Interplay between diaphragm thickening, force generation and lung volume change: a physiological study in health and diaphragm weakness.

INTRODUCTION: Diaphragm ultrasound is increasingly used to assess diaphragm function. Diaphragm thickening (DTF) is widely interpreted as a surrogate of diaphragm force generation. However, the independent contributions of force generation and lung volume change to diaphragm thickening remain unclear. This study aimed to determine the interplay between DTF, force generation and lung volume change. METHODS: Healthy participants and patients with a congenital myopathy performed unloaded concentric contractions, isometric contractions and loaded concentric contractions. Mouth pressure (Pmo), vol…

Daily hyperbaric oxygen therapy increases diaphragm electromyographic activity and alters diaphragm gene expression after cervical spinal cord injury.

Hyperbaric oxygen (HBO) therapy can enhance motor recovery when initiated shortly after spinal cord injury (SCI). HBO can also induce a biphasic ventilatory response that includes hypo- and hyperventilation, but this has not been evaluated after SCI. We studied adult Sprague-Dawley rats with cervical SCI to determine how an acute bout of HBO impacts diaphragm activation and to test the hypothesis that daily HBO treatment can accelerate recovery of diaphragm activation. Indwelling electromyogram (EMG) electrodes were placed in the mid-costal diaphragm prior to lateral mid-cervical contusion inj…

Continuous On-Demand Diaphragm Neurostimulation to Prevent Diaphragm Inactivity During Mechanical Ventilation: A Phase 1 Clinical Trial (STIMULUS).

Rationale: Diaphragm inactivity during invasive mechanical ventilation may predispose the lung and diaphragm to injury and is associated with adverse clinical outcomes. Objectives: Assess the feasibility of continuous on-demand diaphragm neurostimulation-assisted mechanical ventilation to maintain diaphragm activity in the absence of respiratory drive for at least 24 hours of mechanical ventilation. Methods: In a single-center phase 1 clinical trial, patients receiving invasive mechanical ventilation for acute hypoxemic respiratory failure or after thoracic surgery underwent transvenous diaphr…

Temporal evolution of diaphragm thickness and diaphragm excursion among subjects hospitalized with COVID-19: A prospective observational study.

BACKGROUND: The severe acute respiratory syndrome coronavirus-2 (SARS-CoV-2) has an affinity for the angiotensin-converting enzyme 2 (ACE2) receptors, which are present abundantly on the diaphragm. This study aims to describe temporal changes in diaphragmatic thickness and excursion using ultrasonography in subjects with acute COVID-19. METHODS: This prospective observational study included adults hospitalized with COVID-19 in the past 48 hours. The diaphragm thickness at end-expiration (DTE), diaphragm thickening fraction (DTF), and diaphragm excursion during tidal breathing (DE) and maximal…

Diaphragm muscle weakness in adults with diabetes.

BACKGROUND: Diabetes-related pulmonary dysfunction, including impaired spirometry and pulmonary gas exchange, is well characterized. However, data on diaphragm muscle weakness(DMW), a predictor of poor clinical outcomes, are limited. OBJECTIVES: We report the percentage change in forced vital capacity(FVC) from sitting to supine position(∆FVC), a surrogate measure of DMW, in adults with diabetes. METHODS: This cross-sectional study included 537 adults with diabetes without primary pulmonary disease managed at Ghana's National Diabetes Management and Research Centre. Spirometry was conducted …

A Case Series on Unilateral Elevation of Diaphragm.

We report a case series of seven patients aged 24 to 78 years who presented with unilateral elevation of the diaphragm secondary to diverse underlying mechanisms. The diagnoses included sarcoidosis-related pulmonary inflammation, gallbladder adenomyomatosis with sepsis and metabolic derangement, pulmonary tuberculosis with Chilaiditi's syndrome, acute pulmonary embolism with diaphragmatic eventration, superior vena cava syndrome due to non-small cell lung carcinoma, cryptogenic organizing pneumonia with an incidental thymic cyst and pyogenic liver abscess. This series emphasizes that unilatera…

Diaphragm Ultrasound: State-of-the-Art Review.

Diaphragm ultrasound has become a widely adopted bedside tool for assessing respiratory muscle function in both acute and chronic respiratory failure. Initially introduced as a descriptive imaging technique, it is now increasingly used to support physiological interpretation, prognostic evaluation, and monitoring during invasive and noninvasive respiratory support, as well as in chronic respiratory diseases. In clinical practice, diaphragm ultrasound findings are often interpreted using isolated threshold values to classify diaphragm function as normal or abnormal, an approach that may inadequ…

The kidney slit diaphragm resembles a fishnet.

INTRODUCTION: An essential component of the kidney filtration barrier, the slit diaphragm, is a specialized cell-cell junction composed of the cell adhesion molecules Nephrin and Neph1. METHODS: Here, we use cryo-electron tomography of vitreous lamellae from high pressure frozen wild-type C57BL/6J native mouse glomeruli. RESULTS: The cryo-electron tomograms show that the slit diaphragm resembles a fishnet-like architecture. After sub-tomogram averaging and molecular dynamics flexible fitting of Nephrin and Neph1, the resulting molecular model of the slit diaphragm reveals a criss-cross arran…

Ultrasound Evaluation of the Diaphragm in Clinical Anesthesia.

When the human body is anesthetized, the human nerve tissue will be greatly affected, which also affects the breathing of the human body. The respiration during anesthesia is a lack of initiative, and the energy efficiency of the diaphragm in the lungs is very important to the safety of anesthesia. In this paper, the application of the ultrasound evaluation of the diaphragm in clinical anesthesia was studied. In this paper, 24 patients who underwent lung examination under medical anesthesia at our hospital were evaluated by the ultrasound vertical mixed echo method. Through patient voluntary s…

Obstructive Sleep Apnea and Role of the Diaphragm.

Obstructive sleep apnea (OSA) causes multiple local and systemic pathophysiological consequences, which lead to an increase in morbidity and … her muscles related to dilating the upper airways. Sparse research has been published regarding the diaphragm muscle, which is the main muscle structure to insufflate air into the airways. The article reviews the functional anatomy of the muscles used to open the upper respiratory tract and the non-physiological adaptation that follows in the presence of OS …

[Diseases of the pleura, thoracic wall and diaphragm].

In the case of lesions of the pleura, the thoracic wall and the diaphragm, chest x‑ray is of great relevance in everyday clinical practice. In many clinical situations, it is already sufficient for the initial diagnosis or for monitoring the course of the disease; these include, for example, pleural effusion and pneumothorax. In some cases, however, supplementary cross-sectional imaging may be necessary, e.g., if there is a possible pleural empyema or if the soft tissues of the mediastinum and pleural wall are also affected. Further diagnostic workup is also necessary for more advanced questio…

Functional evaluation of the diaphragm with a noninvasive test.

Cardiac surgery with median sternotomy causes iatrogenic damage to the function of the diaphragm muscle that is both temporary and permanent. Myocardial infarction itself causes diaphragmatic genetic alterations, which lead the muscle to nonphysiological adaptation. The respiratory muscle area plays several roles in maintaining both physical and mental health, as well as in maximizing recovery after a cardiac event. The evaluation of the diaphragm is a fundamental step in the therapeutic process, including the use of instruments such as ultrasound, magnetic resonance imaging (MRI), and compute…

Diaphragm Muscle Adaptations in Health and Disease.

Breathing is achieved without thought despite being controlled by a complex neural network. The diaphragm is the predominant muscle responsible for force/pressure generation during breathing, but it is also involved in other non-ventilatory expulsive behaviors. This review considers alterations in diaphragm muscle fiber types and the neural control of the diaphragm across our lifespan and in various disease conditions.

[Ultrasonographic evaluation of diaphragm structure and function].

Diaphragm is the principal respiratory muscle. Diaphragm dysfunction resulting from mechanical ventilation, chronic cardiopulmonary disease, medicine, cardio-thoracic surgery, septic shock and chronic malnutrition predisposes to weaning failure from mechanical ventilation and prolongs intensive care unit (ICU) stay. Traditionally, the method of assessment of diaphragm structure and function was invasive, lacking specific, and was difficult to be used bedside. A number of recent studies have been suggested that ultrasonographic evaluation has been proposed as an accurate, safe and noninvasive m…

Airflow limitation is accompanied by diaphragm dysfunction.

Chronic airflow limitation, caused by chronic obstructive pulmonary disease (COPD) or by asthma, is believed to change the shape and the position of the diaphragm due to an increase in lung volume. We have made a comparison of magnetic resonance imaging (MRI) of diaphragm in supine position with pulmonary functions, respiratory muscle function and exercise tolerance. We have studied the differences between patients with COPD, patients with asthma, and healthy subjects. Most interestingly we found the lung hyperinflation leads to the changes in diaphragmatic excursions during the breathing cycl…

A RARE CASE OF RIGHT DIAPHRAGM AGENESIS.

Diaphragmatic agenesis is the most extreme and rare form of congenital diaphragmatic maldevelopment and therefore diagnosis of it are only few in literature, especially in children. It may be unilateral or bilateral. Frequently, diaphragmatic agenesis is accompanied with diaphragmatic hernia, when through a congenital defect of the diaphragm liver, intestine and other viscera herniate into the chest. Diagnostics typically are made early in infancy and is generally associated with other genetic anomalies, especially aneuploidy syndromes. It is associated with a high mortality, if not treated in…
