Four cases of choroid plexus papilloma extending through the foramen of Monro are presented. All four patients were evaluated by contrast-enhanced CT and two of these were also evaluated by neurosonography. No other intraventricular tumor commonly extends through a ventricular outlet. The radiographic demonstration of this extension of an intraventricular mass through the foramen of Monro, cerebral aqueduct, or foramen of Luschka or Magendie represents an ancillary diagnostic sign of choroid plexus papilloma.

In order to test the hypothesis that overproduction of cerebrospinal fluid can cause the hydrocephalus seen in choroid plexus papillomas, adult mongrel dogs with and without hydrocephalus were subjected to high pressure intraventricular infusions of artificial cerebrospinal fluid (CSF). Intermittent cerebrospinal fluid rhinorrhea occurred in a 34-year-old male with a papilloma of the choroid plexus in the fourth ventricle. Surgical removal of the tumor preceded plastic repair of the frontonasal fistula. I report an observation 28 years after operation.

The ultrastructural findings of two cases of choroid plexus carcinoma occurring in infants 11 and 24 months old are discussed. Both had a similar ultrastructural pattern which demonstrated a clear polygonal cytoplasm, cell polarity towards the lumina, junctional complexes and evidence of a basement membrane, all of which point to an epithelial origin for the proliferating cells. Choroid plexus papillomas are rarely located in the third ventricle. The case of a 49-year-old man who had a total microsurgical excision of such a lesion is reported. The neuroradiological features and the surgical results are discussed, with a review of 25 other cases reported in the literature. Microsurgical technique made possible a good exposure and successful complete removal of the tumor.

Choroid plexus papilloma is a rare intracerebral neoplasm that occasionally disseminates along the neuraxis. The fine structure of such a metastatic papilloma appears to correspond very closely to that of normal choroid plexus. Lack of capillary endothelial pores is the only meaningful difference. There does not appear to be an ultrastructural feature that is unique to disseminating choroid plexus papilloma.

In a 3-year-old boy disseminated histoplasmosis was recognized from a liver biopsy. The patient died shortly afterwards and had foci of the disease in liver, spleen, and kidneys. Large numbers of yeast cells of Histoplasma capsulatum were found within histiocytes in other organs including the eye. This rare finding is remarkable in view of the search for Histoplasma capsulatum in the syndrome known as "so-called histoplasmosis of the choroid."
