Tennis elbow tendinosis (epicondylitis).

Tennis elbow tendinosis (epicondylitis) is most commonly caused by tendon overuse and failed tendon healing. The pathoanatomy of overuse tendinopathy is noninflammatory "angiofibroblastic tendinosis." The specific areas of elbow abnormality include the extensor carpi radialis brevis-extensor digitorum communis complex laterally, the pronator teres, flexor carpi radialis medially, and triceps posteriorly. The primary goal of nonsurgical treatment is to revitalize the unhealthy tissue that produces pain. Successful nonsurgical treatment comprises rehabilitative resistance exercise and progressio

Elbow arthropathy in hemophilia.

Twenty-one affected elbows in 14 known hemophilic patients undergoing treatment at the Hemophilia Clinic of our institution between 1994 and 1997 were evaluated using the clinical evaluation score of the Orthopaedic Advisory Committee of the World Federation of Hemophilia and radiological examination using the Pettersson score. The mean age of patients was 16.2 years (range 10-25 years), and all patients except 1 had severe hemophilia with factor VIII levels less than 1% of normal. The mean duration of disease at the time of presentation was 12.8 years. The mean clinical score was 6, while the

Imaging of elbow disorders.

Numerous modalities are available for imaging the elbow. Radiographs should be the first imaging procedure performed for evaluation of an elbow abnormality. The use of advanced imaging modalities such as MR imaging, CT, and sonography also is discussed in this article.

Neuroanatomy in elbow arthroscopy.

An injury to the median nerve from within the joint during an arthroscopic synovectomy prompted a study of the relationship of the nerves to the capsule and bones of the elbow. Six pairs of cadaveric elbows frozen in 90 degrees of flexion and one pair frozen in extension were sectioned at 5-mm intervals, and the distances from the major nerves to the bones and capsule were recorded. One elbow joint in each pair was filled with saline solution. Saline solution insufflation increased the nerve-to-bone distance with the elbow in flexion. The results were 12 mm for the median nerve and 6 mm for th

Tennis elbow: a review.

Tennis elbow is a common yet sometimes complex musculoskeletal condition affecting many patients treated by physical therapists. The purpose of this article is to review the anatomy, clinical examination, differential diagnosis, conservative care, and surgical treatment for tennis elbow or lateral epicondylitis. Particular attention is given to determining the precise pathological cause of lateral epicondylitis, with consideration of intrinsic and extrinsic factors associated with this condition. This information should assist health care practitioners who treat patients with this disorder.

Ultrasonography of pulled elbow.

We used ultrasonography in ten children with pulled elbow to compare measurements of the radiocapitellar distance (RCD) on the affected and the unaffected sides. Similar measurements were made in a group of ten age-matched normal children. The mean RCD in pronation of the affected and normal sides in the patients with pulled elbows was 7.2 mm +/- 0.7 and 3.8 mm +/- 0.5, respectively (p < 0.0001). In the normal children the mean RCD in pronation was 4.5 mm +/- 0.5. We conclude that ultrasonography is of value for documenting pulled elbow in children.

Recurrent traumatic elbow dislocation.

Another example of a rare complication of a common elbow luxation is added to the recorded cases, coupled with an updated review of the literature. In a 14-year-old boy, the etiology initially was typical trauma and subsequently resulted in chronic instability. The severity of the ligamentous trauma was not appreciated until the lesion was examined by medial and lateral exposures of the elbow. The Osborne and Cotterill repair procedure was found not feasible. Substitution of dual slings of tendinous fascia lata by implantation successfully restored ginglymus joint stability and corrected recur

Elbow injuries in athletes.

Throwing and racquet sports produce large stresses at the elbow joint consisting of tension on the medial or ulnar side, and compression on the lateral or radial side. Medial tension can lead to tears of the common flexor origin or medial collateral ligament, ulnar nerve subluxation, or pronator teres syndrome. Bony lesions include fragmentation and partial avulsion of the medial humeral epicondyle (little league elbow) or even complete avulsion of the epicondyle. The possible sequelae of excessive lateral compression include osteochondritis dissecans of the capitellum, osteochondral fractures

Acute medial elbow ruptures.

Disruption of the ulnar collateral ligament, flexor muscles, and anterior elbow capsule may result from valgus vector forces and subsequently cause difficulty in throwing, pulling, pushing and catching. Complete medial elbow tears were diagnosed acutely in four elbows by abduction stress tests at 15 degrees of flexion. Three elbows had associated ulnar nerve compression. We repaired torn medial structures by direct suture without ligamentous reconstruction. We also decompressed ulnar nerves and performed one anterior transposition. Full range of motion, strength, and return to previous functio

Pathology of the Elbow in Haemophilia.

In this article, we have reviewed the most important modern conceptual developments for the treatment of the elbow in persons with haemophilia. Despite modern preventive strategies, subclinical bleeding can still occur, underscoring the need for constant vigilance. The elbow is particularly susceptible to synovitis due to its extensive synovial tissue and multiplanar movement, which increase exposure to recurrent bleeding episodes. Iron deposition in the joint initiates a complex inflammatory and apoptotic cascade that ultimately leads to changes in the synovial membrane. Point-of-care ultraso

[Not a Chronic Tennis Elbow].

Not a Chronic Tennis Elbow Abstract. Lateral elbow pain is a highly unspecific finding and may be caused by one of several possible differential diagnoses. Unfortunately, it is often assumed to be epicondylitis, commonly referred to as tennis elbow, although instability is a frequent cause in chronic cases. In the presented case, it is the less frequent nerve compression of the radial nerve which must also be considered, together with plica syndrome, osteochondritis dissecans and arthrosis of the radiocapitellar joint, as a differential diagnosis of lateral elbow pain.

Overuse Injuries of the Elbow.

Repetitive microtrauma in the elbow from chronic overuse occurs in athletes and nonathletes. Although the diagnosis is often made clinically, imaging is helpful to confirm the diagnosis, grade the injury, and guide treatment. MR imaging is particularly helpful in evaluating overuse injuries in the elbow, as tendons, ligaments, and bones/cartilage can be assessed. Tendinopathy can be distinguished from partial- or full-thickness tears, and reactive changes in the bone marrow can be easily identified. This article focuses on the MR imaging appearance of overuse injuries of the elbow involving te

[Tendinopathies of the shoulder and elbow].

BACKGROUND: Tendinopathies of the shoulder and elbow joint are a common problem. According to the current state of knowledge tendinopathies can be separated into acute and chronic tendinitis as well as degenerative tendinosis. ORIGIN: The causes of tendinopathy can be intrinsic, extrinsic or a combination of both. A false straining or overuse with repetitive microtrauma is often the cause. Particularly affected are tendons of the rotator cuff, the long biceps tendons and lower arm extensors. TREATMENT: Priority is given to conservative appproaches for these disease processes. Following appro

Elbow Trauma in the Athlete.

In caring for athletes, the physician must be able to accurately diagnose and appropriately treat all forms of elbow injuries. Traumatic injuries to the elbow are common in the athlete. The late cocking phase of throwing produces tremendous valgus stress on the elbow that can lead to medial epicondyle avulsion fractures in adolescents or rupture of the medial ulnar collateral ligament in skeletally mature overhead throwers, such as baseball pitchers and javelin throwers. Common traumatic elbow injuries suffered by athletes, surgical techniques for operative repair of these injuries, as well as

Osteochondritis dissecans of the elbow.

Osteochondritis dissecans affects the elbow of many young, skeletally immature athletes. The incidence of OCD in the elbow is second to its occurrence in the knee and similar to the incidence in the ankle. Young, athletically active individuals are at increased risk for developing this problem. There is a predilection for those involved in overhead-dominant sports and sports that require the arm to be a weight-bearing limb. The diagnosis is occurring earlier because of an increased awareness of the entity and the increased use of advanced imaging techniques, primarily magnetic resonance imagin

Terrible triad of the elbow.

The terrible triad of the elbow is a difficult injury with historically poor outcomes. Improved experience, techniques, and implants have advanced to the point where restoration of elbow stability can be expected. Careful attention to each destabilizing element of the injury pattern is essential and places high demands on the surgeon's mastery of the anatomic complexity of the elbow. Technically, the surgeon must bring every skill to bear, as soft tissue techniques, fracture repair, and joint arthroplasty are routinely required to adequately treat these complex constellations of injury.

[A woman with a painful elbow].

A 24-year-old woman with Ehlers-Danlos syndrome visited the emergency department because of a post-traumatic painful and swollen left elbow with impaired function. A plain radiograph showed chronic luxation of the radial head with impression in the humeral capitellum. The clinical presentation of chronic luxation of the radial head is easily confused with that of an acute dislocation of the radial head, which may wrongly result in subsequent reduction.
