigating this association have used aggregate SES indicators from census tract of residence; we used census block-group data, representing a smaller, potentially more homogenous group.
METHODS: We included all African-American and Caucasian invasive cancers of the colon and rectum, lung and b

BACKGROUND: Industrial facilities are not located uniformly across the United States (U.S.), and little is known about the quantity of suspected human carcinogens emitted from these sources on a nationwide scale. We evaluated differences in potential exposure to these agents among sociodemographic groups in the U.S. and Puerto Rico.
METHODS: We used a U.S. regulatory database to identify emissions (pounds) of 32 probable human carcinogens as classified by the International Agency for Research on Cancer. We linked the 2010-2018 average emissions for each agent to 2010 Census tract boundaries a

The Centers for Disease Control and Prevention (CDC)/Agency for Toxic Substances and Disease Registry (ATSDR) Social Vulnerability Index (SVI) is a census-based metric that includes 15 socioeconomic and demographic factors split into four themes relevant to disaster planning, response, and recovery. Using CDC/ATSDR SVI, health outcomes, and remote sensing data, we sought to understand the differences in the occurrence of overall and cause-specific emergency department (ED) visits before and after a 2017 flood event in Texas following Hurricane Harvey, modified by different levels of social vul

Using census and administrative agency data for 177 urban census tracts, variation in rates of officially reported child maltreatment is found to be related to structural determinants of community social organization: economic and family resources, residential instability, household and age structure, and geographic proximity of neighborhoods to concentrated poverty. Furthermore, child maltreatment rates are found to be intercorrelated with other indicators of the breakdown of community social control and organization. These other indicators are similarly affected by the structural dimensions

This study explores flood-related environmental injustices by deconstructing racial, ethnic, and socio-demographic disparities and spatial heterogeneity in the areal extent of fluvial, pluvial, and coastal flooding across Canada. The study integrates JBA Risk Management's 100-year Canada Flood Map with the 2016 national census-based socioeconomic data to investigate whether traditionally recognized vulnerable groups and communities are exposed inequitably to inland (e.g., fluvial and pluvial) and coastal flood hazards. Social vulnerability was represented by neighbourhood-level socioeconomic d

In a study of the interrelationships among 12 separate indices of social pathology, including drug abuse, in which census tracts were the units of analysis, moderate to high intercorrelations among all indices were observed. A subsequent components-types factor analysis revealed only a single eigenvalue greater than unity, suggesting that the interrelationships among the several indices may be most parsimoniously explained in terms of a single underlying construct-generalized social pathology. Exact factor scores on this construct were computed for each tract, and those may be viewed as estima

We evaluated the association between socioeconomic status (SES) and community-associated Clostridioides difficile infection (CA-CDI) incidence across 2474 census tracts in 10 states. Highly correlated community-level SES variables were transformed into distinct factors using factor analysis. We found low SES communities were associated with higher CA-CDI incidence.

BACKGROUND: Insomnia is the most commonly reported sleep disorder, characterized by trouble falling asleep, staying asleep, or waking up too early. Previous epidemiological data reveal that women are more likely than men to suffer from insomnia symptoms. We investigate the role that mental health history plays in explaining the gender disparity in insomnia symptoms.
METHODS: Using logistic regression, we analyze National Health and Nutritional Examination Survey (NHANES) III interview and laboratory data, merged with data on sociodemographic characteristics of the residential census tract of
