[Bilateral elbow dislocation related to Essex-Lopresti injury].

Elbow dislocation is second in frequency, after the shoulder, whereas bilateral dislocation is uncommon, even less than dislocations with concurrent associated fractures. One of the least frequent associations is the Essex-Lopresti injury which consists of a fracture of the radial head affecting the distal radioulnar joint with injury to the interosseous membrane. This is a case of bilateral elbow dislocation, one of the elbows associated with the Essex-Lopresti injury. During treatment, the premature closed reduction prevails, previously making sure the elbow is stable, the premise which will

Rehabilitation considerations in the management of terrible triad injury to the elbow.

An elbow dislocation associated with a radial head and coronoid fractures is termed a terrible triad. This injury almost always renders the elbow unstable requiring surgical intervention. The primary goal of surgery is to stabilize the elbow to permit early motion to prevent stiffness. Recent literature has improved our understanding of elbow anatomy and biomechanics as well as the pathoanatomy of this injury. This article reviews key concepts that will allow the surgeon and therapist to apply an systematic rehabilitation approach when managing such injuries.

Terrible triad injury of the elbow: how to improve outcomes?

INTRODUCTION: Dislocation of the elbow joint combined with fractures of the radial head and ulnar coronoid process is referred to as Terrible Triad Injury (TTI). The purpose of this study is to report our experience in the management of this specific injury and to validate the therapeutic choices of our treatment. MATERIAL AND METHODS: Between 1996 and 2006, 23 TTI in 22 patients were treated in our department. Fifteen males and seven females of mean age 46 years were included in this series.Internal fixation of the radial head was performed in 13 cases and arthroplasty in four. Fractures of

Medial complex elbow dislocation: an unusual pattern of injury.

A 47-year-old man sustained a medial complex dislocation of the right elbow. After initial evaluation, closed reduction was performed. On examination under general anesthesia, the elbow was unstable under varus and valgus stress. Computed tomography scan showed a medial epicondyle and a coronoid fracture. Both medial and lateral approaches were used to fix the epicondylar fragment, the coronoid fragment, and the complex damage to the soft tissues. Immobilization in a cast for 1 week followed by early motion in a dynamic orthosis resulted in a good outcome. Follow up at 2 years showed a range o

Anterior interosseus nerve injury following elbow arthroscopy.

Elbow arthroscopy is becoming an increasingly invaluable tool for both evaluation of and treatment of a variety of interarticular disease processes. Case reports have documented radial and posterior interosseus injuries following anterior capsular release. To date, this is the first report of an anterior interosseus nerve injury following arthroscopic surgery of the elbow. Arthroscopic surgery offers unparalleled visualization of the multiple articulations and facets of the elbow. It has been reported as useful for removal of loose bodies, synovectomy, release of arthrofibrosis, and the treatm

Increased Elbow Valgus Torque During Pitching Is Not a Risk Factor for Medial Elbow Injuries in Young Baseball Pitchers: A Prospective Cohort Study.

PURPOSE: To examine the relationship between elbow valgus torque during pitching and the occurrence of medial elbow injuries in young baseball pitchers prospectively. METHODS: Between 2019 and 2023, 361 male pitchers aged 9 to 12 years were enrolled. Pitchers with a history of elbow injury or who had not served as pitchers in baseball games for ≥1 year were excluded. Each participant pitched three fastballs, while an inertial measurement unit recorded the elbow valgus torque. Additionally, participants completed a questionnaire assessing age, height, weight, months of baseball experience, cat…

A comprehensive review of ultrasonographic imaging in athletic elbow injuries: a compartment-based approach.

Elbow injury rates are markedly higher among collegiate athletes than in the general population; however, this elevated incidence is largely … y for real-time dynamic assessment. This review illustrates the ultrasonographic spectrum of common elbow injuries in athletes, including epicondylopathy, ulnar collateral ligament insufficiency, and nerve entrapment syndromes. By employing a systematic, compartment-based approach categorizing findings into anterior, medial, lateral, and posterior zones, radiolo …

Evaluation of functional outcome after nonsurgical management of terrible triad injuries of the elbow.

BACKGROUND: Terrible triad (TT) is one of the severe injuries of the elbow that generally requires surgery. Nonsurgical treatment has recently been applied in selected cases of TT injury. Evaluation of the results of this treatment was the main aim of this study. METHODS: In a prospective cohort study, 10 patients with a mean follow-up of 30.6 months were evaluated. The inclusion criteria included a congruent joint after closed reduction, no indication for surgery of the coronoid and radial head fractures, no block in supination and pronation up to 60°, no intra-articular fragments, and a fre

Terrible triad injuries of the elbow: does the coronoid always need to be fixed?

BACKGROUND: The "terrible triad" of the elbow is a complex injury that can lead to pain, stiffness, and posttraumatic arthritis if not appropriately treated. The primary goal of surgery for these injuries is to restore stability of the joint sufficient to permit early motion. Although most reports recommend repair and/or replacement of all coronoid and radial head fractures when possible, a recent cadaveric study demonstrated that type II coronoid fractures are stable unless the radial head is removed and not replaced. QUESTIONS/PURPOSES: The purposes of this study were to determine the (1) r

Development and validation of the paediatric elbow trauma (PET) rules as a decision rule for radiography in traumatic elbow injuries: a study protocol.

BACKGROUND: Traumatic elbow injuries in children occur frequently and are among the most common traumatic injuries seen in the emergency department (ED) and in general practice. The use of a validated decision rule to enhance selective radiography in paediatric patients with possible elbow fractures may reduce unnecessary exposure to radiation in children. METHOD AND ANALYSIS: This study is designed as a multicentre prospective cohort study. An expert panel of orthopaedic elbow surgeons, paediatric orthopaedic surgeons and trauma surgeons will initially determine clinical parameters that prov…

Do Pitching Restriction Policies Reduce Shoulder and Elbow Injuries in High School Baseball Players?

BACKGROUND: Shoulder and elbow injuries (SEIs) remain prevalent among high school (HS) baseball players. Pitching restriction (PR) policies were mandated by the National Federation of State High School Associations (NFHS) in 2016 as an injury prevention strategy to set workload limits and reduce overuse injuries. The effect of PR policies on HS baseball SEIs remains unknown. HYPOTHESES: (1) SEI rates in HS baseball players would be lower in the 3 seasons after PR policy implementation when compared with the 3 seasons before PR policy implementation and (2) specific components of PR policies w…
