BACKGROUND: Carotid artery stenting (CAS) is a widely used procedure for patients with high-risk carotid stenosis, particularly in the Chinese population. However, there are no randomized trials examining the comparative efficacy and safety of different embolic protection devices (EPDs) used during CAS. This study aims to evaluate the safety and effectiveness of two newer-generation EPDs, Spider FX™ and Emboshield NAV6™, using a comprehensive approach that includes intraoperative monitoring, postoperative imaging, and clinical outcomes. Given the specific focus on a Chinese population, caution…

Interventional radiologists have the unique ability to apply their imaging knowledge, wide scope of technical skills, and use of innovative technologies to comprehensively address the percutaneous management of the thromboembolic disease processes. This report illustrates successful management of a thrombosed IVC, while protecting against possible pulmonary embolism. Here, we present a 49-year-old female with stage IIIB ovarian cancer who presented with severe bilateral lower extremity edema and anasarca in setting of occlusive thrombus of IVC. The thrombus was the result of compressionfrom a

BACKGROUND: Stroke and other clinically significant embolic complications are well documented in the early period following transcatheter aortic valve replacement (TAVR). The CAPTIS device is an embolic protection system, designed to provide neurovascular and systemic protection by deflecting debris away from the brain's circulation, capturing the debris and thus avoiding systemic embolisation.
AIMS: We aimed to study the safety and feasibility study of the CAPTIS complete cerebral and full-body embolic protection system during TAVR.
METHODS: A first-in-human study investigated the safety, f

BACKGROUND: Embolic stroke is a formidable complication of transcatheter aortic valve implantation (TAVI) and thoracic endovascular aortic repair (TEVAR). Mechanical strategies to reduce the risk of ischemic embolic lesions include embolic protection devices (EPDs) and carbon dioxide flushing (CDF). This study aims to assess the efficacy for EPD and CDF uses in TAVI and TEVAR.
METHODS: A literature review was performed according to the Preferred Reporting Items for Systematic reviews and Meta-Analysis. All searches were performed via PubMed, OvidSP, MEDLINE, Web of Science Core Collection, an…

OBJECTIVE: Carotid artery stenting (CAS) is commonly performed through transfemoral or transcarotid access. Neurologic protection methods utilized during the procedure include distal embolic protection (DEP) and flow reversal devices. Multiple studies demonstrated more favorable outcomes associated with transcarotid artery revascularization with flow reversal (TCAR) in comparison to transfemoral carotid artery stenting with DEP (TFCAS-DEP). However, not many studies compared TCAR with transcarotid artery stenting with DEP (TCAS-DEP) or TFCAS with flow arrest by proximal balloon occlusion (TFCA

OBJECTIVE: The aim of this study was to examine the incidence of ischemia during protected carotid artery stenting (CAS) as well as to compare the protective efficacy of the balloon and filter devices on diffusion-weighted MR imaging (DWI).
MATERIALS AND METHODS: Seventy-one consecutive protected CAS procedures in 70 patients with a severe (> 70%) or symptomatic moderate (> 50%) carotid artery stenosis were examined. A balloon device (PercuSurge GuardWire) and a filter device (FilterWire EX/EZ, Emboshield) was used in 33 cases (CAS-B group) and 38 cases (CAS-F group) to prevent distal emboliz

OBJECTIVES/DESIGN: The aim of the study was to investigate debris captured in filter embolic protection devices (EPDs) during carotid artery stenting (CAS) and its possible correlation with plaque echogenicity and other risk factors.
MATERIALS/METHODS: Between June 2010 and March 2011, 51 consecutive CAS patients (11 females, mean age 71.2 ± 7, 10 symptomatic) who underwent 53 procedures were included in this prospective study. Ultrasonographic Gray-Weale plaque type (I-V, echolucent to echogenic) characterisation was obtained in all cases. The same type of stent and filter EPD was used. Filt…

Stroke remains one of the most feared complications of transcatheter aortic valve replacement (TAVR). We report our initial experience using a dedicated full-body embolic protection device during TAVR in 3 patients with severe aortic stenosis and high-risk embolic features. This included large thrombus on the aortic valve, a left ventricular outflow tract mass, and persistent left atrial thrombus despite anticoagulation. Device deployment and retrieval were successful in all procedures, with no device-related complications or neurologic events at 30-day follow-up. Macroscopic debris was retrie

Embolic protection devices (EPDs) are often used during carotid angioplasty and stenting (CAS) to reduce procedural cerebral emboli. This manuscript seeks to present an overview of evidence on EPDs during CAS. There are three categories of EPDs: distal occlusion (DO-EPD), filter (F-EPD) and proximal occlusion (PO-EPD). DO and F-EPDs have the disadvantage that the device has to be advanced through the stenosis, without protection and that the device may damage the distal internal carotid artery (ICA). F-EPDs have the advantage of maintaining antegrade flow throughout the procedure. PO-EPDs occl…

Endovascular treatment is a standard procedure for subclavian artery stenosis or obstruction. However, great care should be taken to avoid embolic complications to the vertebral artery, and several methods have previously been reported. Hence, as surgical procedures become increasingly complicated, unintended issues may arise during treatment. Here, the authors report a case where the filter-type protection device was caught in the stent because the patient moved during treatment, leading to open surgery to recover the filter-type protection device. A 78-year-old female suffering from a left s

BACKGROUND: Left atrial appendage closure is increasingly used. LAA closure procedures can rarely be complicated by device embolization, typically towards the aorta. In this case report, we describe the successful percutaneous retrieval of an embolized Amulet device from the left atrium in a patient with previous MitraClips.
CASE PRESENTATION: By guidance of intracardiac echocardiography (ICE) and under local anaesthesia, two guiding systems were introduced into the LA; one to stabilize the device against the lateral LA wall and one large system to retrieve the device. However, the LAA closur

Endovascular intervention has become the mainstay of treatment for subclavian artery stenosis in many centers, with high technical success and low complication rates.1,2 However, potential embolization during proximal subclavian artery intervention can lead to catastrophic posterior circulation ischemic complications.3-5 Although considered a rare complication, the presence of a contralateral hypoplastic vertebral artery with persisting anterograde vertebral blood flow on the affected side is likely to increase the risk of embolization.3 The use of embolic protection devices, such as filters a…

OBJECTIVE: Atherosclerotic stenosis or obstruction of the innominate artery is rare. Traditional surgical management is a technically demanding intervention with acceptable, but not negligible, rates of morbidity and mortality. Endovascular approaches to supraaortic lesions have been successful and are now the preferred treatment for stenoses of the brachiocephalic vessels. The use of cerebral protection devices in subclavian and innominate interventions is less established.
CLINICAL PRESENTATION: A 58-year-old woman had Takayasu giant cell arteritis with a history of a left middle cerebral a

Distal protection devices (DPD) have been advocated for carotid artery stenting to reduce the risk of distal embolization. These devices were designed for deployment in the straight cervical segment of the internal carotid artery. We present a case of total cervical carotid occlusion recanalized with the aid of a distal protection device deployed 'distally' in the intracranial internal carotid artery.
