Valley fever: danger lurking in a dust cloud.

Coccidioides immitis and Coccidioides posadasii contribute to the development of Valley Fever. The ability of these fungal pathogens to evade the host immune system creates difficulty in recognition and treatment of this debilitating infection. In this review, we describe the current knowledge of Valley Fever and approaches to improve prevention, detection, and treatment.

Mixed dust pneumoconiosis occurring in an unusual setting.

Mixed dust pneumoconiosis secondary to domestic wood smoke exposure is a cause of pneumoconiosis in women from developing countries, but is rarely seen in the USA. An elderly female non-smoker, who immigrated to the USA from Pakistan 10 years previously, presented with a worsening non-productive cough and dyspnoea on exertion. She did not have any occupational or environmental exposures other than utilising firewood for cooking while living in Pakistan. Radiographs revealed multiple bilateral pulmonary nodules associated with hilar and mediastinal adenopathy. A video-assisted thoracoscopic bio…

Airborne Asian sand dust enhances murine lung eosinophilia.

There is no experimental study demonstrating the effects of airborne Asian sand dust (AASD) on allergic lung eosinophilia. The organic substances adsorbed onto AASD collected from the atmosphere of Iki-island in Japan were excluded by heat treatment at 360°C for 30 min. The effects of AASD or heated-AASD (H-AASD) towards allergic lung inflammation were compared in murine lungs to investigate the role of organic substances. ICR mice were administrated with the two kinds of AASD and/or ovalbumin (OVA) intratracheally four times at 2-week intervals. AASD and H-AASD enhanced eosinophil recruitment…

Tuberculosis among workers exposed to free silica dust.

Tuberculosis (TB) is a global emergency. Poverty, increasing migration, homelessness and the compulsions to live and work in high-risk environments are making people increasingly susceptible to the disease. Those working in mines, construction work, stone-crushing and in other similar occupations where there is a greater level of exposure to silica dust are specially vulnerable. Though the association between TB and silicosis has been firmly established by the results of epidemiologic studies no attempts have been made to study the epidemiological features of radiologically active TB among fre…

Occupational asthma caused by Ipe (Tabebuia spp) dust.

BACKGROUND: Ipe is a resistant hardwood that contains naphtoquinones. It is easily found and frequently used in South and Central America. Naphtoquinones are skin sensitizers. OBJECTIVE: To describe a case of occupational asthma related to Ipe wood dust. METHODS: The patient was submitted to a clinical evaluation consisting of a respiratory symptom questionnaire, occupational history, serial measurements of lung function by spirometry, skin prick tests, patch tests, specific IgE and specific bronchial provocation tests to Ipe dust. RESULTS: Serial lung function measurements showed sustained…

Dust-induced interstitial lung disease in the tropics.

Inhalation of dusts is an important cause of interstitial lung disease in the tropical countries such as India. While dusts of organic origin, such as the cotton dust causing byssinosis, generally cause bronchial or bronchiolar involvement and hypersensitivity pneumonitis, inorganic metallic dusts cause progressive pulmonary fibrosis. Silicosis, coal workers' pneumoconiosis, and asbestosis are the three most commonly recognized forms of pneumoconiotic pulmonary fibrosis. Pulmonary tuberculosis is an important complication seen in up to 50% of patients of silicosis in some reports from India. T…

[Sideroelastosis of pulmonary vessels after welder dust exposure].

Sideroelastosis pulmonum was first described by Ceelen (1931) and Gellerstedt (1939), mentioning an idiopathic type, but also vascular alterations in chronic pulmonary congestion. We analysed the question wether secondary sideroelastosis can be demonstrated also in lungs after previous welder dust exposure. Lung samples of 43 patients and deceased with various exposure periods to welder dust were investigated. On each sample, light microscopical, scan electron microscopical and energy dispersive x-ray micro analyses (EDX) were performed. In six samples (14%) vascular alterations in the area of…

Sinonasal cancer and occupational exposure to textile dust.

Data from a case-control study conducted at 27 hospitals in France in 1986-88 were analyzed to examine the association between exposure to textile dust and sinonasal cancer. The study included 207 cases and 409 controls. Detailed information on occupational history and other potential risk factors for sinonasal cancer was collected. Exposure to textile dust (probability and level of exposure, type of textile fiber) was assessed by an expert in industrial hygiene. Among women, exposure to textile dust was associated with an elevated risk of squamous cell carcinoma (odds ratio (OR) = 2.45, 95% c…

Occupational exposure to dust in slate pencil manufacture.

Slate pencils are manufactured from natural rock known as Binota Shale in small factories. Since the dust generated by stone-cutting and groove-making machines during the process contains a very high percentage of free silica and the particles are of sizes ranging up to a few microns in diameter the exposure both to respirable and to total inhalable airborne dust was assessed. Dust sizing revealed that all of the dust was respirable (less than 2.5 microns). Measurement of the intake velocity of the exhaust system in many cases showed it to be less than 10 m s-1. Suggestions are made for improv…

Carcinogenic effects of wood dust: review and discussion.

Occupational exposure to wood dust (alone or chemically treated) is associated with an increased risk of developing adenocarcinoma of the nasal cavity. The specific causative agents, i.e., wood dust alone (natural products), wood dust with additives used in the processing or manufacturing of wood products, and/or physical determinants of wood dust and the associated risk factors, are not known or understood. The strongest association of exposure to wood dust and development of nasal cancer is observed in those occupations where workers are exposed to hard wood dust and chemical additives are n…

[Dust deposits in lung tumors--cause and significance].

In the diagnosis of a so-called scar carcinoma, one of the accepted morphological characteristics is the deposition of dust in tumours of the lung. In 50 bronchial carcinomas in former miners and patients not occupationally exposed to dust, the cause and significance of such lesions were investigated in autopsy and surgical material. Impregnation with dust must be considered to be a secondary phenomenon of the infiltrative tumour growth. Such depositions depend upon previously present dust depositions in the pulmonary parenchyma and on the localisation of the tumour, and reveal a clear correla…

[Estimation of pulmonary dust load using magnetic pneumography].

If an occupational history enables the identification of subjects at high risk from occupational disease, it yields no information on the individual dust level on account of very important inter-individual variations in pulmonary dust retention. Magnetopneumography is the only technique which enables pulmonary dust loads to be estimated on living subjects. The first measures of dust load by magnetopneumography made at the Ambroise Pare Hospital are reported. By knowing the magnetic properties of occupationally inhaled aero-contaminants, this technique is sensitive and non-invasive and enables …

Induction of thoracic malignancy in inorganic dust pneumoconiosis.

Pulmonary carcinoma is now the leading cause of death due to cancer in men and women. Aside from cigarette smoking, occupational exposure to carcinogens is the most important cause of lung cancer, accounting for up to one third of all cases. The following article is a review of occupationally induced thoracic neoplasms with an emphasis on those related to the inhalation of inorganic dust. After introducing some basic terms, describing the research methods, and reviewing the process of carcinogenesis, current information on the relationship between exposure to asbestos, synthetic mineral fibers…

[House-dust mites as vectors of human diseases].

The genuine European house-dust mite, Dermatophagoides pteronyssinus, is the producer of the most common and most potent allergen in the house-dust in Central Europe. Such allergens causing asthma, rhinitis, tracheitis, bronchitis and/or conjunctivitis in man may also be produced by the other species of house-dust mites such as D. farinae and Euroglyphus maynei. House mites and mites of stored products rarely account for such diseases but more frequently for acarodermatitis--especially on farms. Allergies due to house-dust mites mainly occur in midsummer and in early autumn. Particularly child…

Pulmonary asbestos and dust content in East Anglia.

Measurements were made of the asbestos fibre and dust content of samples from 96 surgically excised lungs; 42 necropsies on patients with lung cancer, 11 necropsies on patients with non-pulmonary malignancies, and 59 necropsies on patients without any malignant disease. The patients' ages ranged from 45 to 74 years at the time of study. None of the patients had asbestosis. The distribution of fibres and dust content of the lungs showed a log-normal distribution. There was no significant difference in fibre counts or dust content between men and women, and between lung cancer and non-cancer pat…

Cancer incidence and exposure to iron oxide dust.

Iron oxides, particularly hematite, have been suspected for carcinogenic properties based on epidemiological observations and experimental data. This case-control (case-referent) study concerns a worker population with high exposure to dust of iron oxides, particularly hematite, and with some impurities of pentavalent arsenic and other metals. In spite of the high exposure levels no excess of cancer has been observed, either in the respiratory system or at other sites. It is suggested that earlier findings of respiratory cancers as possibly associated with iron oxides exposure might rather be …

Recurrent nocturnal asthma after exposure to grain dust.

Bronchial provocation tests performed on two seperate occasions in a farmer with dust from his own grain led to immediate and late respiratory reactions followed, without further exposure to grain dust, by severe respiratory reactions during subsequent nights. Studies of lung function, including measurements of closing volume, suggested that both large and small airways rather than lung parenchyma were primarily involved. Examination of the grain samples together with the results of skin and serologic tests suggested that the grain mite, Glycyphagus destructor, might be an important allergen. …
