A comparison of perioperative outcomes for da Vinci robotic-assisted, laparoscopic and open surgery in oncologic procedures: a systematic literature review and meta-analysis of evidence from China.

The use of robot-assisted surgery (RAS) has increased markedly in China, but comparative evidence on its perioperative performance in cancer care is dispersed across specialties and study designs. This review synthesized Chinese evidence to examine the safety and effectiveness of da Vinci robot-assisted surgery (dV-RAS) relative to laparoscopic/video-assisted thoracoscopic surgery (LAP/VATS) and open surgery across seven oncologic procedures. We systematically reviewed comparative studies published from 2010 to 2024 and pooled estimates for dV-RAS versus LAP/VATS or open surgery. Prespecified

Laparoscopic Reconstruction in Post-Tubercular Urinary Tract Strictures: Technical Challenges.

BACKGROUND: Genitourinary tuberculosis still continues to plague developing countries and is a significant cause of morbidity as well as mortality in the developing world. At present, nearly 55% of the patients of genitourinary tuberculosis (GUTB) need surgical management. Owing to the presence of dense adhesions and loss of normal anatomical planes, GUTB was considered to be a contraindication to laparoscopic surgery. However, recent literature shows laparoscopy to be feasible in GUTB. Our study aimed at identifying the challenges in laparoscopic urinary tract reconstructive surgery in genito

Success rate of natural orifice specimen extraction after laparoscopic colorectal resections.

PURPOSE: To date, no data have been available to inform which cases are appropriate for natural orifice specimen extraction (NOSE) after laparoscopic colorectal resections (LCRRs). Our aim was to evaluate the success rate and the factors affecting the failure in patients who were scheduled for NOSE after LCRRs. METHODS: Seventy-two consecutive cases that were intended for NOSE after LCRR were enrolled. The transanal route was always chosen as the first option, and when it failed, the transvaginal route was tried in female patients. If both failed, the specimen was judged as unsuitable for NOS

Risk factors for recurrence of congenital diaphragmatic hernia in the era of minimally invasive surgery.

PURPOSE: To elucidate the long-term incidence and risk factors of congenital diaphragmatic hernia (CDH) recurrence in the era of minimally invasive surgery (MIS). METHODS: In a Japanese CDH Study Group database between 2006 and 2023, we included 1,115 patients with CDH who underwent surgery for diaphragmatic repair, with follow-up data available for up to 12 years of age. The following variables were assessed as potential risk factors for recurrence: right-sided hernia, large diaphragmatic defect (international classification C and D), liver herniation, stomach herniation, patch use, and MIS

Factors Associated with and Impact of Open Conversion in Laparoscopic and Robotic Minor Liver Resections: An International Multicenter Study of 10,541 Patients.

INTRODUCTION: Despite the increasing widespread adoption and experience in minimally invasive liver resections (MILR), open conversion occurs not uncommonly even with minor resections and as been reported to be associated with inferior outcomes. We aimed to identify risk factors for and outcomes of open conversion in patients undergoing minor hepatectomies. We also studied the impact of approach (laparoscopic or robotic) on outcomes. METHODS: This is a post-hoc analysis of 20,019 patients who underwent RLR and LLR across 50 international centers between 2004-2020. Risk factors for and periope

Robotic Versus Laparoscopic Rectal Cancer Surgery in Obese Patients: A Frequentist and Bayesian Meta-Analysis.

BACKGROUND: Obesity poses significant technical challenges during rectal cancer surgery (RCS). Robotic surgery may improve operative performance in this setting. We therefore performed a systematic review and meta-analysis comparing robotic and laparoscopic approaches in obese patients. METHODS: A systematic search was conducted across PubMed, Web of Science, Scopus, and the Cochrane Central Register of Controlled Trials up to May 2026. Pooled odds ratios (ORs) and mean differences (MDs) with 95% confidence intervals (CIs) were calculated using random-effects models. Bayesian analyses were ad

Laparoscopic cholecystectomy. A prospective analysis of the potential causes of failure.

Most experienced laparoscopic units suggest a rate of conversion to open cholecystectomy of about 5%. Some failures are predictable preoperatively. We have reviewed the prospective data collected on our first 285 laparoscopic cholecystectomies to provide a basis for advising patients about the likelihood of conversion (failure) if laparoscopic cholecystectomy is attempted. Risk factor analysis was performed to assess the effect on the conversion rate of clinical presentation, preoperative ultrasound features, previous abdominal surgery, and morbid obesity. The overall conversion rate was 4.9%.

Impact of Neoadjuvant Therapy, Tumor Size, and Surgical Approach on Conversion to Open Thoracotomy During Lobectomy.

BACKGROUND: Neoadjuvant therapy options for non-small cell lung cancer (NSCLC) are increasingly used before minimally invasive resection. These operations are generally more challenging and have higher rates of conversion to open surgery. In this context, we compare perioperative outcomes of robotic and video-assisted thoracoscopic surgery (VATS) after neoadjuvant therapy. METHODS: We used national hospital-level data to identify adult patients diagnosed with stage II or III NSCLC undergoing minimally invasive lobectomy after neoadjuvant chemoimmunotherapy, chemoradiotherapy, or chemotherapy

Laparoscopic Splenectomy: Outcomes and Conversion Trends from A 20-Year Single-Center Experience.

Background: Laparoscopic splenectomy has become the preferred approach for most elective splenic indications; however, conversion to open surgery remains a relevant concern, particularly in complex cases. Long-term institutional experiences provide valuable insight into outcomes, learning curves, and evolving indications. Methods: A retrospective observational study was conducted at a tertiary referral center, including all consecutive patients who underwent laparoscopic splenectomy over a 20-year period. Demographic data, surgical indications, conversion to open surgery, and postoperative hos

Prognostic risk factors for conversion in laparoscopic cholecystectomy.

Laparoscopic cholecystectomy (LC) is the gold standard technique for the treatment of gallbladder disease. However, in some cases, conversion to open surgery is still necessary. Identification of patients with high risk of conversion is of great importance to prepare the surgical scenario and to anticipate the convalescence. The objective of this study is to identify the factors that may predict a conversion to an open procedure. A total of 1386 LC were performed in Dr. Manuel Gea González General Hospital, from January 2009 to May 2013. A retrospective analysis of 41 parameters in these patie

Laparoscopic versus robotic surgery for colorectal cancer: an updated meta-analysis with comparison against published systematic reviews.

The comparative effectiveness of robotic versus laparoscopic surgery for colorectal cancer remains debated. We performed an updated meta-analysis through June 2026, with cross-validation against published systematic reviews. PubMed was searched for comparative studies of laparoscopic versus robotic colorectal surgery. After deduplication, 51 primary studies met inclusion criteria; three verified published meta-analyses (2023-2025) were cross-referenced. Meta-analysis used Mantel-Haenszel odds ratios (OR) with DerSimonian-Laird random-effects models, computed as robotic/laparoscopic (OR > 1 ind

Outcomes of Laparoscopic Common Bile Duct Exploration by Chopstick Technique in Choledocholithiasis.

BACKGROUND AND OBJECTIVES: Laparoscopic cholecystectomy with common bile duct exploration (LC with LCBDE) remains the preferred technique for difficult common bile duct stone (CBDS) removal. The chopstick method uses commonly available instruments and may be cost-saving compared to other techniques. We studied the outcome of LCBDE using the chopstick technique to determine if it could be considered a first-choice method. METHODS: Data from all patients that underwent LCBDE from January 1, 2012 to April 30, 2019 were retrospectively analyzed. A standard 4-port incision and CBDS permitted extra
