INTRODUCTION: Implant-related discomfort (soft-tissue irritation, pain and cosmetics) is often seen in patients with surgically treated clavicle fracture. The aim of this study was to investigate indications, complications, functionality and patient satisfaction following implant removal of surgically treated clavicle fractures.
METHODS: A total of 97 patients (73 males, mean age 43 years) had a clavicle plate removed at our department between 2007 and 2014. A purpose-made questionnaire was used to assess self-perception of cause of implant removal, remission, complications and overall satisf

Birth injuries are frequently seen in newborn infants. Clavicular fractures are the most commonly encountered bony injuries seen in clinical practice. The incidence of clavicle fracture ranges from 0.35 per cent to 2.9 per cent of births and remains undetected at the time of discharge from hospital in up to 40 per cent of cases. Clinical suspicion of fractured clavicle may be raised from history (shoulder dystocia) or clinical examination (spongy feeling or crepitus on palpation). This should be confirmed with imaging studies. Parents should have the diagnosis explained and be reassured that h

BACKGROUND AND PURPOSE: Traditionally, clavicle fractures have been treated nonoperatively. However, many recent studies have concentrated on the results of operative treatment. We assessed and compared the outcomes of operative and nonoperative treatment for acute clavicle fractures in adults.
METHODS: We performed a systematic search of the medical literature from 1966 until the end of March 2011. We included randomized controlled trials and controlled clinical trials comparing operative and nonoperative treatment and studies comparing different operative and nonoperative treatments. We req

Excision of the distal clavicle (DCE) is a commonly carried out surgical procedure used in the management of acromioclavicular joint pathology. Although successful outcomes after both open and arthroscopic distal clavicle excision occur in a high percentage of patients, treatment failures have been reported, creating a difficult clinical scenario for the treating orthopedic surgeon. The most common mode of failure after DCE is persistent pain and potential etiologies include under-resection, over-resection leading to joint instability, postoperative stiffness, heterotopic ossification, untreat

An asymptomatic enlargement of the medial clavicle is a condition that is often both missed and misdiagnosed. We review the most common causes of an isolated and asymptomatic enlargement of the medial clavicle. Underlying aetiologies include osteoarthritis of the sternoclavicular joint, condensing osteitis, spontaneous dislocation of the sternoclavicular joint, and sternocostoclavicular hyperostosis. Key points in the history and physical examination as well as characteristic radiographic findings are sufficient for correct diagnoses. Treatment is conservative.

Open intramedullary fixation of 37 fresh midshaft clavicular fractures in 35 patients was performed using a 6.5 partially threaded cancellous screw. Mean age was 38 years (range 18-65). The screw was inserted from the medial fragment after retrograde drilling of that fragment. Average follow-up period was 21 months (range 9-36). Radiological evidence of union was apparent in all cases within six to eight weeks after surgery (mean 7.8). Two cases had intraoperative failure of fixation, nine complained of subcutaneous prominence of the screw head, five experienced decreased sensation over the si

Congenital pseudarthrosis of the clavicle is a rare entity. It presents on the right side in 90% of the patients, and bilaterally in up to 10%. The authors report the case of a 4-year-old boy who presented with a painful deformity over his right mid-clavicular area. Plain radiographs were inconclusive, although the opposite is true in most cases. A computed tomography (CT) 3-D reconstruction showed a pseudarthrosis of the clavicle, and excluded a neoplastic, infective or traumatic origin. Treatment involved excision of the pseudarthrosis, internal fixation with a contoured reconstruction locki

OBJECTIVE: To investigate the result of tissue flap transferring for wound repair of the clavicle.
METHODS: From 1994 to 2000, 3 patients (1 with clavicle osteosynthesis, 1 with chronic clavicle osteomyelitis, and 1 with radioactive ulcer in clavicular region accompanied by chronic osteomyelitis of clavicle) were reconstructed with turnover adipofascial flap, myocutaneous flap of pectoris, and myocutaneous flap of latissimus dorsal respectively. The outcome was observed. The operation principles of tissue flaps transferring for wound repair of the clavicle were summarized.
RESULTS: Follow-up

Stress fractures of the clavicle are rare. This is the report of one such fracture in a 10 year old female gymnast, who presented with a six week history of medial clavicular pain. Radiographs and a computed tomography scan showed an undisplaced fracture through the medial third of the clavicle extending inferiorly to the rhomboid fossa in the inferior aspect of the clavicle. Pathological fracture was excluded by magnetic resonance imaging. The patient was treated conservatively and was able to return to full training eight weeks later.

BACKGROUND: Internal fixation of one or both bones is the recommended treatment for floating shoulder injuries (ipsilateral fractures of the scapula and clavicle). Perceived risks of nonoperative treatment include abduction weakness, decreased range of motion, chronic pain, malunion, and nonunion. None of these problems, however, have been confirmed by clinical studies. The purpose of this retrospective study was to analyze the clinical and radiographic results of nonoperative treatment of floating shoulder injuries.
METHODS: Twenty patients with a floating shoulder injury were treated with e

Although often viewed as benign injuries, clavicular fractures can lead to complications, particularly nonunions. The nonunion rate has been reported to be between 0.1% and 15%. Contributing factors to nonunion include severe initial trauma, marked initial displacement and shortening, soft tissue interposition, primary open reduction and internal fixation, refracture, open fracture, polytrauma, and inadequate initial immobilization. A clavicular nonunion is rarely asymptomatic and often results in disability from pain at the site of nonunion, altered shoulder mechanics, or a compression lesion

Seventy-three patients had operative resection of the lateral end of the clavicle for the treatment of a painful condition of the acromioclavicular joint. Thirty-two of the patients had had a traumatic separation of the acromioclavicular joint, eight had had a fracture of the lateral end of the clavicle, and thirty-three had primary acromioclavicular osteoarthrosis. An average of sixteen millimeters (range, fine to thirty-three had primary acromioclavicular osteoarthrosis. A patient receiving home parenteral nutrition developed osteomyelitis of the clavicle associated with a Hickman catheter which had been functioning for 14 months. The infection was treated with bone curettage, parenteral antibiotics, and catheter removal. Although this has been a reported complication of subclavian vein catheters, this is the first reported case associated with a Hickman catheter.

A woman with rheumatoid arthritis developed persistent sterile drainage from a cutaneous fistula after biopsy of an inflamed supraclavicular mass. Radiographs showed several cavities in the underlying clavicle. Inability to culture a pathogen and failure of the fistula to heal despite empirical courses of antibiotic therapy led to surgical intervention. The final diagnosis, based on careful histological analysis by special staining techniques, was rheumatoid bursitis extending into the clavicle and to the skin surface.
