Outlook for a dengue vaccine.

Dengue is an increasing medical problem in subtropical and tropical countries. The search for a safe and effective vaccine is complicated by the fact that there are four types of dengue virus and that, if a vaccine is live attenuated, it should be proven not to cause the life-threatening form of dengue, dengue haemorrhagic fever. So far one vaccine candidate, a four-valent chimeric vaccine constructed from a yellow fever vaccine strain, has reached large clinical trials and has been shown to offer protection against dengue types 1, 3 and 54 but not against dengue type 2. It is highly likely th…

Corticosteroids for dengue infection.

BACKGROUND: Dengue is a common and important mosquito-borne viral infection. In many low- and middle-income countries it is endemic and is an important public health problem. Severe dengue is an important cause of death in children. There is no specific treatment for dengue, but observational studies suggest corticosteroids may have a benefit in dengue-related shock, and some people believe corticosteroids may prevent the progression to severe illness if given early in the course of the illness. OBJECTIVES: To compare treatment of dengue with and without use of corticosteroids or placebo in r…

Organic mania in dengue.

Dengue fever is considered to be one of the major health problems in south east Asia. In the recent past, epidemic outbreaks of Dengue have also been noticed in India. Initially, the neurological manifestations which were associated with Dengue received little attention, but now, there have been several reports of encephalitis and encephalopathy . However, nowhere in the literature has Dengue fever been mentioned as a cause of acute psychosis or mania. Here, we are reporting a 21-year-old male, a resident of Delhi, India who after the Dengue infection, developed an episode of classical mania.

Dengue and haemophagocytic lymphohistiocytosis.

Dengue is characterized by biphasic fever, myalgia or arthralgia, rash, leukopenia, and lymphadenopathy. It is self-limiting, and dengue-associated haemophagocytic lymphohistiocytosis has been reported in fewer than 20 children worldwide. We report the case of a 4-y-old boy with dengue who continued to have fever for 30 days, with hepatomegaly, thrombocytopenia, and leukopenia. Bone marrow examination showed haemophagocytes. The child was treated with steroids, instead etoposide and oral cyclosporine.

Controversies in dengue pathogenesis.

Research into the pathogenesis of dengue fever has exploded over the last half-century, with issues that were considered simple becoming more complex as additional data are found. This has led to the development of a number of controversies that are being studied across the globe and debated in the literature. In this paper, the following six controversies are analysed and, where possible, resolved: the 1997 World Health Organization (WHO) case definition of dengue haemorrhagic fever (DHF) is not useful; DHF is not significantly associated with secondary dengue infection; DHF results from infe…

Diagnosis of dengue: an update.

Early diagnosis of dengue, the most common mosquito-borne disease globally, remains challenging. Dengue presents initially as undifferentiated fever, with symptoms becoming more pathognomonic in the later stages of illness. This limits the timeliness in the delivery of appropriate supportive interventions. Laboratory tests are useful for diagnosis although the short-lived viremia and the presence of secondary infection with one of the four heterologous viral serotypes collectively complicate the choice and interpretation of laboratory tests. In this article, the authors review the various appr…

Quasispecies of dengue virus.

Pathogenic viruses have RNA genomes that cause acute and chronic infections. These viruses replicate with high mutation rates and exhibit significant genetic diversity, so-called viral quasispecies. Viral quasispecies play an important role in chronic infectious diseases, but little is known about their involvement in acute infectious diseases such as dengue virus (DENV) infection. DENV, the most important human arbovirus, is a causative agent of dengue fever (DF) and dengue hemorrhagic fever (DHF). Accumulating observations suggest that DENV exists as an extremely diverse virus population, bu…

[Dengue as haemorrhagic fever].

Dengue virus is distributed in tropical and subtropical regions and transmitted by mosquitoes of the genus Aedes. In September 2010 two cases of indigenous dengue fever were diagnosed in metropolitan France for the first time and next DENV infection was diagnosed in a German traveler returning from a trip to Croatia. The Aedes albopictus mosquitoes were found in several European countries (for example in greenhouses in Netherlands). The indigenous DENV infections in Europe are rare diseases, probably acquired after bites of infected mosquitoes imported by airplanes from endemic areas. Nonspeci…

Dengue in an elderly patient.

Although elderly populations are more exposed to the risk of getting dengue, the clinical peculiarities of this disease in this age range are not well known. This report is about an 80-year-old woman with dengue complications, self-medicated with salicylate. Literature indicates a more severe clinical condition, high hospitalization rate and significant mortality. This is caused by previous infections by other serotypes of this arbovirus, presence of chronic diseases, immunosenescence and high drug consumption, especially salicylates and the like. Analyses are required in a public health persp…

Stroke in patients with dengue.

BACKGROUND: Central nervous system infections are one of the common causes of stroke in developing countries. Stroke after dengue is uncommon. CASE REPORTS: A total of 1148 dengue cases were seen in a tertiary referral hospital during the epidemic from September to November 2008. We present 3 patients who had stroke caused by dengue. The first patient was a 45-year-old woman with dengue hemorrhagic fever who developed sudden right arm and leg weakness. Magnetic resonance imaging (MRI) revealed multiple hemorrhagic foci in the left parietal and temporal lobes. The second patient was a 35-year-…

[Indication for haemoderivatives in dengue].

Management of patients with dengue hemorrhagic fever (DHF) and dengue shock syndrome (DSS) requires especial care. It is based on physiological replacement therapy and fluid control. The use of blood products has its own criteria, especially during the disseminated intravascular coagulation (DIC) in the hemorrhagic phase. Monitoring bleeding manifestations and laboratory tests are needed. It has been shown that preventive transfusion does not have advantages in the treatment of this disease; on the contrary it increases the length of hospitalization and the development of pulmonary edema, amon…
