BACKGROUND: In the absence of low-titer group O whole blood, conventional components are often transfused to hemorrhaging trauma patients in a ratio designed to replicate whole blood. An 81-year-old man, admitted under the Ear, Nose and Throat Team with persistent epistaxis, developed an acute confusional state. He was previously physically independent and mentally competent. Immediate investigations did not reveal a cause for his deterioration. He eventually made a full recovery and returned home. Subsequently, his serum bismuth level was noted to be within the toxic range and was felt to have been the cause of his confusional state.

The authors describe a case of milk-alkali syndrome in a man who consumed antacid tablets (Rennie) for chronic epigastric pain. Simultaneous occurrence of hypercalcemia, metabolic alkalosis, and renal insufficiency, in conjunction with the appropriate history of ingestion of calcium carbonate-containing antacids, was suggestive of the syndrome. The syndrome became uncommon with the advent of modern ulcer therapy, but currently is increasing in frequency with the calcium supplementation drugs taken to prevent osteoporosis. This syndrome may produce life-threatening hypercalcemia.

A 67 year old man is admitted with mental confusion. He has been treated 10 months before the present complication for a diffuse bronchiolo-alveolar carcinoma with a good response to chemotherapy. Brain imaging shows symmetrical lesions in the periventricular areas and in the semi-oval centers. A test will allow to obtain the diagnosis.

We describe six independent cases of Mesocestoides infection in dogs presenting with diarrhea. Between November 2022 and August 2024, we were contacted by veterinarians regarding the identification of a species of small tapeworm excreted in dog feces. The veterinarians suspected the organism was Echinococcus multilocularis and believed it should be reported to health centers as a notifiable disease.
