A decision-analysis model was developed to estimate the effects and costs of alternative initial management strategies for women presenting with dysuria and pyuria. We compared days of morbidity and direct medical costs associated with single-dose and multiple-dose regimens of amoxicillin and trimethoprim-sulfamethoxazole and examined the cost-effectiveness of doing an initial urine culture. Two heifer calves with dysuria were presented to the Large Animal Clinic of the Western College of Veterinary Medicine. Although no external indications of omphalitis were present, urachal abscessation, resulting in impeded urinations, was highly suspect due to the age of the animals. Intravenous contrast urography showed abnormal bladder position in both cases and consequently afforded a presurgical diagnosis. The cases and the diagnostic techniques employed are discussed.

Eighty-seven Egyptians with Schistosoma haematobium infection (current or past) and chronic dysuria seen at the Theodor Bilharz Research Institute outpatient urology clinic in Imbaba, Cairo, Egypt were investigated for Chlamydia trachomatis infection. Recent research into the pathogenesis and treatment of urinary tract infection (UTI) clearly indicates that our traditional approach has overestimated the need for extensive evaluation and prolonged antibiotic therapy. The great majority of UTIs can be managed adequately with urinalysis and single-dose or three-day antibiotic regimens; only complicated UTIs or those occurring in unusual hosts require cultures and longer courses of treatment.

Urethral malignant tumors are rare and can lead to stenosis, causing dysuria. We report a case of urethral metastasis secondary to esophageal cancer. At the time of diagnosis, a patient with esophageal squamous cell carcinoma presented with voiding difficulties, feeble stream, terminal dribbling and incomplete voiding. INTRODUCTION: Patients in the emergency department may experience sudden decompensation despite initially appearing stable.
CASE PRESENTATION: A 37-year-old transgender man presented to the emergency department (ED) with several months of fevers, myalgias, and weight loss. The patient acutely became febrile, tachycardic, and hypotensive after an initially reassuring assessment in the ED.
DISCUSSION: This case takes the reader through the differential diagnosis and work-up of the decompensating patient initially presenting with subacute symptoms.
