Postburn contractures are a common occurrence after severe burn injuries. It is important to understand the pathologic condition and anatomy of specific postburn deformities in order to provide comprehensive surgical care. Postburn contractures can result in a flexion contracture, boutonniere deformity, burn syndactyly, metacarpophalangeal extension contracture, wrist contracture, or claw hand. A patient evaluation is performed before proceeding to the operating room. Surgery sequences require proper incision design, release of the skin, and deeper contracted structures and coverage with an ap…

Several functionally limiting sequelae can follow deep thermal injury to the hand. Despite appropriate initial management, contractures are common. Whereas acute burn care is often managed by multidisciplinary, specialized burn units, postburn contractures may be referred to hand surgeons, who should be familiar with the patterns of burn contracture and nonsurgical and operative options to improve function and expected outcomes. The most common and functionally limiting sequelae are contractures of the webspace, hand, and digits. Webspace contractures and postburn syndactyly are managed with s…

Dynamism of Dupuytren's contractures may occur if a contracture crosses over both the metacarpophalangeal (MCPJ) and proximal interphalangeal (PIPJ) joints of one finger. In this situation the extension deficit at one joint can be influenced by the position of the other. This may affect clinical measurements and introduce errors and bias into research using angular deformity as an endpoint. This study reports the dynamism encountered when measuring passive joint extension deficits in pre-operative contractures. Eighty-five digits with a contracture affecting the PIPJ or MCPJ and PIPJ on 72 han…

A wide range of conditions can lead to intrinsic contractures of the thumb. A thorough understanding of the normal and pathologic anatomy as well as the disease processes and their effect on thumb function is essential in understanding and treating these contractures. Because intrinsic contractures of the thumb rarely present in isolation, a patient-specific approach based on functional needs is required. Prevention of iatrogenic contractures and progression of predictable contractures regardless of etiology is the health care provider's primary responsibility.

Contractures of the intrinsic muscles of the fingers disrupt the delicate and complex balance of intrinsic and extrinsic muscles, which allows the hand to be so versatile and functional. The loss of muscle function primarily affects the interphalangeal joints but also may affect etacarpophalangeal joints. The resulting clinical picture is often termed, intrinsic contracture or intrinsic-plus hand. Disruption of the balance between intrinsic and extrinsic muscles has many causes and may be secondary to changes within the intrinsic musculature or the tendon unit. This article reviews diagnosis, …

Burn contractures are common after deep burns of the limbs. When rehabilitation fails to restore joint function and normal range of motion, surgical contracture release should be performed. Different plastic surgery techniques can be used, from Z-plasty to skin grafts, artificial dermis and flaps. Surgery can also be used in case of esthetic concerns. Available surgical techniques and indications are presented.

Dupuytren's contracture is a connective tissue disorder characterized by nodular fibroblastic proliferation of palmar fascia leading to a shortening and progressive digital flexion deformity. It seems that this pathological process is initiated by the differentiation of fibroblast into myofibroblast which has morphologic characteristic of both fibroblast and a smooth muscle cell. Myofibroblasts are able to biosynthesize and secrete growth factors which subsequently bind to the receptors anchored on its surface, bringing about an autocrine and paracrine modality of control cell growth, differen…

The standard treatment of Dupuytren's contracture of the digits, to date, is surgical. The aetiology, pathology, clinical findings and present day treatment are discussed and possible treatment for the future is suggested.

A cross-sectional study of 28 patients with multiple congenital contractures of miscellaneous origin is presented. We describe the clinical, genetic, and neurological diagnosis and the involvement of upper and lower extremities and spine. All treatments that patients received so far as well as functional outcome were studied. We compared these factors in children with anterior horn cell degeneration (AHCD) or amyoplasia with those of children with contractures of other origin. A correct genetical diagnosis of multiple congenital contracture is important because children with AHCD will need mor…

Dupuytren's contracture is a deforming, fibrotic condition of the palmar fascia which has confounded clinicians and scientists since the early descriptions by Guillaume Dupuytren in 1831. It predominantly affects elderly, male caucasians, has a hereditary predisposition and has strong associations with diabetes, alcohol consumption, cigarette smoking and HIV infection. The major morphological features are an increase in fibroblasts, particularly around narrowed fibroblasts; a finding consistent with localised ischaemia. During ischaemia, adenosine triphosphate (ATP) is converted to hypoxanthin…

Dupuytren's disease is a fibrotic process that can impair hand function by producing flexion contractures of the digits. The hypertrophic changes and contracture of the palmar fascia are frequently encountered in the geriatric population but can also occur in the adolescent age group. There is a definite genetic predisposition, and familial occurrences are common. Females of any age and males under age 40 are more prone to postoperative complications than other patients.

Quadriceps contracture associated with a healed malunion fracture of the left femur was diagnosed in a 1-year-old cat. The proximal portion of the distal fracture piece protruded through the skin of the thigh. The protruding bone was removed surgically and the area of ulcerated skin was debrided. Because the fracture was stable and the cat was able to bear weight on the affected limb, surgical correction of the malunion was not performed. Quadriceps contracture is uncommon in the cat.
