Eyelid dermatitis.

Cosmetic alteration of a patient's orbital skin is a common reason for professional consultation. This review presents the differential diagnosis and recommended evaluation of inflamed eyelids. To better understand the diseases, each is individually addressed clinically, pathogenetically, and therapeutically. It is critical to recognize the lesions correctly and to have full knowledge of the putative clinical disease process. An algorithm for an appropriate work-up for each disease is offered. With this background, a successful therapeutic response can be anticipated.

[Atopic dermatitis].

For the past decades, the prevalence of atopic dermatitis is on the rise. Three phases of the disease are recognized according to the age of the patient, either in the newborn age, in infancy or after puberty. A partial enzymatic defect in delta-6 desaturase has been reported. Such metabolic anomaly would be responsible for the alterations in both the skin barrier function and in the Th2 inflammatory reactions in part mediated through IgE. Some microorganisms colonizing the skin play an important causal role in the clinical exacerbations. The influence of food allergy on atopic dermatitis rema…

Contact dermatitis.

LEARNING OBJECTIVES: Reading this article will reinforce the reader's knowledge of the definition, pathophysiology, differential diagnosis, evaluation, and management of the most common of all the "eczemas," contact dermatitis, which can have an allergic and/or an irritant pathogenesis. DATA SOURCES: Relevant articles and current texts on contact dermatitis were referenced and reviewed. The personal experiences of the authors in an Environmental Medicine Clinic, their private practices, and their teaching of residents and other physicians were evaluated. A MEDLINE database using subject keywo…

Garlic dermatitis.

Allergic contact dermatitis to garlic usually has a typical clinical presentation but this is often masked if it presents concurrently with another form of hand dermatitis. Patch testing with 1% diallyl disulfide in petrolatum is recommended when garlic allergy is suspected.

Dermatitis herpetiformis.

Dermatitis herpetiformis (DH) is a relatively rare skin disorder with an estimated incidence of 1:10,000 in the UK. It is characterized by urticarial plaques and blisters on the elbows, buttocks, and knees, although other sites may also be involved. The eruption tends to be persistent: only 10-15% of patients have spontaneous remission over a 25-year study period. The disease is characterized by the presence of IgA deposits in the upper dermis of uninvolved skin and the diagnosis should not be made in the absence of these deposits. Two-thirds of patients have a small intestinal enteropathy wit…

Exfoliative dermatitis.

Exfoliative dermatitis or erythroderma is a clinical syndrome characterized by widespread erythema, fine or large scales, and desquamation of a significant portion of the body surface. Appropriate management of the patient during the acute phase of erythroderma requires a firm understanding of the pathophysiology and consequences of this clinical syndrome.

Footwear dermatitis.

We conducted a study to determine the prevalence and clinical patterns of footwear dermatitis, patch testing 50 patients with suspected footwear dermatitis and 30 controls with 22 allergens of a shoe series (Chemotechnique Diagnostics AB, Malmö, Sweden). The overall prevalence among contact dermatitis cases was 11.7%. The dorsa of the feet and toes were the commonest sites involved. 70% of patients showed sensitivity to footwear allergen(s), as opposed to 6.67% of controls. Potassium dichromate and colophony were the commonest sensitizers. Other sensitizers were the rubber accelerators 2-merca…

Contact dermatitis.

Contact dermatitis is a common inflammatory skin disorder caused by exposure to various antigens and irritants. It is associated with significant morbidity, and it is one of the most common reasons for worker's compensation claims for skin disease. Based on the mechanism by which contact allergy develops, several types are recognized, including allergic contact dermatitis, irritant contact dermatitis, contact photo-dermatitis and contact urticaria. The clinical presentation of contact dermatitis, regardless of the mechanism, ranges from localized vesicles and bullae on erythematous skin in acu…

Dermatitis herpetiformis.

The state of our understanding of the pathogenesis of DH relies on the integration of several key characteristics: (1) a high frequency of the HLA antigens HLA-B8, HLA-DR3, and HLA-DQw2, (2) an associated GSE, (3) the resolution of both the skin lesions and gut abnormalities in response to a gluten-free diet, and (4) the presence of granular deposits of IgA in normal and perilesional skin. The role of the HLA class II antigens expressed in patients with DH most likely relates to the afferent or initiating arm of the immune system. The association of the HLA-A1, -B8, -DR3, -DQw2 haplotype with

Yuletide dermatitis.

An unwelcome aspect of the festive season is contact dermatitis. This may be a gift of cosmetics, jewellery or clothing, or may be food related. It is important to be aware of the possible causative agents as the mainstay of management is withdrawal.

Contact dermatitis.

Allergic contact dermatitis and irritant contact dermatitis in the small animal may result in clinical lesions with similar anatomical locations. Allergic contact dermatitis is generally noted in ventral and lightly haired regions as a papular, erythematous, and sometimes vesicular dermatitis. It is intensely pruritic and is the result of many months (and usually years) of exposure to the contact allergen. Total avoidance of the allergenic substance is the only successful long-term management. Irritant contact dermatitis occurs when an irritating substance is applied to the skin. It is a seque…
