During joint inflammation, various reactive oxygen species (ROS) are present in the surrounding tissue and joint fluid. In the laboratory, hydrogen peroxide (H2O2) is typically used to simulate inflammatory conditions, and media containing proteins and hyaluronic acid (HA) are employed to simulate joint synovial fluid. Electrochemical interactions between H2O2 and HA in the presence of a CoCrMo surface are expected, since HA molecules contain redox-active moieties. During the years 1982-1998, we encountered 7 adolescents and one young woman suffering from KLS. In 4 patients, hypersomnolence was accompanied by hyperphagia and hypersexuality, while in the remaining 4, recurrent hypersomnia was the only symptom. Mean age at onset of hypersomnolent attacks was 15.1+/-3.5 yrs. The mean duration of a hypersomnolent attack was 9.9+/-5.4 days, and the number of attacks per patient was 6.2+/-3.4. Three patients with Creutzfelt-Jakob disease (CJD) showed characteristic ocular manifestations. The head was turned left or right with the eyes deviated downward and skewed. The authors report on clinical and electrophysiological studies of a patient with Type I Oguchi's disease. Numerous small pigmentations of the retinal pigment epithelium (RPE) causing focal disruption of the Oguchi reflex were observed, a phenomenon which has not previously been reported. Consistent with reports of pathologic changes in the RPE, an abnormal electro-oculogram (EOG) was recorded in this patient. On the basis of this information, the pigmentations were interpreted as dysplastic changes.

In a case of bilateral horizontal gaze paralysis, vertical gaze was clinically intact but eye movement recordings demonstrated a transient reduction of vertical saccadic velocities. Horizontal caloric vestibulo-ocular responses were absent. CT scanning and NMR imaging showed a hematoma in the median pontine tegmentum. This case provides additional evidence that vertical and horizontal saccade genesis may be independent of caudal paramedian pontine reticular formation lesions.

An unusual form of retinal degeneration is reported in 15-year-old girl and 11-year-old girl with different pedigrees, which resembles the cases reported by Gouras and associates (1983). We examined monocular spatial vision of strabismic amblyopes by measuring errors of relative directionalization (specifying whether or not two targets are in vertical alignment) and partitioning (equating left- and right-field spaces). A woman, aged 57, reported a considerable loss of vision in her right eye within approx. 60s after lying down. She regained her vision in the upright position within approx. 90s. The Ganzfeld ERG, recorded in the upright position, showed a moderately reduced amplitude, but was entirely absent when recorded in the supine position, closely correlated to the subjective fading of the vision. Although all other clinical tests revealed no further hint to the cause of this remarkable amaurosis fugax, a temporary hypoxy of the retina due to circulatory deficiencies is suspected.

An internuclear ophthalmoplegia was diagnosed and recorded electro-oculographically in a patient with pontomedullary listeriosis. The etiological differential diagnosis in case of internuclear ophthalmoplegia is discussed. It is emphasized that listeria monocytogenes encephalitis should be considered because this pathology may be electively located in the pontomedullary area and because of the encreasing frequency of human listeriosis.
