A carbapenem-resistant and colistin-heteroresistant clinical isolate of Enterobacter cloacae was obtained from an inpatient in Okinawa, Japan. The minimum inhibitory concentrations of both imipenem and meropenem were 32 μg/mL. The isolate showed heteroresistance to colistin using the Etest method and resistance to colistin using the broth microdilution method. It had a disrupted ompC and a mutation in the promoter region of blaACT-2, but did not harbor any genes encoding carbapenemase. The disruption of ompC and the mutation in blaACT-2 was associated with the carbapenem resistance of this iso

We describe a clonal dissemination of KPC-producing Enterobacter cloacae in a Brazilian hospital. Patients diagnosed with theses isolates showed high mortality rate (41.8%) and were associated with previous use of antibiotics and urinary catheterization. Therefore, infection control measures and use of stricter antibiotic policies are required to control the spread of these organisms.

BACKGROUND: The utility of carbapenems, which are considered 'last-line' agents, is being diminished by the growing incidence of various resistance mechanisms in bacteria. We aimed to investigate the molecular mechanism of carbapenem resistance in Enterobacter cloacae recovered from a 76-year-old patient who had undergone coronary artery bypass grafting and repair of the mitral and tricuspid valves. Interestingly, the patient had no prior history of hospital admission abroad.
METHODS: The Carba-NP test II and synergy testing were performed to confirm carbapenemase activity. PCR was used to de

We report the case of using a long-term combination of meropenem and amikacin to treat infective endocarditis caused by Enterobacter cloacae resistant to third- and fourth-generation cephalosporins. Multi-drug resistant Gram-negative bacilli, such as the E. cloacae in our study, may become possible pathogens of infective endocarditis. Our experience with this case indicates that long-term use of a combination of β-lactam and aminoglycosides might represent a suitable management option for future infective endocarditis cases due to non-Haemophilus, Actinobacillus, Cardiobacterium, Eikenella, Ki

INTRODUCTION: Enterobacter cloacae is responsible for 65-75% of all Enterobacter infections, bacteremia being the most common syndrome. The majority of infections are nosocomially acquired and in patients with predisposing factors.
CASE PRESENTATION: We present a case of E. cloacae bacteremia secondary to acute cholecystitis in a 60-year-old man with recent diagnosis of cholelithiasis. The diagnosis was established with abdominal echography and positive blood and biliary cultures. The patient was managed successfully with cholecystectomy and antibiotic therapy.
CONCLUSION: The peculiarity of

In this study, a polysaccharide (ECP) was isolated from the mycelium of Enterobacter cloacae and was found to exhibit strong antibacterial activities against E. cloacae producing SHV-12 ESBL with the increase of the inhibition zone diameter. Its minimum inhibitory concentration (MIC) and minimal bactericidal concentration (MBC) were 12.5 mg/L and 25 mg/L, respectively. ECP at these concentrations immediately inhibited planktonic growth of the bacteria especially at the time from 2 to 10 h. Flow cytometry analysis further revealed that almost all the bacterial cells were damaged following ECP t

STUDY OBJECTIVE: To determine whether cloacal complexity, defined by common channel length, is associated with early introital or vaginal stenosis following primary cloacal repair.
METHODS: Retrospective analysis of a prospective cohort of patients with persistent cloaca treated at a quaternary-care children's hospital between January 2020 and May 2026. Patients who underwent primary cloacal repair with vaginal reconstruction and post-repair exam under anesthesia were included; those with cloacal exstrophy, cloacal variants, or prior repair at an outside institution were excluded. The cloaca is an embryonic cavity that is divided into the urogenital sinus and rectum upon differentiation of the cloacal epithelium triggered by tissue-specific transcription factors including CDX2. Defective differentiation leads to persistent cloaca in humans (PC), a phenotype recapitulated in Cdx2 mutant mice. PC is linked to hypo/hyper-vitaminosis A. Although no gene has ever been identified, there is a strong evidence for a genetic contribution to PC. We applied whole-exome sequencing and copy-number-variants analyses to 21 PC patients and their unaffected parents. The damaging p.Cys132

INTRODUCTION: Colistin has emerged as the last resort for treating multidrug-resistant Enterobacter cloacae complex (ECC) infections. The primary purposes of this study were to demonstrate the presence of colistin heteroresistance in ECC and to further investigate their clinical characteristics, molecular epidemiology and mechanisms.
METHODS: Population analysis profiles (PAP) were performed to confirm the heteroresistance phenotype. Average nucleotide identity (ANI) was determined to classify ECC species. Phylogenetic analysis based on core genome single nucleotide polymorphisms (cg-SNPs), m

BACKGROUND: Enterobacter cloacae (E. cloacae) is a notorious pathogen that poses serious threat to both human and animal health, causing severe gut infections and contributing to food spoilage. Traditional chemical treatment have led to increased drug resistance and environmental pollution. This study investigates the potential of Sanguinarine (SAN), a natural plant extract, as an alternative to chemical antibiotics.
RESULTS: In light of the escalating issue of antibiotic resistance, we examined the antibacterial efficacy and mechanisms of SAN against E. cloacae in vitro. Our findings reveale

PURPOSE: To report a case of Enterobacter cloacae (E. cloacae) keratitis in a patient who underwent corneal crosslinking (CXL) for progressive keratoconus.
OBSERVATIONS: A 19-year-old female underwent CXL for keratoconus in her left eye. The patient neglected her post-procedure medications and missed the follow-up visit. Subsequently, she presented on day-10 post CXL with redness and pain in the treated eye. Clinical examination revealed a ring-shape infiltrate measuring 7.8 mm in diameter. Culture indicated the presence of E. cloacae. Treatment with gentamicin failed after the emergence of r

We evaluated the β-lactam resistance phenotypes of clinical and environmental strains of the Enterobacter cloacae complex (ECC) isolated from three Algerian hospitals. The first combination of API 20E, matrix-assisted laser desorption/ionization time-of-flight mass spectrometry (MALDI-TOF MS), and hsp60 genetic clustering methodologies were carried out for the identification of ECC strains. Our research showed that API 20E and MALDI TOF MS are satisfactory in genus identification of ECC strains, but sequence-based methods are then necessary to discriminate the species and subspecies levels. Four weeks after a bilateral total knee arthroplasty (TKA), an immunocompetent, 61-year-old, Caucasian man presented with a periprosthetic joint infection (PJI) of the left knee by Enterobacter cloacae (an enteric bacteria). The most likely source of his infection was due to an anastomotic leak after a bariatric surgery done 6 months before TKA. There is a growing focus on stratifying the risk of PJI after TKA. Hematogenous seeding of enteric bacteria leading to PJI is an unexplored risk that will become more prevalent as bariatric procedures before TKA continue to increase in frequency. We pr

OBJECTIVES: The Enterobacter cloacae complex (Ecc), routinely referred to as "E. cloacae" in clinical microbiology, encompasses several species with 12 genetic clusters and one sequence crowd that can be identified based on hsp60 sequencing. Little is known about the pathogenicity and distribution of resistance to antibiotics among the Ecc.
METHODS AND RESULTS: In this prospective multicentre study, a total of 193 Ecc clinical isolates were collected from 10 academic hospitals distributed nationally across France and identified at the genetic cluster level on the basis of hsp60 sequencing. E.

The water-soluble polysaccharides (EPS) were isolated from culture broth of Enterobacter cloacae Z0206, and the hypoglycemic and hypolipidemic effects of EPS were investigated. In this study, KKAy mice were gavaged once daily with either distilled water or EPS (200 mg/kg body weight) for 6 weeks. Results showed that EPS possessed significant hypoglycemic effects. Improved oral glucose tolerance, reduced serum insulin levels as well as decreased serum triglycerides (TG), cholesterol (TC) and low density lipoprotein cholesterol (LDL-c) were observed after treatment with EPS. Furthermore, EPS upr

Enterobacter cloacae is a gram negative bacillus that is ubiquitous as a contaminant and a pathogen in adult, paediatric, and neonatal ICUs. Its transmission is almost exclusively nosocomial with community acquired infection reported rarely. We report a case of community acquired, rapidly progressive E. cloacae meningo-encephalitis in a neonate. A three-week-old term infant presented from home, having been discharged from hospital within two days of delivery. She rapidly progressed to multi-organ dysfunction. Initial CT of her brain showed evidence of severe hypoxic changes and herniation of t

A multiresistant E. cloacae strain spread during a six month period in a paediatric oncology ward amongst nine children, who had different tumors and malformations. Three children who had shared a room were especially affected. E. cloacae was isolated 122 times from the children with tumors and five times from their environment. Specimens from which the bacteria were isolated, included blood cultures, catheter tips, wound swabs, drains, skin and mucous membranes from most parts of the body. The majority of the E. cloacae strains were resistant to ampicillin, mezlocillin, piperacillin, azlocill

Lemierre syndrome describes septic thrombophlebitis of the internal jugular vein (IJV) and metastatic spread of the infection following a recent oropharyngeal infection in a setting of bacteremia caused by Fusobacterium necrophorum. Lemierre-like syndrome describes similar clinical scenario with no preceding oropharyngeal infection and/or in the setting of non-Fusobacterium cause. We report a case of Lemierre-like syndrome in a setting of Enterobacter cloaca bacteremia without known preceding oropharyngeal infection. History and physical exam revealed an irritable infant with cough, tachypnea,
