Development of a SYBR Green I-based real-time PCR for detection of elephant endotheliotropic herpesvirus 1 infection in Asian elephants (Elephas maximus).

Elephant endotheliotropic herpesvirus 1 (EEHV1) can cause fatal hemorrhagic disease in Asian elephants (Elephas maximus). Several studies have described this virus as a major threat to young Asian elephants. A SYBR Green I-based real-time polymerase chain reaction (PCR) was developed to identify EEHV1 on trunk swabs and necropsied tissues. Two of 29 (6.9%) trunk swab samples from healthy Asian elephants were positive for EEHV1. The viruses were analyzed and classified as EEHV1A based on 231 nucleotides of the terminase gene. Necropsied spleen and heart tissue showed the highest level and secon…

Carboxyhemoglobin and depletion of blood oxygen in sleeping elephant seals.

An exceptionally large blood O2 store underlies the remarkable dive performance of elephant seals. However, elevated carboxyhemoglobin (COHb) concentrations in these seals complicate estimations of blood O2 content and O2 depletion rates, and may also affect monitoring of brain oxygenation in seals with new non-invasive near-infrared (NIR) recorders. Using hemoximetry analyses of blood samples during sleep apneas of juvenile northern elephant seals (Mirounga angustirostris), we constructed in vivo Hill plot equations and O2-Hb dissociation curves (ODCs). We found: (a) COHb and methemoglobin (b

MELEGROS: Monolithic Elephant-Inspired Gripper with Optical Sensors.

The elephant trunk exemplifies a natural gripper where structure, actuation, and sensing are seamlessly integrated. Inspired by the distal morphology of the African elephant trunk, we present MELEGROS, a Monolithic ELEphant-inspired GRipper with Optical Sensors, emphasizing sensing as an intrinsic, co-fabricated capability. Unlike multi-material or tendon-based approaches, MELEGROS directly integrates six optical waveguide sensors and five pneumatic chambers into a pneumatically actuated lattice structure (12.5 mm cell size) using a single soft resin and one continuous 3D print. This eliminate

The elephant in the room: addressing systemic inequalities in dental healthcare.

The epidemiology of dental caries has changed over the last decades. Historically, repairing the ravages of dental caries has been the backbone of general dental practice. This opinion article highlights the challenges of moving from a repair service to a preventive service from the viewpoint of: what is the future health market? It also touches on the negative aspects of how organisations manage the challenges of the change process.

Frozen elephant trunk for blunt aortic arch injury.

Total arch replacement with the frozen elephant trunk technique is expanding its indications to treat a wide range of aortic arch and descending aortic conditions. Although blunt thoracic aortic injury commonly occurs at the aortic isthmus or descending thoracic aorta, we encountered a 36-year-old male patient with Grade III blunt aortic injury located in the mid-arch. An endovascular approach was not feasible and thus the patient underwent total arch replacement using the frozen elephant trunk technique.

Interstage mortality in two-stage elephant trunk surgery.

PURPOSE: Diffuse mega-aorta is challenging. Prior studies have raised concerns regarding the safety of the open two-stage elephant trunk (ET) approach for extensive thoracic aortic aneurysm (TAA), specifically in regard to interstage mortality. This study evaluates the safety of the two-stage ET approach for management of extensive TAA. METHODS: Between 2003 and 2018, 152 patients underwent a Stage I ET procedure by a single surgeon (mean age 64.5 ± 14.8). Second stage ET procedure was planned in 60 patients (39.4%) and to-date has been performed in 54 patients (90%). (in the remaining patien

Homograft Treatment for Infected Frozen Elephant Trunk Prosthesis.

A 46-year-old male received total arch replacement with frozen elephant trunk for acute non-A/non-B aortic dissection. Two months later, he underwent emergency reoperation for contained rupture of the left common carotid ostium at its insertion on the aortic arch. Three months after the reoperation, he developed tracheoesophageal fistula and infection of the prosthesis in the region of the aortic arch and the proximal descending aorta. Second reoperation was performed with replacement of the aorta with a composite of three aortic homografts, and the fistula was permanently closed with a direct

Arch replacement with collared elephant trunks: The Siena approach.

OBJECTIVE: To illustrate our experience and results in patients with diffuse aneurysmal disease treated with arch replacement using the Siena collared graft, a device designed in 2002 to improve the elephant trunk technique. Results of the first step surgical implant and the subsequent treatment strategies, with extensive use of endovascular techniques, are reported. METHODS: All aortic arch-replacement procedures using the Siena graft between February 2002 and January 2020 were retrospectively analyzed for early and late clinical outcomes. RESULTS: Of 146 patients (54 women, 36.9%) with a m

The Elephant in the Room: Too Much Medicine in Musculoskeletal Practice.

While advances in assessment and management of musculoskeletal conditions have improved care for many people, there have been other, less beneficial developments in the provision of care for people with musculoskeletal pain conditions, one of which is the worrying tendency to provide too much medicine. Too much medicine occurs when the provision of either investigation or intervention (or both) is unjustifiably excessive. Another concern in musculoskeletal health care is medicalizing normality-when a normal human function or condition is labeled as abnormal. In this Viewpoint, the authors argu

Potentially Pathogenic Leptospira in the Environment of an Elephant Camp in Thailand.

Leptospira is the causative agent of leptospirosis, a globally emerging zoonotic disease. The infection is commonly acquired through contact with the contaminated environment. To extend the knowledge on environmental source of leptospirosis, we investigated the presence of Leptospira in an elephant camp setting where the interaction between humans, animals, and the shared environment occur particularly when engaging in recreational activities. In this study, a total of 24 environmental samples were collected from an elephant camp area in western Thailand. All samples were processed for Leptosp

Aortic reinterventions after the frozen elephant trunk procedure.

OBJECTIVE: The frozen elephant trunk (FET) procedure has emerged as a potential single-step treatment for pathologies of the thoracic aorta, but the procedure's true potential to be a single-step treatment remains unclear. The aim of this study was to evaluate the need and outcomes of aortic reinterventions after previous FET implantation. METHODS: Patient characteristics and follow-up data of 107 patients following the FET procedure were evaluated and compared between patients with and without aortic reinterventions. A competing risk regression model was analyzed to identify independent pred

The elephant in the room: understanding the pathogenesis of Charles Bonnet syndrome.

PURPOSE: Charles Bonnet syndrome (CBS) is a syndrome characterised by complex visual hallucinations in individuals who are cognitively normal, though often elderly and visually impaired. Although first described over 250 years ago, the condition remains poorly understood and difficult to treat. RECENT FINDINGS: Our understanding of CBS pathogenesis has advanced little since it was first described, and much of the recent literature consists of case studies strikingly similar to the first published account of CBS. However, imaging studies have provided some indication as to the cortical areas i

Unilateral phacoemulsification in a captive African elephant (Loxodonta africana).

BACKGROUND: The following case reports describe the clinical presentation, surgical protocol, post-operative care, and long-term follow-up of an African elephant (Loxodonta Africana) presenting with a unilateral cataract. CASE DESCRIPTION: A 42-year-old female African elephant presented for the assessment of ocular discomfort and visual deterioration in the left eye. Pre-surgical treatment included topical anti-inflammatory medication for 20 days prior to surgery. On the day of surgery, following anesthetic induction, a two-handed phacoemulsification technique was performed in the left eye. S
