Between January 1983 and May 1987, 255 esophagectomies were performed for carcinoma of the middle (40 patients) or lower (215 patients) esophagus. All patients were operated upon through a left thoracolaparotomy and underwent a radical en bloc resection of the tumor along with all palpable mediastinal nodes. Ten patients had chylothorax develop postoperatively. There were seven men and three women with a mean age of 65.7 years (range of 37.0 to 81.0 years). Parameters that were statistically evaluated for possible correlation to increased incidence of chylous fistula were age, sex, site and si…

A young patient who had traumatic amputation of left arm developed extensive chylothorax on right side of chest. The pathophysiology appears to be left subclavian thrombosis secondary to gross sepsis of the amputation stump which resulted in occlusion of thoracic duct opening and chylothorax. The child was successfully treated by closed chest tube drainage.

Authors report five consecutive cases of spontaneous neonatal chylothorax (SNC). Three patients had also Down's syndrome, one a VSD and the remaining one was a premature. Two infants with bilateral pleural effusions started with severe neonatal asphyxia in absence of previous fetal alteration. Two right-side chylothoraces made a satisfactory recovery. Fith patient died a few hours after birth because of prematurity. Special mention is made of difficulties for an accurate etiologic diagnostic in the majority of SNC. Interest of association with Down's syndrome, of which only two cases have been…

Bilateral lethal chylothoraces developed in a premature infant secondary to superior vena caval obstruction related to central venous catheterization for total parenteral nutrition. The accumulation of chyle in the lungs was preceded radiographically by the typical lymphatic pattern in the lungs, usually associated with lymphangiectasia. A nuclear medicine lymphangiogram showed abnormal accumulation of the isotope in the lungs secondary to the lymphatic obstruction.

BACKGROUND: Chylothorax is a medical condition characterised by the abnormal accumulation of lymphatic fluid in the pleural cavity. In its congenital form, chylothorax develops as a result of an idiopathic abnormality in the thoracic duct. Congenital chylothorax (CCT) is the leading cause of pleural effusion in infants, a condition where excess fluid builds up in the space between the lungs and the chest wall. This scoping review includes a wide range of published studies from 1980 to January 2024, obtained from multiple reputable databases, including Google Scholar, PubMed, Springer and BioMe…

A 74-year-old man presented with leg edema and was found to have nephrotic-range proteinuria. Renal biopsy revealed AL amyloidosis. Subsequent bone marrow examination demonstrated an increase in monoclonal plasma cells accounting for 13% of nucleated cells. Right-sided pleural effusion developed and was confirmed to be chylothorax. Although treatment with daratumumab, lenalidomide, and dexamethasone (DLd regimen) was initiated, the pleural effusion continued to increase. Lymphangiography using ethiodized oil identified a leakage point in the anterior mediastinum. A chest drain was inserted on …

We report a case of chylothorax secondary to severe mitral regurgitation, in the absence of heart failure with a low ejection fraction. A 49-year-old male presented with mitral valve infective endocarditis, complicated by severe mitral regurgitation with persistent vegetations 5 weeks into treatment. He declined valvular repair. He was admitted four times in the following year for symptomatic right pleural effusion requiring therapeutic chest drainage. Pleural fluid was milky, with elevated triglyceride levels consistent with chylothorax. He underwent mitral valve replacement, with no recurren…

Chylothorax is a rare condition characterized by the accumulation of chyle in the pleural space. While it accounts for a small percentage of pleural effusions, chylothorax can lead to significant morbidity and mortality. This article provides a comprehensive overview of chylothorax, covering its relevant anatomy, aetiology, pathophysiology, clinical features, diagnosis, and management. Injury or disruption to the thoracic duct (which is responsible for chyle transport) leads to the development of chylothorax. This may result from trauma, such as iatrogenic injury during surgery, or non-traumat…

Chylothorax is a known complication of postcardiac surgery and the most common cause of pleural effusion in neonates. Conservative management is usually adopted, including Nil-per-Oral (NPO), treatment of underlying etiology of infection, and use of octreotide. Chylothorax resistant to medical therapy and drainage is often treated by chemical pleurodesis. Previously used pleurodesis agents have included talc, minocycline, OK-432, bleomycin, and povidone-iodine. 50% Dextrose (D50) has been reported to be useful for pleurodesis in adults. We successfully managed two cases of prematurely born inf…

An 8-year-old male neutered American English Coonhound was presented for a 2-day history of increased respiratory effort and rate with an oc … cavity. The mass was diagnosed as a lipoma and its removal led to rapid and complete resolution of chylothorax. Based on the literature search, this is the first case report of chylothorax secondary to a cervical mass or subcutaneous lipoma.

Idiopathic postpartum chylothorax is an uncommon finding, with only four cases described in the literature. We present the case of a 37-year-old female who was diagnosed with chylothorax three days after the delivery of her baby. Chylothorax was managed with chest tube placement, a low-fat diet, and octreotide. As opposed to the current literature, her chylothorax resolved with medical management and chest tube insertion without further surgical intervention. The chest tube was removed 11 days after chest tube placement, and she was discharged in stable condition. We also review the most recen…

There are currently limited published case reports and clinical studies looking at octreotide as a potential therapeutic agent for treating surgery- and malignancy-related chylothorax in adult patients. Few case reports have shown that low-dose subcutaneous octreotide can be used to treat malignant chylothorax. We report the case of a 57-year-old high-grade follicular lymphoma patient with malignant chylothorax which responded rapidly and was successfully treated with octreotide. Significant improvements were noted in her dyspnea, abdominal distention and pain, and chylous output. This case al…

Chylothorax or collection of lymphatic fluid in the pleural space is an exceedingly rare cause of pleural effusion and respiratory distress in neonates. In adults, prompt diagnosis and effective treatment regimen can aid in the resolution of chylothorax; however, in neonates treating the condition can be an onerous challenge for the pediatricians due to the rarity and perplexing clinical presentation. We elucidate a case of spontaneous chylothorax in a 15-day-old neonate who presented to us with respiratory distress, chest indrawing, and cyanosis for nine days. The prenatal and birth history o…

Chylothorax is an exceedingly rare but serious complication of orthotopic heart transplantation (OHT). Prompt diagnosis and appropriate management are essential for a good outcome. Management is similar to that of nontransplant patients, but special attention must be given to patients' nutritional and immunological status. Relevant literature on this topic is limited. We describe our experience in the management of chylothorax after OHT and provide a summary of reported cases of this complication after isolated heart and combined heart/lung transplant.

BACKGROUND: Although chylothorax is rare in patients with malignant lymphoma, it has been reported in cases with mediastinal lymphadenopathy, a bulky retroperitoneal mass, or thoracic ducts blocked by lymphocytes in chronic lymphocytic leukemia (CLL). The clinical picture is not specific. Patients become more immunocompromised and malnourished after repeated evacuations. Chylothorax is usually diagnosed when the triglyceride level is > 1.24 mmol/l and the cholesterol level is < 5.18 mmol/ l in the effusion. Chemotherapy or radiotherapy of the associated malignant tumors has been used as the fi…
