The Israeli eye care system through a public and global health lens.

Global eye health is increasingly recognized as integral to achieving universal health coverage (UHC), with equity, access, and service integration serving as key indicators of system performance. Israel provides a distinctive case study, having established UHC through the 1994 National Health Insurance Law, which guarantees essential services to all residents. This review assesses Israel’s eye care system through a public and global health lens, applying the WHO health system building blocks to examine its organization, strengths, and persistent challenges. Israel benefits from strong integra

Avoidable pitfalls on the path to health financing self-reliance in low-income and middle-income countries.

Low-income and middle-income countries (LMICs) are facing an urgent and complex challenge: how to transition to greater self-sustainability in health financing amid declining donor support. While this shift is inevitable, the policy responses it elicits carry significant implications for health system equity and access. This commentary highlights four policy choices increasingly observed in LMICs that we argue are unacceptable in the pursuit of sustainability. These include: (1) shifting the financial burden to out-of-pocket payments; (2) over-reliance on contributory health insurance schemes;

Implementation determinants of risk-stratified gestational diabetes mellitus screening in community-based women's peer groups in rural western Kenya.

BACKGROUND: Gestational Diabetes Mellitus (GDM) contributes to maternal and neonatal complications and is a growing concern in Kenya. The 2-hour oral glucose tolerance test (OGTT) is recommended to diagnose GDM between 24 and 28 weeks of pregnancy, but uptake remains low in settings experiencing financial and structural barriers to health care access. The Stratification Risk of Diabetes in Early pregnancy (STRiDE) tool is a risk-prediction model designed to identify women at elevated risk for GDM. The STRiDE-GDM screening approach integrates this tool within community-based peer groups to guid

Insurance- and medical provider-related barriers and facilitators to staying on PrEP: results from a qualitative study.

Pre-exposure prophylaxis (PrEP) is a highly effective biobehavioral strategy for preventing HIV acquisition. Although PrEP uptake has increased steadily, discontinuation rates are high among members of key populations like gay and bisexual men (GBM). Understanding the challenges that arise for PrEP users is key to better PrEP implementation and sustained use over time. We report on barriers that arose for PrEP-using GBM, as well as facilitating factors that aided PrEP persistence, with the goal of informing PrEP implementation efforts. In 2015-2016, 103 PrEP-using GBM in NYC completed qualitat

Mental health disparities, mechanisms, and intervention strategies: perspective from Hawai'i.

PURPOSE OF REVIEW: Although there is a growing body of literature describing the scope and impact of mental health disparities, there is relatively less literature focused on youth and on interventions that are grounded in the cultures of youth most significantly affected by disparities. From the perspective of Hawai'i, one of the world's most diverse communities where disparities nonetheless exist, the authors review the varieties of diversity encountered in psychiatry and healthcare, specific youth mental health disparities, and examples of locally tailored solutions. RECENT FINDINGS: Menta

Potentially inappropriate medications in older adults: a population-based cohort study.

BACKGROUND: Non-optimal medication use among older adults is a public health concern. A concrete picture of potentially inappropriate medication (PIM) use is imperative to ensure optimal medication use. OBJECTIVE: To assess the prevalence of PIMs in community-dwelling older adults and identify associated factors. METHODS: A retrospective population-based cohort study was conducted using the Quebec Integrated Chronic Disease Surveillance System (QICDSS). The QICDSS includes data on drug claims for community-dwelling older adults with chronic diseases or at risk of developing chronic diseases

Health service utilization and medical costs for in-hospital death at the end of life.

BACKGROUND: Despite comprising < 1% of the population, end-of-life (EOL) patients consume 8%–11.2% of global health expenditures. China’s aging population and limited health service urgently require cost-factor analyses. Data from 3,323 in-hospital deaths (2021–2023) were collected to explore health service utilization and medical costs at the EOL and provide evidence-based recommendations. METHODS: Based on a review of medical records from 2021 to 2023 in a tertiary hospital, data from 3,323 in-hospital deaths were compiled and analyzed. Descriptive statistics were applied to summarize demogr

Issues navigating the healthcare delivery system among caregivers of children and youth with special healthcare needs: a qualitative study.

INTRODUCTION: Caregivers of children and youth with special health care needs (CYSHCN), including those with disabilities, have numerous roles managing care for their children. While caregiver burden is well documented, less is known about how barriers within the healthcare delivery system contribute to this burden and how systems can be redesigned to better support families. This study explored Illinois caregivers' experiences navigating the healthcare delivery system for CYSHCN and sought their perspectives on resources needed to reduce caregiver burden. METHOD: We conducted a qualitative s

Determining successful pain clinic treatment through validation of cost effectiveness.

Initially, the Center for Pain Control designed and implemented multiple measures to indicate treatment efficacy in an outpatient setting. Decreased use of medical care at 1 year after treatment was one of five measures used. Economic concerns, both in the health and the insurance industries, mandated greater emphasis on cost effectiveness at a primary success determinate. Cost effectiveness was measured through broad diagnostic categories: lumbar pain with or without radiculopathy, cervical pain with and without radiculopathy, and a combination of lumbar and cervical pain. Cost for surgical t

Conversations about Abnormal Mammograms on Distress and Timely Follow-up Across Ethnicity.

Communication with healthcare providers, family, and friends is associated with increased mammography use. Less is known about the abnormal mammogram experience, especially in terms of the interval between screening and follow-up appointments (time to follow-up) and psychological distress. The impact of communication may vary across ethnicity, depending on cultural emphases placed on interpersonal relationships. The current study's objectives were to (a) explore the role of family/friend and provider communication with regard to time to follow-up and distress and (b) examine if family/friend a

Attitudes and beliefs regarding colorectal cancer screening among African American patients of community health centers in North-Central Florida and stakeholder perspectives: a qualitative study.

BACKGROUND: Colorectal cancer (CRC) is one of the leading causes of cancer mortality in the US with African Americans continuing to experience CRC health disparities due to numerous factors. While colonoscopy remains the most utilized test for CRC screening, stool-blood tests are cost-effective and offered by community health centers (CHC) serving un- and underinsured patients. Qualitative research has identified multilevel barriers to CRC screening and can potentially inform the development of tailored interventions to promote stool-based screening. METHODS: In this qualitative study, we exp
