Keratinocytes in atopic dermatitis: inflammatory signals.

Atopic dermatitis (AD) is a chronic inflammatory skin disorder that usually predates the development of allergic airway disease. In most cases, this is thought to be an allergen-driven disease with prominent roles played by antigen presenting cells and effector Th2 cells. But keratinocytes, by virtue of their location, provide an important window to the environment and are also thought to contribute to the development of AD. In this review, we discuss several biologic attributes of keratinocytes that are relevant for AD: 1) intrinsic defects in barrier function, 2) production of inflammatory m

Ocular surface disease in atopic dermatitis.

PURPOSE: To describe the ocular surface disorders of 362 patients with severe active atopic dermatitis (AD) treated in the Ophthalmology Department of Kobe University Hospital and Kobe Rosai Hospital Eye Clinics during the period 1994-1996. METHODS: Routine ophthalmic examinations as well as tear film break-up time (BUT) and Schirmer tests were carried out. RESULTS: Lid eczema (65.7%), atopic keratoconjunctivitis, and superficial punctate keratopathy (67.5%) were the dominant ocular diseases in these patients. Tear function tests showed a BUT value of less than 10 seconds in 62.4% of the eye

Retinal detachment associated with atopic dermatitis.

BACKGROUND: Retinal detachment associated with atopic dermatitis, one of the most common forms of dermatitis in Japan, has markedly increased in Japan in the past 10 years. To clarify pathogenic mechanisms of retinal detachment in such cases, we retrospectively studied clinical characteristics of retinal detachment associated with atopic dermatitis. METHODS: We examined the records of 80 patients (89 eyes) who had retinal detachment associated with atopic dermatitis. The patients were classified into three groups according to lens status: group A, eyes with clear lenses (40 eyes); group B, ey

New therapeutic approaches in atopic dermatitis.

Novel approaches to the therapy for atopic dermatitis (AD) can now be considered because of recent advances in the pathogenesis of the disease. Several of these concepts are being tested in clinical trials. The effectiveness of cyclosporin A, plus reports of adoptive transfer of AD, or its clearing, following bone marrow transplantation, verify the criticalness of immune cells in AD pathogenesis. Thus, there is renewed interest in immunosuppressives, such as azathioprine and methotrexate, as well as new adhesion molecule and T-cell activation inhibitors. Therapy with the T-cell lymphokine, int

Atopic and seborrheic dermatitis: practical management.

Atopic dermatitis is common and causes sleep loss, a disfiguring appearance, an unpleasant odour, teasing, short stature and restriction of career choice in severe cases. There are no drugs which control the scratching; distraction, keeping the nails short and smooth, and the use of mittens at night are all helpful. The sedative action of the older H1 antihistamines makes them useful if scratching prevents a child from falling asleep. Emollients are useful for the associated skin dryness. The least potent topical steroids should be used sparingly to avoid the main hazards of skin atrophy, syst

Contact Dermatitis in Atopic Dermatitis Children-Past, Present, and Future.

Allergic contact dermatitis (ACD) used to be considered a rarity in children, but recently has been estimated to effect 4.4 million children in the USA alone, with a notable rise in investigative research in the field of pediatric ACD. Researchers have shown that patch testing is safe and effective in afflicted children and that those with atopic dermatitis (AD) have similar sensitization rates, although they have a higher sensitization to certain allergens, thought to be related to the inflammatory (IL-4) milieu. Patch testing assessment guidelines in children include five key considerations:

Comorbid Scenarios in Contact Dermatitis: Atopic Dermatitis, Irritant Dermatitis, and Extremes of Age.

Contact dermatitis (allergic and irritant) occurs when the skin encounters haptens that elicit a T cell-mediated hypersensitivity reaction (allergic) or a nonimmunologic, toxic reaction (irritant). Patch testing is the reference standard for diagnosing allergic contact dermatitis (ACD), although positive results are not always relevant. Therefore, the definitive diagnosis of ACD requires an astute clinician able to connect the results of patch testing appropriately with the clinical history and the cutaneous examination findings. Comorbid conditions such as atopic dermatitis can confound the a

Alteration in skin mycobiome due to atopic dermatitis and seborrheic dermatitis.

A microbiome consists of viruses, bacteria, archaea, fungi, and other microeukaryotes. It influences host immune systems and contributes to the development of various diseases, such as obesity, diabetes, asthma, and skin diseases, including atopic dermatitis and seborrheic dermatitis. The skin is the largest organ in the human body and has various microorganisms on its surface. Several studies on skin microbiomes have illustrated the effects of their composition, metabolites, and interactions with host cells on diseases. However, most studies have focused on the bacterial microbiome rather tha

A Practical Approach to Recalcitrant Face and Neck Dermatitis in Atopic Dermatitis.

Face and neck dermatitis in the atopic dermatitis patient is a diagnostically challenging entity with broad differential diagnoses. Recent case reports reporting face and neck dermatitis in patients on dupilumab therapy have added further complexity to diagnosis and management. Herein, we discuss a broad diagnostic algorithm and practical management strategy for recalcitrant face and neck dermatitis in the atopic patient with an emphasis on face and neck dermatitis associated with dupilumab therapy. Our aim is to raise awareness about the probable entity of drug-associated face and neck dermat
