Effective Management of Mosaic Epidermolytic Ichthyosis in a Pediatric Patient With Urea 30% Cream: A Case Report.

Epidermolytic ichthyosis (EI) is a rare autosomal dominant keratinization disorder characterized by blistering at birth followed by progressive hyperkeratosis. Treatment is primarily supportive, involving emollients, keratolytics, and occasionally systemic agents. We report a pediatric case of clinically and histologically confirmed linear EI that showed marked improvement with topical urea 30% cream. Therapy led to substantial reduction in scaling, smoother skin texture, and improved comfort, with no adverse effects. This case highlights the potential role of high-concentration urea as a safe

Chronic Pruritus in an 18-Month-Old Male Infant Due to Anicteric Cholestasis.

An 18-month-old male infant was referred to us with pruritus from 6 months of age. He was being managed as atopic eczema with emollients and antihistamines without any response to treatment. On examination, he was found to have extensive scratch marks, mild hepatomegaly and no icterus. Blood investigations were suggestive of anicteric cholestatic liver disease. Liver biopsy was suggestive of progressive familial intrahepatic cholestasis type 3. He showed symptomatic improvement after starting ursodeoxycholic acid and fat-soluble vitamins and is under follow up.

"Collodion baby": A unique challenge for newborn hearing screening.

We present an infant with collodion membrane who had an obstructed external auditory canal, causing the infant to fail her newborn hearing screen (otoacoustic emissions) bilaterally. An auditory brainstem response (ABR) test was deferred due to the reported increased risk of infections in these babies. Meticulous but gentle debridement of the membranes on the external auditory canal, using a combination of otic drops (ofloxacin), emollients (baby oil/mineral oil), and suctioning, permitted the infant to ultimately pass otoacoustic emissions bilaterally and subsequent serial audiograms.

Sjögren-Larsson syndrome: importance of early diagnosis and aggressive physiotherapy.

Sjögren-Larsson syndrome (SLS) is a congenital ichthyotic disorder with spasticity. We describe a case of a 5-year-old boy with SLS diagnosed clinically based on congenital ichythosis, quadriplegia, and mental retardation. The child responded well to emollients and antihistamines. His quadriplegia was managed by aggressive physiotherapy and mental retardation by stimulation techniques. After a 3-year follow up, significant improvement was seen in his motor and mental disability. This case highlights the importance of clinical diagnosis and early intervention for such a disabling disorder.

Harlequin ichthyosis.

A 2-hour-old newborn boy hospitalized in the neonatal intensive care unit was examined for unusual cutaneous lesions. He had firm plaques covering his body, with fissures especially in flexural areas. Other remarkable findings included edematous hands and feet, ectropion, eclabium, and contractures (Figure). Topical emollients and etretinate were advised, but the newborn died a few hours later. The parents were first-degree relatives. There was no family history of similar lesions. On the basis of clinical features, the diagnosis of harlequin ichthyosis was made.

Acute percutaneous lactic acid poisoning in a child.

Percutaneous poisoning is quite rare, and in particular, topical lactic acid poisoning is even more so. We report a young girl with lamellar ichthyosis and symptoms of poisoning, with clinical signs of irritability, agitation, myoclonia, and difficulty in walking, accompanied by lactic acidosis as a result of the more frequent than recommended application of emollients containing lactic acid. The clinical symptoms resolved upon discontinuation of the topical treatment. Among the possible causes of metabolic acidosis, health care providers should be aware of the possibility of percutaneous pois

Topical tacrolimus in the management of lichen sclerosus.

The effective management of vulval lichen sclerosus (LS) currently depends upon the use of topical steroids and emollients. There are concerns with regard to the long-term toxicity of potent steroids and therefore is a need to consider effective alternatives. Immunomodulatory macrolactams offer an alternative to steroids in the management of some other inflammatory skin disorders and it would seem reasonable therefore to assess their activity in LS. This pilot study of 16 histologically confirmed cases of LS suggests that macrolactams have a positive pharmacological effect.

Moisturizers: what they are and how they work.

Moisturizers are widely used in various dermatologic and cosmetic skin therapies. Different classes of moisturizers are based on their mechanism of action, including occlusives, humectants, emollients and protein rejuvenators. Commercially available moisturizers often utilize components of each of these classes to provide their beneficial effect. Dry skin (xerosis) is the major indication of use. Others include atopic dermatitis, irritant contact dermatitis, ichthyosis, and dermatoheliosis. Although generally efficacious, moisturizers can cause a number of unwanted side-effects, including occl

Keratosis pilaris rubra successfully treated with topical sirolimus: Report of a case and review of the literature.

Keratosis pilaris rubra (KPR) is a subtype of keratosis pilaris (KP) presenting with numerous "grainlike" follicular papules in a background of confluent erythema most often affecting the face and upper extremities with persistence beyond puberty. Treatment has remained challenging with inconsistent benefit from topical therapies such as emollients, keratolytics, corticosteroids, and retinoids, though case reports documenting success with pulsed dye laser therapy have been found. We present a case of KPR in a 15-year-old boy who was successfully treated with topical sirolimus 1% cream.

[Current and upcoming treatments of adult atopic dermatitis].

The treatment of atopic dermatitis in adults is based on the use of topical steroids and emollients. When AD is resistant to a well-conducted topical treatment, phototherapy or systemic treatments can be used: ciclosporin, methotrexate, azathioprine or mycophenolate mofetil. The therapeutic landscape of adult AD is about to change and even be revolutionized by the imminent arrival of new treatments: topical phosphodiesterase 4 inhibitors, topical or systemic JAK inhibitors, anti-IL-4 and/or antiIL-13 biotherapies (dupilumab, tralokinumab, lebrikizumab), anti-IL-31 (nemolizumab), anti-TSLP.

Childhood psoriasis: often favorable outcome.

(1) Plaque psoriasis is the most common form of psoriasis in children. Topical agents should be tried first, especially well-tolerated products such as emollients. Topical corticosteroids are sometimes useful during exacerbations but, given adverse effects, they should only be used for short periods; (2) UVB phototherapy is an option for extensive psoriasis refractory to local treatments, but it carries a long-term risk of skin cancer. Immunosuppressants have not been well assessed in this setting, but methotrexate has been better evaluated than the others.

[Antipruritic therapy in atopic eczema].

In patients with atopic eczema, the quality of life is seriously affected by tormenting pruritus. As the pathogenesis of pruritus is not understood, symptomatic measures are the mainstay of treatment along with avoiding aggravating factors. They include emollients, topical and systemic active ingredients and physical therapy (e. g. phototherapy) adjusted to the acuity the disease. Behavioral therapy offers additional support. New treatment modalities, partly still in experimental stage, such as immunomodulators, immunosuppressive agents and opiate antagonists are reserved for nonresponding pat

Botulinum toxin type A in chronic non-dyshidrotic palmar eczema: A side-by-side comparative study.

New indications are being reported for intradermal botulinum toxin type A (BTX-A) owing to its anti-inflammatory and antipruritic actions. Its successful use for dyshidrotic hand eczema and lichen simplex has been reported in a few cases, while its utility in dry palmar eczema not associated with hyperhidrosis has not yet been investigated. The aim of this study was the assessment of the additive efficacy and tolerability of BTX-A in chronic dry palmar eczema. This prospective non-randomized side-by-side comparative study included 30 cases of chronic bilateral dry palmar eczema with no associa

Patients' understanding of cellulitis and views about how best to prevent recurrent episodes: mixed-methods study in primary and secondary care.

BACKGROUND: Cellulitis is a common painful infection of the skin and underlying tissues that recurs in approximately one-third of cases. The only proven strategy to reduce the risk of recurrence is long-term, low-dose antibiotics. Given current concerns about antibiotic resistance and the pressure to reduce antibiotic prescribing, other prevention strategies are needed. OBJECTIVES: To explore patients' views about cellulitis and different ways of preventing recurrent episodes. METHODS: Adults aged ≥ 18 years with a history of first-episode or recurrent cellulitis were invited through primary

Skin care for preterm and term neonates.

Neonatal skin experiences a progressive adaptation to the extrauterine environment during which special care is needed. The immaturity of the epidermal barrier in the neonatal period may cause dry skin, vulnerability to trauma, rapid onset of microbial colonization and percutaneous drug toxicity. This article reviews the practical implications for hygiene, bathing practices, skin integrity, emollient use, infection control and exposure to percutaneous toxic agents in preterm and term infants.

[Has the pregnancy outcome of women with pregestational diabetes mellitus improved in ten years?].

BACKGROUND: The high prevalence and chronic nature of psoriasis leads to high costs in relation to the treatment and control of the disease. A number of clinical trials have shown that a combination therapy of calcipotriol cream (Daivonex/Dovonex), Leo Pharmaceutical Products) and ultraviolet B phototherapy (UVB) decreases the total number of UVB exposures required compared to UVB treatment alone. From a societal point of view, the addition of calcipotriol to UVB therapy could achieve cost savings due to the fewer UVB treatments needed and the reduced travelling and time off work for patients.

Vancomycin-induced acute generalised exanthematous pustulosis.

Antibiotics have been well documented to result in several dermatological adverse reactions. Many of these adverse reactions are rashes that are difficult to distinguish. We present a case of a female patient in her 30s with acute generalised exanthematous pustulosis following vancomycin treatment for left breast cellulitis. Vancomycin was discontinued, and the patient was provided supportive care that included pain control, topical steroids and topical emollients. The rash improved over the following weeks. It is important to recognise drug reactions and differentiate between distinct dermato

Midface Toddler Excoriation Syndrome (MiTES): A Review.

Midface toddler excoriation syndrome (MiTES) is a rare dermatological condition characterized by chronic, self-inflicted excoriations of the midface, often leading to significant scarring and hyperpigmentation. It typically affects young children, with a male predominance. MiTES is linked to genetic mutations in the PRDM12 gene. Treatment approaches vary, including topical antimicrobials, emollients, steroids, and oral medications aimed at reducing scratching impulses. However, responses are inconsistent, and many patients show only partial or moderate improvement. This review consolidates cur

End-Stage Renal Disease Chronic Itch and Its Management.

End-stage renal disease chronic itch is a frequent symptom that bothers patients with advanced stages of chronic kidney disease. The pathogenesis of the chronic itch symptom is complex and not yet fully understood and includes many metabolic, immunologic, and neurogenic factors. A significant burden of the disease results in decreased quality of life with sleep impairment, depressive symptoms, and increased mortality of affected individuals. No treatment of choice is available; topical therapy (emollients), phototherapy (UV-B), and systemic therapy (antiepileptics, opioid agonists, and antagon
