Fifty-seven patients, twenty-six males and thirty-one females, aged 6 to 50 years (mean 12.6 years) with proven Salmonella typhi or S. paratyphi A septicaemia, were treated in an open randomized parallel study with either aztreonam or chloramphenicol. Aztreonam was given intramuscularly at a level of 50 to 80 mg/kg body weight per dose every 8 h for 7 days to thirty patients. We prospectively studied the pharmacokinetics of intravenous Chloramphenicol succinate (CS) in children (age 6 months-14 years) with culture proven typhoid fever (n = 30) and non typhoidal illnesses (n = 10). CS was administered in three different dosage regimens (50, 75 and 100 mg/kg/d-q 6 hourly). Liver function tests were monitored. Clinical decision analysis is a technique which is used to deal with uncertainities in clinical medicine. We used this technique to compare the cost-effectiveness of chloramphenicol and ciprofloxacin in the treatment of enteric fever. The study shows that the most economical alternative can be predicted if the cost of hospital stay and the sensitivity of the organism to chloramphenicol are known. Chloramphenicol was used to treat confirmed typhoid fever in 50 patients, but 14 patients did not respond to treatment. When their treatment was changed to ofloxacin (400 mg twice daily for six days, followed by 200 mg for eight days, administered orally), all responded. Ofloxacin was well tolerated and appears to be an effective agent in the treatment of typhoid fever.

The authors have conducted an open randomized study to compare the clinical efficacy and safety of cefoperazone with those of chloramphenicol in the treatment of typhoid fever. Of 2356 strains of Salmonella typhi isolated in Britain in the 8-year period 1978-85, 2345 (99.53%) were sensitive to all antibiotics tested and 11 (0.47%) were chloramphenicol-resistant; chloramphenicol resistance was plasmid-mediated in 6 strains. It is concluded that chloramphenicol remains a satisfactory first-line choice of drug for typhoid fever in Britain.

Ototoxic interaction between chloramphenicol and noise was studied in two separate investigations. In the first study, permanent ototoxicity was demonstrated in a group of rats which were subjected to short-duration, high-intensity noise and were then given chloramphenicol orally. We recently found that bilirubin interferes with the colorimetric chloramphenicol assay of Kakemi et al. (K. T. Kakemi, T. Arita, and S. Ohashi, Yakugaku Zasshi 82:342-345, 1962). Levels of serum bilirubin alone (4 to 6 mg/dl) resulted in apparent concentrations of chloramphenicol which appear to be in the therapeutic range. Concentrations of serum bilirubin greater than 8 mg/dl resulted in levels of apparent chloramphenicol associated with toxicity (>50 mug/ml). An open trial was carried out on 63 patients in London to assess the efficacy of 1% rifampicin eye ointment in comparison with 1% chloramphenicol eye ointment in the treatment of sexually transmitted TRIC infection of the eye. Patients included were selected on the basis of positive cultures for Chlamydia trachomatis.
