[A clinical prediction model for N2 lymph node metastasis in clinical stage I non-small cell lung cancer].

OBJECTIVE: To estimate the probability of N2 lymph node metastasis and to assist physicians in making diagnosis and treatment decisions. METHODS: We reviewed the medical records of 739 patients with computed tomography-defined stage I non-small cell lung cancer (NSCLC) that had an exact tumor-node-metastasis stage after surgery. A random subset of three fourths of the patients (n=554) were selected to develop the prediction model. Logistic regression analysis of the clinical characteristics was used to estimate the independent predictors of N2 lymph node metastasis. A prediction model was the

Breast radiotherapy planning: A decision-making framework using deep learning.

BACKGROUND: Effective breast cancer treatment planning requires balancing tumor control while minimizing radiation exposure to healthy tissues. Choosing between intensity-modulated radiation therapy (IMRT) and three-dimensional conformal radiation therapy (3D-CRT) remains pivotal, influenced by patient anatomy and dosimetric constraints. PURPOSE: This study aims to develop a decision-making framework utilizing deep learning to predict dose distributions, aiding in the selection of optimal treatment techniques. METHODS: A 2D U-Net convolutional neural network (CNN) model was used to predict d

Influence of analytical bias and imprecision on the number of false positive results using Guideline-Driven Medical Decision Limits.

BACKGROUND: Diagnostic decisions based on decision limits according to medical guidelines are different from the majority of clinical decisions due to the strict dichotomization of patients into diseased and non-diseased. Consequently, the influence of analytical performance is more critical than for other diagnostic decisions where much other information is included. The aim of this opinion paper is to investigate consequences of analytical quality and other circumstances for the outcome of "Guideline-Driven Medical Decision Limits". TERMS: Effects of analytical bias and imprecision should b

Integrated analysis of genetic and proteomic data identifies biomarkers associated with adverse events following smallpox vaccination.

Complex clinical outcomes, such as adverse reaction to vaccination, arise from the concerted interactions among the myriad components of a biological system. Therefore, comprehensive etiological models can be developed only through the integrated study of multiple types of experimental data. In this study, we apply this paradigm to high-dimensional genetic and proteomic data collected to elucidate the mechanisms underlying the development of adverse events (AEs) in patients after smallpox vaccination. As vaccination was successful in all of the patients under study, the AE outcomes reported li

[Application of the localization technique in video-assisted thoracoscopy for the diagnosis and treatment of solitary pulmonary nodule].

BACKGROUND AND OBJECTIVE: Video-assisted thoracoscopy is widely used in diagnosis and treatment of solitary pulmonary nodule (SPN). However, localization of the nodule during surgery is occasionally difficult. The purpose of the current research is to evaluate the localization technique during diagnosis and treatment of SPN with video-assistant thoracoscopic surgery (VATS). METHODS: Three-dimensional computed tomography is performed for SPN localization. If SPN is less than 1.5 cm or more than 2.0 cm beneath the pleura, Hook-wire localization technique is used. During operation, initial actio

Machine learning for intraoperative prediction of viability in ischemic small intestine.

OBJECTIVE: Evaluation of intestinal viability is essential in surgical decision-making in patients with acute intestinal ischemia. There has been no substantial change in the mortality rate (30%-93%) of patients with acute mesenteric ischemia (AMI) since the 1980s. As the accuracy from the first laparotomy alone is 50%, the gold standard is a second-look laparotomy, increasing the accuracy to 87%-89%. This study investigates the use of machine learning to classify intestinal viability and histological grading in pig jejunum, based on multivariate time-series of bioimpedance sensor data. APPRO

An observational study showed that explaining randomization using gambling-related metaphors and computer-agency descriptions impeded randomized clinical trial recruitment.

OBJECTIVES: To explore how the concept of randomization is described by clinicians and understood by patients in randomized controlled trials (RCTs) and how it contributes to patient understanding and recruitment. STUDY DESIGN AND SETTING: Qualitative analysis of 73 audio recordings of recruitment consultations from five, multicenter, UK-based RCTs with identified or anticipated recruitment difficulties. RESULTS: One in 10 appointments did not include any mention of randomization. Most included a description of the method or process of allocation. Descriptions often made reference to gamblin

Memory accumulation mechanisms in human cortex are independent of motor intentions.

Previous studies on perceptual decision-making have often emphasized a tight link between decisions and motor intentions. Human decisions, however, also depend on memories or experiences that are not closely tied to specific motor responses. Recent neuroimaging findings have suggested that, during episodic retrieval, parietal activity reflects the accumulation of evidence for memory decisions. It is currently unknown, however, whether these evidence accumulation signals are functionally linked to signals for motor intentions coded in frontoparietal regions and whether activity in the putative

An fMRI study of risk-taking following wins and losses: implications for the gambler's fallacy.

Human decision-making involving independent events is often biased and affected by prior outcomes. Using a controlled task that allows us to manipulate prior outcomes, the present study examined the effect of prior outcomes on subsequent decisions in a group of young adults. We found that participants were more risk-seeking after losing a gamble (riskloss) than after winning a gamble (riskwin), a pattern resembling the gambler's fallacy. Functional MRI data revealed that decisions after riskloss were associated with increased activation in the frontoparietal network, but decreased activation i

Improved accuracy using recursive bayesian estimation based language model fusion in ERP-based BCI typing systems.

RSVP Keyboard™ is an electroencephalography (EEG) based brain computer interface (BCI) typing system, designed as an assistive technology for the communication needs of people with locked-in syndrome (LIS). It relies on rapid serial visual presentation (RSVP) and does not require precise eye gaze control. Existing BCI typing systems which uses event related potentials (ERP) in EEG suffer from low accuracy due to low signal-to-noise ratio. Henceforth, RSVP Keyboard™ utilizes a context based decision making via incorporating a language model, to improve the accuracy of letter decisions. To furth

Imaging characteristics of focal splenic and hepatic lesions in type 1 Gaucher disease.

In Gaucher disease (GD) imaging of liver and spleen is part of routine follow-up of GD patients. Focal lesions in both liver and spleen are frequently reported at radiological examinations. These lesions often represent benign accumulations of Gaucher cells, so-called "gaucheroma", but malignancies, especially hepatocellular carcinoma, are more frequently found in GD as well. We report the imaging characteristics of all focal lesions in liver and spleen in the Dutch GD cohort. Of the 95 GD1 patients, 40% had focal splenic and/or hepatic lesions, associated with more severe GD. Lesions identifi

Biochemical changes related to hypoxia during cerebral aneurysm surgery: combined microdialysis and tissue oxygen monitoring: case report.

OBJECTIVE AND IMPORTANCE: The objective of this study was to monitor brain metabolism on-line during aneurysm surgery, by combining the use of a multiparameter (brain tissue oxygen, brain carbon dioxide, pH, and temperature) sensor with microdialysis (extracellular glucose, lactate, pyruvate, and glutamate). The case illustrates the potential value of these techniques by demonstrating the effects of adverse physiological events on brain metabolism and the ability to assist in both intraoperative and postoperative decision-making. CLINICAL PRESENTATION: A 41-year-old woman presented with a Wor

External validation of the REDUCTION clinical decision aids for ruling out fractures in patients with midfacial and mandibular injuries in the emergency department: a protocol for a prospective multi-centre cohort study.

BACKGROUND: Computed Tomography (CT) is the gold standard for diagnosing midfacial and mandibular fractures in emergency department (ED) patients. However, its increasing availability has led to more frequent use, resulting in higher healthcare costs and unnecessary radiation exposure. Although various clinical parameters can assist in diagnosis of maxillofacial fractures, none are reliable enough to rule them out individually. To support clinical decision-making, and to reduce unnecessary imaging, the REDUCTION studies developed four decision aids based on combinations of clinical parameters.

Understanding the basis of treatment choices for varicose veins: a model for decision making with the repertory grid technique.

ObjectivesTo use the repertory grid technique as a method for identifying and rating the criteria that clinicians use to make a choice between the different treatment options for patients with a common condition such as varicose veins.DesignThe "expert panel" consensus method for rating the appropriateness of clinical procedures was modified with an existing psychometric method, the repertory grid technique. To identify the criteria used to decide about treatment, the panel members compared and contrasted a range of nine "treatment prototypes". They were then required to rate each criterion fo

Decision-making for endovascular treatment of a fusiform basilar trunk artery aneurysm with visualization of chronological blood mass distribution using computational fluid dynamics.

Fusiform basilar trunk artery aneurysms are rare lesions, to which stent-assisted coil embolization or flow diversion have been generally applied. When a stent is placed from the distal basilar artery or the posterior cerebral artery to the vertebral artery (VA) through a fusiform basilar artery aneurysm, the side of the VA may be determined according to its size, shape, accessibility and aneurysm projection. In the present report, we constructed stent-placement models from the distal basilar artery to either VA across the aneurysm, and investigated blood mass distribution using computational

Computer-Vision- and Edge-Enabled Real-Time Assistance Framework for Visually Impaired Persons with LPWAN Emergency Signaling.

In recent decades, various assistive technologies have emerged to support visually impaired individuals. However, there remains a gap in terms of solutions that provide efficient, universal, and real-time capabilities by combining robust object detection, robust communication, continuous data processing, and emergency signaling in dynamic environments. In many existing systems, trade-offs are made in range, latency, or reliability when applied in changing outdoor or indoor scenarios. In this study, we propose a comprehensive framework specifically tailored for visually impaired people, integra
