INTRODUCTION: Membranous nephropathy (MN) is the most common cause of nephrotic syndrome in Caucasian adults. Phospholipase A2 receptor (PLA2R)- and exostosin 1 (EXT1)/exostosin 2 (EXT2)-associated MN represent the most common primary and secondary forms of MN. A female patient with disfiguring lupus erythematosus profundus (LEP) from the age of 13 years was found to have an isolated partial C4 deficiency, with reduced levels of both allotypes, C4A and C4B. A genetic basis for the hypocomplementaemia was confirmed by a family study of complement and HLA types which revealed heterozygous null alleles for C4A and C4B in the proband. Marked improvement in her cutaneous lesions occurred with thalidomide.

Low levels of complement components C3, C4 and factor B were found in the sera of most patients with early and advanced Gambian trypanosomiasis. C3 levels increased after treatment with melarsoprol (Mel B). Detailed complement system studies were performed in 22 patients with adult coeliac disease. Activation products of C3 were observed in the fresh sera of all untreated patients, while only 4 had activation products of factor B of the alternate pathway. Levels of C4 and C3 were lower than normal mean, but only the depression of C4 reached a level of statistical significance. Serum C3 and C4 levels have been determined in patients with Addisonian pernicious anaemia (PA) and megaloblastic anaemia due to vitamin B12 deficiency from other causes, before and after treatment, in order to study the interaction between vitamin B12 deficiency and complement and the role of complement in the pathogenesis of the gastric lesion of PA.
