Enoxacin photosensitivity.

A 71-year-old Korean farmer had pruritic erythematous patches on sun-exposed areas after enoxacin treatment for prostatitis. Suggestive features of photoallergy, such as moderate blurring of the margin and possible ectopic flare phenomenon, were reproduced by oral provocation test with enoxacin. Because all quinolones have structural similarities, and photosensitivity was observed in nalidixic acid and enoxacin, it would be advisable to warn all patients taking quinolones about photosensitivity.

Actinic dermatitis and phototoxic and idiopathic photodermatoses occur relatively often during childhood. Photodermatoses stemming from enzyme defects and genetic disorders are much less common, while photoallergic and chronic actinic dermatitis have not been dealt with to date. A specific sensitivity of a child's skin to light is often the first manifest clinical symptom of a photodermatosis, the key is to ensure early diagnosis. However, it is often difficult to reach a diagnosis, as the lesions are barely perceptible or have even healed by the time the patient is examined. This is why photo

Photoallergy is an entity of allergy in the dermatological field. Sunlight is involved in the pathophysiology of photoallergy. Photoallergy is classified into intrinsic and extrinsic diseases. As extrinsic diseases, photosensitivity due to drugs and photocontact dermatitis are important for physicians as well as dermatologists. Physicians should have accurate understanding of these disorders.

Genetically determined susceptibility to COX-2 inhibitors: a report of exaggerated responders to diclofenac 3% gel in the treatment of actinic keratoses.

Diclofenac 3% gel is an effective treatment for actinic keratoses (AKs) and is reported to be generally well tolerated with only mild local reactions. However, there is a subset of patients that seem to be susceptible to developing severe local reactions following application of diclofenac 3% gel. Although some of these reactions can be explained as being allergic contact dermatitis and/or photoallergic contact dermatitis, others cannot. We report a series of 10 patients who all developed severe local reactions following application of diclofenac 3% gel, despite negative diclofenac patch testi

Facial erythema as a result of benzophenone allergy.

Benzophenones are common causes of photoallergy and are increasingly used in products other than traditional sunscreens. Patients may be unaware of any sunscreen exposure when using a product such as shampoo containing benzophenone. Benzophenones also may produce photoallergic contact urticaria, in addition to delayed contact and photocontact dermatitis, which may complicate the clinical presentation. Allergy to benzophenone should be considered in the diagnosis of patients with patchy erythema of the face and neck that is not typically eczematous and that may otherwise be attributed to a rosa

Photopatch testing: Clinical characteristics, test results, and final diagnoses from the North American Contact Dermatitis Group, 2009-2020.

BACKGROUND: Photoallergic contact dermatitis (PACD) is a delayed hypersensitivity reaction to allergens only in the presence of ultraviolet radiation in sunlight. Photopatch testing (PhotoPT) is necessary to confirm the diagnosis of PACD. There are few published studies of PhotoPT in North America. OBJECTIVE: To summarise the results of patients photopatch tested by members of the North American Contact Dermatitis Group (NACDG), 2009-2020. METHODS: Retrospective analysis of patient characteristics and PhotoPT results to 32 allergens on the NACDG Photopatch Test Series. RESULTS: Most of the

Drug-induced Photosensitivity.

Ultraviolet radiation is considered the main environmental physical hazard to the skin. It is responsible for photoaging, sunburns, carcinogenesis, and photodermatoses, including drug-induced photosensitivity. Drug-induced photosensitivity is an abnormal skin reaction either to sunlight or to artificial light. Drugs may be a cause of photoallergic, phototoxic, and photoaggravated dermatitis. There are numerous medications that can be implicated in these types of reactions. Recently, non-steroidal anti-inflammatory drugs have been shown to be a common cause of photosensitivity. As both systemic

False-negative chemicals in ESR-based photosafety test (ESR-PT) and their significance for photosafety evaluations: examples of bithionol, fenticlor and cilnidipine.

The electron spin resonance (ESR)-based photosafety test (ESR-PT) is a non-animal prediction test for photosafety evaluations that can be used even for hydrophobic chemicals; the method is based on the detection of singlet oxygen generation using ESR spectroscopy and showing high accuracy for compounds with known photosafety information. During the process of extending the application data for ESR-PT, we found three false-negative chemicals: bithionol, fenticlor and cilnidipine. These chemicals did not show the characteristic triplet signal of 4-hydroxy-2,2,6,6-tetramethyl-piperidine-1-oxyl (4

Benzophenones.

Benzophenones are ultraviolet light filters that have been documented to cause a myriad of adverse cutaneous reactions, including contact and photocontact dermatitis, contact and photocontact urticaria, and anaphylaxis. In recent years, they have become particularly well known for their ability to induce allergy and photoallergy. Topical sunscreens and other cosmetics are the sources of these allergens in most patients, but reports of reactions secondary to use of industrial products also exist. Benzophenones as a group have been named the American Contact Dermatitis Society's Allergen of the

Ketoprofen-induced Photoallergic Reaction.

Dear Editor, Photoallergic reactions are classic T-cell-mediated or delayed-type hypersensitivity reactions of the skin in response to a photoallergen (or a cross-reacting chemical) to which a subject was sensitized in the past (1). The immune system recognizes the changes caused by ultraviolet (UV) radiation; it produces antibodies and causes inflammation of the skin in the exposed areas (2). Common photoallergic drugs and ingredients are included in some sunscreens, aftershave lotions, antimicrobials (especially sulfonamides), non-steroidal anti-inflammatory drugs (NSAIDs), diuretics, antico

Lime Dermatitis.

Phytophotodermatitis are skin eruptions caused by contact with a plant substance and exaggerated by exposure to sunlight. They are due to the presence of furocoumarins (psoralens, xanthotoxins, and bergaptenes) in the plant and involve 2 mechanisms of skin photosensitivity: phototoxicity or photoallergy.

What is the relationship between photosensitizing drugs and skin cancer?

Many medications are associated with phototoxicity or photoallergy, the two types of photosensitivity. Recently, a warning related to increased skin cancer risk was added to the labeling of the popular diuretic hydrochlorothiazide. This article reviews some photosensitizing medications and describes patient education on preventing and recognizing photosensitivity reactions and skin cancer.

Tiaprofenic acid induced photosensitivity.

45 patients (34 patients with PLE, 11 with phototoxic or photoallergic dermatitis) were photopatch tested to tiaprofenic acid. In 23 patients (51%), phototoxic reactions were found. Positive reactions appeared within 0.5 to 48 h of irradiation. Although each patient was irradiated with 2 different UVA light sources, with significantly different energy outputs (10 J/cm2 against 72 J/cm2), the intensity and duration of positive reactions was virtually the same. We found a considerably higher rate of positive reactions than other investigators, independent of the light energy delivered. None of t

Contact allergy to topical corticosteroids and sunscreens.

Topical corticosteroids and sunscreens are extensively used formulations, both as over-the-counter products and as prescription medicines. Topical corticosteroids are increasingly being recognized as causes of allergic contact dermatitis. Because of their anti-inflammatory property, contact allergy to these agents may be difficult to suspect and prove. With corticosteroid allergy, there are special issues in patch testing that need to be considered: Screening tests need to be done with budesonide and tixocortol pivalate, and delayed readings are essential to pick up all positive cases. Prevent

[Cutaneous side-effects of nonsteroidal anti-inflammatory drugs (NSAID)].

NSAID are able to induce cutaneous side-effects by both systematical and topical application. Nearly all kinds of exanthema are possible by any type of these drugs. However, particular substances are more likely to induce certain drug eruptions; aspirin and indometacine may induce urticarial reactions, whereas piroxicam can lead to phototoxic or photoallergic dermatitis. Contact dermatitis induced by topical NSAID is still rare but increasing. Ketoprofen and bufexamac were major contact allergens based on the number of reports, but local differences among different countries were observed. The
