The effect of weight reduction on biliary lipid metabolism in patients with hyperlipoproteinemia type IV.

Nine slightly obese patients with hyperlipoproteinemia type IV were studied, before and after a mean weight reduction of about 15 kg, with respect to bile acid kinetics, cholesterol balance and biliary lipid composition. The bile acid pool size was not consistently changed. The synthesis of cholic acid and chenodeoxycholic acid was decreased by about 65% and 50%, respectively. The net steroid balance, calculated as bile acid synthesis plus fecal excretion of neutral steroids minus dietary cholesterol intake, was reduced by about 50%. In all but one of the patients bile was supersaturated with

Bile acid analysis in human disorders of bile acid biosynthesis.

Bile acids facilitate the absorption of lipids in the gut, but are also needed to maintain cholesterol homeostasis, induce bile flow, excrete toxic substances and regulate energy metabolism by acting as signaling molecules. Bile acid biosynthesis is a complex process distributed across many cellular organelles and requires at least 17 enzymes in addition to different metabolite transport proteins to synthesize the two primary bile acids, cholic acid and chenodeoxycholic acid. Disorders of bile acid synthesis can present from the neonatal period to adulthood and have very diverse clinical sympt

Tauroursodeoxycholate prevents taurolithocholate-induced cholestasis and toxicity in rat liver.

Ursodeoxycholate has been advocated for the treatment of cholestatic liver diseases. The coinfusion of tauroursodeoxycholate with taurolithocholate in the perfused rat liver completely prevented the decrease of bile flow and the increase of oxygen uptake found with taurolithocholate only. Bile flow and bile salt secretion were increased with the coinfusion of both bile acids as compared with the infusion of tauroursodeoxycholate only (+4.30 microliters/g liver per 30 min) with 16 and 32 mumol/l tauroursodeoxycholate (+1.55 microliters/g liver per 30 min with 80 and 160 mumol/l). Morphological

Recent Advances in Primary Biliary Cholangitis Treatment.

Primary biliary cholangitis (PBC) is a rare autoimmune liver disease that leads to chronic cholestasis and progressive liver dysfunction. It is often accompanied by extrahepatic symptoms such as pruritus and fatigue, which significantly impair the quality of life. Current treatment options include ursodeoxycholic acid, the standard first-line therapy, along with second-line agents like obeticholic acid, and recently approved seladelpar, and elafibranor. These treatments aim to alleviate symptoms, improve liver function, and slow disease progression. This article focuses on recently approved th

Cholic acid as a treatment for cerebrotendinous xanthomatosis: a comprehensive review of safety and efficacy.

Cerebrotendinous xanthomatosis (CTX) is a rare treatable bile acid disorder caused by homozygous or compound heterozygous variants in CYP27A, a gene that encodes the mitochondrial enzyme sterol 27-hydroxylase (CYP27A1). CYP27A1 facilitates the production of both cholic acid (CA) and chenodeoxycholic acid (CDCA). Deficiencies in CYP27A1 limit the production of both CA and CDCA, leading to multisystemic cholestanol deposition in membranes, including those of neurons, smooth muscle cells, tendons, and the eye. Because of increased concentrations of cholestanol, a byproduct of cholesterol metaboli

The gut microbiome predicts response to UDCA/CDCA treatment in gallstone patients: comparison of responders and non-responders.

The treatment of gallbladder (GB) stones depends on condition severity. Ursodeoxycholic acid (UDCA) and chenodeoxycholic acid (CDCA) are commonly used to treat GB stones, but the factors affecting response rates have not been fully identified. Therefore, we investigated the relationship between response to UDCA/CDCA treatment and changes in the gut microbiomes of patients with GB stones with the intention of identifying gut microbiomes that predict susceptibility to UDCA/CDCA treatment and treatment response. In this preliminary, prospective study, 13 patients with GB stones were treated with

Long-Term Obeticholic Acid Therapy Improves Histological Endpoints in Patients With Primary Biliary Cholangitis.

BACKGROUND & AIMS: Primary biliary cholangitis (PBC) is an autoimmune disease characterized by bile duct destruction that can progress to cirrhosis. A liver biopsy substudy was conducted in the PBC obeticholic acid (OCA) International Study of Efficacy (POISE) to determine the long-term effects of OCA on liver damage and fibrosis in patients with PBC. POISE is a phase 3, double-blind, placebo-controlled, randomized trial with a 5-year open-label extension that evaluated 5 to 10 mg OCA daily in patients who were intolerant or unresponsive to ursodeoxycholic acid. METHODS: Liver biopsy specimen

Non-surgical options for the management of gallstone disease: an overview.

The modalities for the non-surgical treatment of gallstones include oral dissolution by bile salts, local dissolution by methyl-tert-butyl-ether (MTBE), extracorporeal shockwave lithotripsy (ESWL) and percutaneous gallstone clearance. The results of oral bile salt therapy for cholesterol stones have been disappointing, and the only indication for this treatment is after ESWL. The high efficacy initially reported for MTBE has not been confirmed by subsequent experience in other centres: this therapy is toxic and best confined to specialized centres. ESWL, though effective in noncalcified stones

Dissolution of human cholesterol gallstones in vitro with bile salts. A scanning and stereoscanning electron microscopic study.

This report deals with the in vitro dissolution of cholesterol gallstones by bile salts, as observed with the scanning electron microscope. Scanning electron microscopic techniques permit excellent visualization of the substructure of these stones and hence allow insight into the structural basis of gallstone dissolution. The small faceted, mixed cholesterol gallstone was used since this is the most common type clinically. Scanning electron microscopy revealed that the gallstone has four structurally different zones; an outer surface, a cortex of compactly layered cholesterol plates, an inner

[Treatment of cholestatic liver diseases].

MODE OF ACTION OF URSODEOXYCHOLIC ACID: In cholestasis, the impaired biliary secretion of toxic bile acids causes their accumulation in the liver and a subsequent hepatic failure. This bile acid-induced liver damage can be treated successfully with the atoxic ursodeoxycholic acid (UDCA). This bile acid is formed by 7-beta-epimerization of chenodeoxycholic acid by intestinal bacteria. It represents 1-3% of the total human bile acid pool. The hepatocellular content of toxic bile acids is reduced by UDCA. This protects the liver cell from injury by toxic bile acids. THERAPEUTIC RESULTS: The thera

Serum and urinary bile acids in patients with primary biliary cirrhosis.

Serum concentrations and daily urinary excretions of unsulphated and total cholic (C) and chenodeoxycholic (CDC) acid were determined by radioimmunoassay in 15 patients with primary biliary cirrhosis. Thirteen patients had increased fasting serum bile acid concentrations; two of them had an increase of C only. An increase of C and CDC in serum was always followed by an increased urinary excretion of C and CDC. The individual serum bile acids were separated by gas-liquid chromatography. 3 beta-hydroxy-5-cholenoic acid was increased in nine patients, and its serum concentration was correlated to

Ursodeoxycholic acid corrects defective natural killer activity by inhibiting prostaglandin E2 production in primary biliary cirrhosis.

We evaluated the effect of ursodeoxycholic acid on the defective natural killer activity in primary biliary cirrhosis. Administration of ursodeoxycholic acid (600 mg daily) for one month significantly increased natural killer activity in patients with primary biliary cirrhosis (P < 0.05). Ursodeoxycholic acid also enhanced the in vitro natural killer activity of lymphocytes from healthy volunteers, while other hydrophobic bile acids depressed it. Furthermore, ursodeoxycholic acid reduced the prostaglandin E2 concentration in culture supernatants of lymphocytes from healthy volunteers to a lowe

The effect of phenobarbital on biliary lipid metabolism and hepatic microsomal drug metabolism in patients with cholesterol cholelithiasis.

Six patients with cholesterol cholelithiasis were studied before and after 3 weeks treatment with phenobarbital, 120 mg/day. The kinetics of cholic and chenodeoxycholic acid and biliary lipid composition were not significantly affected by phenobarbital treatment although hepatic mixed-function oxidase was stimulated by the treatment as shown by increased breath output of 14CO2 after administration of (14C)aminopyrine. It is concluded that treatment with phenobarbital is unlikely to be effective therapy for patients with cholesterol cholelithiasis.

[Physiology of bile secretion].

Canalicular bile secreted by the hepatocyte consists of at least two fractions: one is initiated by active transport of bile acids, the other is due to active transport of inorganic ions. Bilirubin and other organic anions, organic cations, and neutral compounds are secreted into the canalicular bile as well as lecithin and cholesterol. Subsequently the biliary tree, the gallbladder, and the enterohepatic circulation of bile acids have an effect on bile formation. Choleresis induced by drugs may be due to a direct osmotic effect of the compound secreted in bile or to an increase of the bile ac

Genetically and clinically confirmed atypical cerebrotendinous xanthomatosis with normal cholestanol and marked elevations of bile acid precursors and bile alcohols.

BACKGROUND: Cerebrotendinous xanthomatosis (CTX) is a rare autosomal recessive lipid disorder. Affected patients often remain undiagnosed until the age of 20-30 years, when they have already developed significant neurologic disease that may not be reversible. An elevated plasma cholestanol concentration has been accepted as a diagnostic criterion for CTX for decades. OBJECTIVE: Full biochemical characterization was performed for three genetically and clinically confirmed atypical CTX cases with normal plasma cholestanol levels. METHODS: Clinical assessment and genetic/biochemical testing for
