[Delirium in psychiatric consultation services].

BACKGROUND: Inquiries about delirium are a very common request in psychiatric consultation and liaison services (CL). OBJECTIVES: What practical procedures can be derived from current recommendations? METHODS: Evaluation of current guidelines and literature recommendations, as well as of the experiences in interdisciplinary planning of delirium management at various hospitals and of comments of participants in the author's workshops. RESULTS: Delirium is seen as a major burden in the treatment of seriously ill patients across all medical disciplines. There are national and international med…

Case Report: Prolonged Delirium Tremens.

Delirium tremens represents the most severe condition of alcohol withdrawal, with the associated highest mortality rate. The primary treatment for cases of delirium tremens consists of benzodiazepines. Within the literature, prolonged cases of delirium tremens have been identified that do not respond to high-dose benzodiazepine treatments or respond late. Different treatment modalities, such as propofol, dexmedetomidine, and parenteral antipsychotic administrations, are being attempted in the management of these cases. In this case, a case of prolonged delirium tremens with insufficient respon…

Alcohol Withdrawal with Delirium Tremens.

AUDIENCE: Emergency medicine (EM) residents (1st year and 2nd year levels), 4th year medical students and advanced practice providers. [Prevention of Post-operative Delirium].

Postoperative delirium (POD) is an adverse but often preventable complication of surgery and surgery-related anaesthesia, and increasingly prevalent. This article provides an overview on non-pharmacological preventive measures, divided into individualized and non-individualized measures. Non-individualized measures, such as the most minimally invasive surgical procedure, avoidance of unnecessary fasting before surgery, and the most tolerable anaesthesia are used to minimize the risk of POD in all patients. Based on the results of preoperative screenings for risk factors such as frailty or cogn…

[Pharmacological Management of Postoperative Delirium].

Postoperative delirium (POD) is a complex disorder with significant implications for health and well-being. Over the last few years, there has been a significant increase in awareness of the pathophysiological processes, the different clinical forms and the prevention of POD. It is known that POD develops when anaesthetic- and surgery-related precipitating factors coincide with the patient's predisposing vulnerability. Consequently, assessing the preoperative physical, cognitive, psychological, social and resilience capabilities of patients scheduled for surgery is critical to assessing overal…

Pediatric Delirium: An Overlooked Diagnosis?

BACKGROUND: Delirium is well-recognized in adult inpatient care. However, it is often overlooked in children, being mistaken for pain, anxiety, or age-appropriate agitation. METHODS: To assess the impact of a formal teaching session on the diagnostic rates and management of pediatric delirium (PD) in a tertiary care center, we conducted a retrospective chart review of all hospitalized children diagnosed with PD between August 2003 and August 2018 at the CHU Sainte-Justine (Montreal, Canada). Diagnostic incidence and management were compared before (2003 to 2014) and after (2015 to 2018) a for…

Identifying Medicare Beneficiaries With Delirium.

BACKGROUND: Each year, thousands of older adults develop delirium, a serious, preventable condition. At present, there is no well-validated method to identify patients with delirium when using Medicare claims data or other large datasets. We developed and assessed the performance of classification algorithms based on longitudinal Medicare administrative data that included International Classification of Diseases, 10th Edition diagnostic codes. METHODS: Using a linked electronic health record (EHR)-Medicare claims dataset, 2 neurologists and 2 psychiatrists performed a standardized review of E…

[Delirium in idiopathic Parkinson's disease].

BACKGROUND: The development of delirium in patients with idiopathic Parkinson's disease (IPD) is a feared complication, which is often associated with sustained worsening of motor symptoms and psychopathological sequelae. Little is known regarding the prevalence and incidence rates, course and prognosis. Clinical studies from which recommendations for evidence-based management of delirium in IPD can be derived are lacking. OBJECTIVE: To summarize the state of the art regarding epidemiological and clinical features of delirium in IPD. Discussion of prevention strategies and non-pharmacological…

[Severe delirium after GHB abuse].

BACKGROUND: The management of benzodiazepine-resistant GHB withdrawal requires careful consideration of GHB pharmacodynamics. CASE PRESENTATION: A young woman was admitted with tachycardia, confusion, agitation and delusions the day after attempting to quit a daily, high-dose GHB habit. A total of 225 mg of diazepam had no effect. She was sedated with propofol and intubated. An extubation attempt after 24 hours was followed by recurrence of delirium. After reintubation she required high doses of propofol, alfentanil and dexmedetomidine to maintain sedation for two days. Baclofen and diazepam …

Excited Delirium: A Systematic Review.

OBJECTIVE: We aimed to clarify the definition, epidemiology, and pathophysiology of excited delirium syndrome (ExDS) and to summarize evidence-based treatment recommendations. METHODS: We conducted a systematic literature search of MEDLINE, Ovid, Web of Knowledge, and Cochrane Library for articles published to March 18, 2017. We also searched the gray literature (Google Scholar) and official police or medical expert reports to complete specific epidemiologic data. Search results and full-text articles were independently assessed by two investigators and agreements between reviewers assessed w…

Altered Mental Status and Delirium.

Older patients who present to the emergency department frequently have acute or chronic alterations of their mental status, including their level of consciousness and cognition. Recognizing both acute and chronic changes in cognition are important for emergency physicians. Delirium is an acute change in attention, awareness, and cognition. Numerous life-threatening conditions can cause delirium; therefore, prompt recognition and treatment are critical. The authors discuss an organized approach that can lead to a prompt diagnosis within the time constraints of the emergency department.
