Critical analysis of the performance of double-contrast barium enema for detecting colorectal polyps > or = 6 mm in the era of CT colonography.

OBJECTIVE: The purpose of our study was to perform a meta-analysis comparing the performance of double-contrast barium enema (DCBE) with CT colonography (CTC) for the detection of colorectal polyps > or = 6 mm using endoscopy as the gold standard. MATERIALS AND METHODS: Prospective DCBE and CTC studies were identified. Percentages of polyps and of patients with polyps > or = 10 mm and 6-9 mm were abstracted. The performance of DCBE versus CTC was determined by separately evaluating each technique's performance versus that of endoscopy, and contrasting the techniques. The I-squared statistic a

The demise of the instant/unprepared contrast enema in large bowel obstruction.

OBJECTIVE: Imaging the colon in suspected acute large bowel obstruction (LBO) is traditionally carried out with a supine abdominal X-ray (AXR) and erect chest X-ray. If there is no clinical or radiological evidence to suggest a perforation, then an unprepared barium or water-soluble contrast enema (CE) can be performed to confirm the presence of and demonstrate the site of obstruction. The advent of modern, fast multidetector CT (MDCT) scanners has changed management strategies for acute abdominal conditions including suspected LBO in all groups of patients especially the elderly, infirm and t

Colorectal cancer screening: a guide to the guidelines.

The best known guidelines for colorectal cancer screening in the United States are those of the US Multisociety Task Force on Colorectal Cancer, the American Cancer Society, the United States Preventive Services Task Force, the American College of Gastroenterology, and the American Society of Gastrointestinal Endoscopy. Screening is currently endorsed for both average-risk and high-risk persons by all guideline groups. Some guidelines offer a menu of options for average-risk persons, from which patients and physicians can select according to their perceptions and values regarding effectiveness

CT colonography for follow-up after surgery for colorectal cancer.

OBJECTIVE: The purpose of this article is to discuss the CT colonography (CTC) findings and the role of CTC for follow-up after curative surgery for colorectal cancer. CONCLUSION: Contrast-enhanced CTC can be effective for surveillance for colorectal cancer recurrence after curative surgery because it enables simultaneous evaluation of distant abdominal metastasis, pericolic recurrence, intraluminal recurrence, and metachronous lesions. The appearances of anastomotic recurrences at CTC overlap with those of more common inflammatory polyps and rare benign ulcers.

Diagnostic performance of computed tomography colonography following incomplete colonoscopy and its associated factors: A single-centre retrospective study.

INTRODUCTION: Incomplete colonoscopy presents an important clinical challenge, as segments of the colon that are not visualised may contain significant pathology. Computed tomography colonography (CTC) provides a minimally invasive alternative for comprehensive luminal evaluation, but local evidence regarding its diagnostic yield remains limited. This study aimed to evaluate the diagnostic yield of CTC following incomplete colonoscopy and to determine its associated factors with demography. MATERIALS AND METHODS: A retrospective cohort study was conducted among 93 adult patients who underwent

Computed tomography colonography performs poorly in detection of sessile serrated lesions.

BACKGROUND: Computed tomography colonography (CTC) is an alternative to colonoscopy for the detection of polyps and colorectal cancer (CRC). One-third of CRCs arise via the sessile serrated pathway. Evidence supports using CTC to detect adenomas and CRC; however, its accuracy for sessile serrated lesions (SSLs) remains uncertain. This study aimed to determine the accuracy of CTC in detecting SSLs compared with colonoscopy. METHOD: Electronic records identified all colonoscopy procedures where a histologically validated SSL was excised over a 11-month period. In those patients who had a CTC wi

Risk factors, prevention and screening of colorectal cancer: A rising problem.

Colorectal cancer (CRC) is the third most commonly diagnosed cancer and the second leading cause of cancer death worldwide. The leading risk factors for CRC include male gender, age over 50, family history, obesity, tobacco smoking, alcohol consumption, and unhealthy diet. CRC screening methods vary considerably between countries and depend on incidence, economic resources and healthcare structure. Important aspects of screening include adherence, which can vary significantly across ethnic and socioeconomic groups. Basic concepts of CRC screening include pre-stratification of patients by ident

CT colonography: implementation, indications, and technical performance - a follow-up national survey.

BACKGROUND: Computed tomography colonography (CTC) is an accepted complement or alternative to optical colonoscopy (OC) but its implementation is incompletely analyzed, and technical performance varies between centers. PURPOSE: To evaluate implementation, indications, and technical performance of CTC in Sweden and to evaluate compliance to international guidelines. MATERIAL AND METHODS: A structured, self-assessed questionnaire regarding implementation and technical performance of CTC was sent to all eligible radiology departments in Sweden. Eighty-six out of 89 departments replied. Comparis

CT Colonography: an update on current and future indications.

Computed tomographic colonography (CTC) is a minimally invasive, patient-friendly, safe and robust colonic imaging modality. The technique is standardized and consolidated evidence from the literature shows that the diagnostic performances for the detection of colorectal cancer and large polyps are similar to colonoscopy (CS) and largely superior to alternative radiological exams, like barium enema. A clear understanding of the exact role of CTC will be beneficial to maximize the benefits and minimize the potential sources of frustration or disappointment for both referring clinicians and pati

Expanding utilization and regional coverage of diagnostic CT colonography: early Medicare claims experience.

PURPOSE: The aim of this study was to assess national and regional trends in utilization and Medicare coverage of diagnostic CT colonography (CTC). METHODS: Medicare claims for diagnostic CTC were identified for the first 4 complete years for which Current Procedural Terminology® tracking codes existed (2005-2008). The frequencies of billed and denied services were extracted on a national and regional basis, along with physician provider specialty and site of service. RESULTS: Total annual claims for diagnostic CTC for Medicare fee-for-service beneficiaries increased from 3,660 to 10,802 (+1

Choosing between CT colonography and colonoscopy in the diagnostic context: a qualitative study of influences on patient preferences.

OBJECTIVE: To explore information needs and preferences on diagnostic bowel tests and elicit preferences for CT colonography (CTC) vs. colonoscopy (CC). BACKGROUND: CTC is a new technology for large-bowel imaging that has been widely assumed to be more acceptable than CC because it is non-invasive. DESIGN: Semi-structured focus groups discussing information choices and procedure preferences. SETTING AND PARTICIPANTS: Non-patient sample of 26 asymptomatic volunteers (mean age 64 years). MAIN OUTCOME MEASURES: Information choices and CC-vs.-CTC preferences were recorded following stepwise pr

Colon imaging in radiology departments in 2008: goodbye to the routine double contrast barium enema.

We radiologists are free to choose DCBE or CTC when patients are referred to us for colonic examination. The studies reported during 2007 have confirmed that CTC is more accurate, preferred by patients, with a shorter room time, fewer complications, lower radiation exposure, and reveals therapeutically significant extracolonic lesions in 5% to 10% of cases, so that it is beginning to seem rather irresponsible to continue to offer routine DCBE examinations. In older patients the yield of extracolonic abnormalities is even greater, with 505 abnormalities found in 268 of 400 consecutive patients

Vasovagal reactions in CT colonography.

We describe the occurrence of vasovagal reaction in two patients who underwent CT colonography (CTC). The patients, asymptomatic, were submitted to CTC in one case after right colectomy and in one case for screening purposes. The vasovagal symptoms occurred after pneumocolon and acquisition in the prone decubitus, and included headache, hypotension, bradycardia, cold sweat and pallor, nausea, and diaphoresis. Abdominal pain was also referred. All symptoms resolved within 30 min to 3 h from their onset. In all cases the vasovagal reaction occurred after prone decubitus. CTC images showed a sign

Current and evolving strategies for colorectal cancer screening.

BACKGROUND: Colorectal cancer is a major cause of cancer mortality and morbidity. Screening can potentially prevent most colorectal cancers by detection and removal of precursor adenomas. METHODS: The literature and clinical practice guidelines are reviewed, with an emphasis on advances of the last 10 years and evolving screening methods. RESULTS: Colonoscopy has come to be used for screening in persons at average risk for colorectal cancer because of the comparative ineffectiveness of other methods, although these methods continue to be recommended. Virtual colonoscopy and fecal DNA testing

Current colorectal cancer screening strategies: overview and obstacles to implementation.

Colorectal cancer is the second leading cause of cancer death but is largely preventable. Current strategies to prevent colorectal cancer vary considerably with regard to effectiveness, up-front costs, risks, and invasiveness. Current levels of participation in colorectal cancer screening in the U.S. population are low. Both physician and patient attitudes contribute to low levels of screening uptake. New colorectal cancer tests are one mechanism to improve adherence. Virtual colonoscopy may have an important impact if techniques to perform it without bowel preparation can be developed. Stool-

Does virtual colonoscopy have a major role in population-based screening?

In summary, technical advances in the performance of VC are occurring at a very rapid pace. These technical improvements will undoubtedly improve the polyp detection rate and reduce false-positive and false-negative examinations. The concept of VC is clearly attractive and the general public is enamored with everything that has an association with virtual reality. As other articles in this issue have revealed, there are many techniques in development to help stratify patients at risk for colon cancer. As we begin to focus our health care resources on those at highest risk, the less need there

CT colonography for longitudinal in-vivo assessment of colonic lengthening in middle-age and older adults.

BACKGROUND: Although a general trend for longer colons among older adults has been observed, longitudinal demonstration of intra-patient elongation is lacking. We assessed colonic length at serial CT colonography (CTC), which allows for precise length measurement over time. METHODS: Longitudinal CTC evaluation (mean time interval, 12.8 years; range, 11.6-16.6 years) in 100 asymptomatic adults (mean age, 54.2 years at index study; 50 men, 50 women) were analyzed using a dedicated software system (V3D Colon, Viatronix). Segmental and total colonic lengths were obtained at interactive 2D and 3D
