Blood oxygen transport and depletion in diving emperor penguins.

Oxygen store management underlies dive performance and is dependent on the slow heart rate and peripheral vasoconstriction of the dive response to control tissue blood flow and oxygen uptake. Prior research has revealed two major patterns of muscle myoglobin saturation profiles during dives of emperor penguins. In Type A profiles, myoglobin desaturated rapidly, consistent with minimal muscle blood flow and low tissue oxygen uptake. Type B profiles, with fluctuating and slower declines in myoglobin saturation, were consistent with variable tissue blood flow patterns and tissue oxygen uptake dur

Depth of SCUBA Diving Affects Cardiac Autonomic Nervous System.

The present study investigated the influence of SCUBA dives with compressed air at depths of 10 and 20 m on ECG-derived HRV parameters in apparently healthy individuals. We hypothesized that cardiac sympathetic activity (measured by HRV parameters) adapts proportionally to diving depth, and that both time- and frequency-domain parameters are sensitive enough to track changes in cardiac ANS function during diving activities and subsequently during the recovery period. Eleven healthy middle-aged recreational divers (nine men and two women, age 43 ± 8, all nonsmokers) volunteered to participate i…

Carbon monoxide poisoning while scuba diving: a rare event?

Contamination of breathing gas is a risk for all divers. Some hydrocarbon contaminants will be sensed by the diver and the dive profile aborted. On the contrary, carbon monoxide may not be recognized by the diver and catastrophic consequences can result. Reported here is the fatal case of carbon monoxide poisoning while scuba diving, an event that has rarely been reported in the medical literature. A detailed review of other published cases of CO poisoning while scuba diving is included, attempting to identify causes in common and propose methods of prevention.

The Mammalian Diving Response: Inroads to Its Neural Control.

The mammalian diving response (DR) is a remarkable behavior that was first formally studied by Laurence Irving and Per Scholander in the late 1930s. The DR is called such because it is most prominent in marine mammals such as seals, whales, and dolphins, but nevertheless is found in all mammals studied. It consists generally of breathing cessation (apnea), a dramatic slowing of heart rate (bradycardia), and an increase in peripheral vasoconstriction. The DR is thought to conserve vital oxygen stores and thus maintain life by directing perfusion to the two organs most essential for life-the hea…

Serum tau concentration after diving - an observational pilot study.

INTRODUCTION: Increased concentrations of tau protein are associated with medical conditions involving the central nervous system, such as Alzheimer's disease, traumatic brain injury and hypoxia. Diving, by way of an elevated ambient pressure, can affect the nervous system, however it is not known whether it causes a rise in tau protein levels in serum. A prospective observational pilot study was performed to investigate changes in tau protein concentrations in serum after diving and also determine their relationship, if any, to the amount of inert gas bubbling in the venous blood. METHODS: S…

Physiological and Genetic Adaptations to Diving in Sea Nomads.

Understanding the physiology and genetics of human hypoxia tolerance has important medical implications, but this phenomenon has thus far only been investigated in high-altitude human populations. Another system, yet to be explored, is humans who engage in breath-hold diving. The indigenous Bajau people ("Sea Nomads") of Southeast Asia live a subsistence lifestyle based on breath-hold diving and are renowned for their extraordinary breath-holding abilities. However, it is unknown whether this has a genetic basis. Using a comparative genomic study, we show that natural selection on genetic vari…

Edmund Goodwyn and the first description of diving bradycardia.

Diving bradycardia is a primordial oxygen-conserving reflex by which the heart rate of air-breathing vertebrates, including humans, slows down in response to water immersion. Its discovery is attributed to Paul Bert, whose seminal observation was published in 1870 as part of a series of experiments that examined physiological adaptations to asphyxia in ducks and other animals. However, Edmund Goodwyn, a British physician who studied medicine at the University of Edinburgh, had already described this reflex in his doctoral thesis, which was originally published in Latin in 1786 and again in Eng…

Cognitive control of heart rate in diving harbor porpoises.

Marine mammals have adapted to forage while holding their breath in a suite of aquatic habitats from shallow rivers to deep oceans. The key to tolerate such extensive apnea is the dive response, which comprises bradycardia and peripheral vasoconstriction. Although initially considered an all-or-nothing reflex [1], numerous studies on freely diving marine mammals have revealed substantial dynamics of the dive response to meet the impending dive demands of depth, duration and exercise [2]. Such adjustments are not only autonomic responses, but are under acute cognitive control in pinnipeds [3] l…

Persistent (patent) foramen ovale (PFO): implications for safe diving.

Diving medicine is a peculiar specialty. There are physicians and scientists from a wide variety of disciplines with an interest in diving and who all practice 'diving medicine': the study of the complex whole-body physiological changes and interactions upon immersion and emersion. To understand these, the science of physics and molecular gas and fluid movements comes into play. The ultimate goal of practicing diving medicine is to preserve the diver's health, both during and after the dive. Good medicine starts with prevention. For most divers, underwater excursions are not a professional nec…

Diving-related deaths in Hong Kong waters, 2006-2009.

Diving is a popular recreational activity in Hong Kong, and there is an associated incidence of mortality. This individual case review of reported diving-related deaths occurring in Hong Kong waters between 2006-2009 inclusive, was conducted as part of the ongoing DAN Asia-Pacific dive fatality reporting project. The eight reported deaths involved one snorkeler and seven scuba divers. Six of the victims were male and two were female. The disabling injury in at least one death, possibly two, appeared to have been cardiac-related; two involved trauma from impact with boats; and asphyxia was beli…

Scuba diving and pregnancy: can we determine safe limits?

No human data, investigating the effects on the fetus of diving, have been published since 1989. We investigated any potential link between diving while pregnant and fetal abnormalities by evaluating field data from retrospective study No.1 (1990/2) and prospective study No.2 (1996/2000). Some 129 women reported 157 pregnancies over 1,465 dives. Latest gestational age reported while diving was 35 weeks. One respondent reported 92 dives during a single pregnancy, with two dives to 65 m in the 1st trimester. In study No.2 >90% of women ceased diving in the 1st trimester, compared with 65% in the…

Supraventricular tachycardia in infants: use of the "diving reflex".

The effectiveness of the "diving reflex" in treating paroxysmal supraventricular tachycardia (PSVT) in 10 infants aged under 6 months is demonstrated. No rhythm disturbances were encountered during the maneuver, even in patients receiving digitalis. The recommended water temperature is 5 degrees C and the total time of facial immersion, between 6 and 7 seconds. The diving reflex might be considered the first therapeutic approach in infants under 6 months old with PSVT.

Use of the "diving reflex" in paroxysmal atrial tachycardia.

Immersing the patient's face in cold water to induce the "diving reflex" is a quick and simple method of treating paroxysmal supraventricular tachycardia. Patients can be trained to perform the "diving reflex" at home. There are many advantages to this method of treatment when compared to many of the other therapies. Polyuria up to 3 L is often noted during the first 90 minutes of a paroxysmal tachycardia. The diuresis is thought to be due to atrial distention with resultant inhibition of antidiuretic hormone secretion.

Advances in the delivery of cardiopulmonary resuscitation in a diving bell.

This commentary discusses the provision of cardiopulmonary resuscitation to casualties in a diving bell. This single resource consolidates recent advances in the field, published in different medical journals, to support dissemination across the wider diving industry. It summarises the evaluation of techniques for the provision of manual cardiopulmonary resuscitation (CPR) to a seated casualty, including head-to-chest, knee-to-chest, and prone knee-to-chest compression delivery, and concludes that the only safe and potentially effective approach in a diving bell setting without room for a supi…
