UNLABELLED: The concept of an 'internal supervisor' has been used in psychotherapy to describe the way in which the supervisory relationship is internalized and utilized by the supervisee. This research explores the possibility, and potential benefit, of training therapists to develop a 'compassionate internal supervisor'. A training programme was developed for trainee cognitive-behavioural therapists using adapted versions of compassion-focused therapy interventions. The training focused on guided imagery exercises and reflective practices undertaken for a 4-week period. Seven trainee cogniti

OBJECTIVE: Suicidal ideation is a central clinical risk in psychotherapy. Patients' engagement with therapy between sessions, captured by intersession processes (ISP), may be particularly informative for understanding risk and resilience. This study examined associations between ISP and suicidal ideation, distinguishing between-person vulnerabilities from within-person crisis dynamics.
METHOD: We analyzed routine outpatient data (N = 73, 2,586 total sessions), including an eight-item short version of the Intersession Experiences Questionnaire (IEQ-S) and the PHQ-9 suicidal ideation item. Mult

The study investigates the psychological effects of alexithymia and maladaptive coping strategies on individuals exposed indirectly to trauma through media coverage. Focusing the October 7th, 2023, terrorist attacks in Israel, we explore how alexithymia characterized by difficulty in processing emotions, exacerbates trauma-related symptoms. The research examines 213 participants using the Toronto Alexithymia Scale (TAS-20) as well as trauma, somatization and coping questionnaires. Results indicate that individuals with alexithymia experience more severe and prolonged trauma symptoms, including

Borderline personality disorder (BPD) is a complex personality disorder characterised by immense emotional dysregulation, impulsivity, aggression and substantial interpersonal difficulties. This review begins with examining DSM-5-TR diagnostic clusters for BPD, highlighting the importance of accurate classification. It provides an in-depth analysis of BPD, starting with its epidemiology, diagnostic subtypes, core symptoms, and the challenges these symptoms pose for patients and their support networks. The review explores common co-occurring conditions, such as mood disorders, anxiety disorders

Although stress is an inevitable part of everyday life, its chronicity, severity, and perceived burden can result in enduring distress, which may manifest as heightened emotionality, contributing to a number of self-regulatory failures. Specifically, distress disorders are characterized, in part, by heightened sensitivity to underlying motivational systems related to threat/safety, reward/loss, or both. Further, individuals suffering from these conditions typically engage in perseverate negative thinking (e.g., worry, rumination, self-criticism) in an effort to manage motivationally relevant d

The aims of this study were to examine whether therapists' emotional reactions to their patients mediate the effect of personality disorders and interpersonal problem behaviours on the outcome of treatment, focusing on an Axis I disorder; and whether therapists' reactions mediate the effect of personality disorders on the course of interpersonal problems. Therapists completed a checklist of emotional reactions to individual patients after the end of residential cognitive or guided mastery therapy for 46 inpatients with panic disorder with agoraphobia. The severity of DSM-III-R personality diso

BACKGROUND: Abnormal emotional processing serves as a common deficit across psychiatric disorders and is a primary target of pharmacotherapy. However, the neuroimaging findings underlying emotion processing recovery (EPR) are heterogeneous, hampering the development of new therapies.
METHODS: Here, we reviewed neuroimaging studies of pharmacotherapy in psychiatric disorders, focusing on brain activation associated with pharmacotherapy-induced EPR. Using a novel therapeutic network mapping approach with a functional connectome dataset (N = 652), we tested whether the identified brain activatio

Trauma-Focused Cognitive-Behavioral Therapy (TF-CBT) is the gold standard treatment for pediatric PTSD. Nonetheless, clinical outcomes in TF-CBT are highly variable, indicating a need to identify reliable predictors that allow forecasting treatment response. Here, we test the hypothesis that functional neuroimaging correlates of emotion processing predict PTSD symptom reduction during Trauma-Focused Cognitive-Behavioral Therapy (TF-CBT) among adolescent girls with PTSD. Thirty-four adolescent girls with PTSD related to physical or sexual assault were enrolled in TF-CBT, delivered in an approxi

BACKGROUND: The impairing neurodevelopmental course of bipolar disorder (BD) suggests the importance of early intervention for youth in the beginning phases of the illness.
OBJECTIVE: We report the results of a 3-site randomized trial of family-focused therapy for youth at high-risk (FFT-HR) for BD, and explore psychosocial and neuroimaging variables as mediators of treatment effects.
METHODS: High-risk youth (<18 years) with major depressive disorder or other specified BD, active mood symptoms, and a family history of BD were randomly assigned to 4 months of FFT-HR (psychoeducation, communi

BACKGROUND: Attention deficit hyperactivity disorder (ADHD) is frequently associated with emotional dysregulation (ED). ED is characterized by excessive and inappropriate emotional reactions compared to social norms, uncontrolled and rapid shifts in emotion, and attention focused on emotional stimuli. Few studies have evaluated non-pharmacological interventions to improve ED in children with ADHD. The current randomized controlled trial assesses the efficacy of a cognitive behavioral therapy (CBT) intervention compared with a theater-based intervention (TBI) in children with ADHD and ED.

As one of the areas of greatest concern for people with serious mental illness (SMI) are unmet social needs, psychosocial interventions have been developed to address them. The current study utilized a randomized controlled trial to examine the effectiveness of social cognition and interaction training (SCIT) versus a therapeutic alliance focused theraphy (TAFT) versus a treatment-as-usual (TAU) control group on social functioning and quality of life as primary outcomes and social cognition variables as secondary outcomes. Sixty-three persons between the ages of 24 and 69 years with SMI (41 me

Dropout rates in trauma-focused treatments for adult posttraumatic stress disorder (PTSD) are high. Most research has focused on demographic and pretreatment predictors of dropout, but findings have been inconsistent. We examined predictors of dropout in cognitive processing therapy (CPT) by coding the content of trauma narratives written in early sessions of CPT. Data are from a randomized controlled noninferiority trial of CPT and written exposure therapy (WET) in which CPT showed significantly higher dropout rates than WET (39.7% CPT vs. 6.4% WET). Participants were 51 adults with a primary

OBJECTIVE: In this narrative review, we summarize relevant literature pertaining to psychosocial risk factors for headache and migraine progression, current behavioral and psychological treatments, and consider promising treatments.
BACKGROUND: Headache and migraine are common and associated with significant burden and disability. Current treatments targeting psychosocial risk factors show modest outcomes and do not directly address the impact of early life adversity, including the development of maladaptive emotional processing. An intervention that could address these factors and include co

BACKGROUND AND OBJECTIVE: War-related mass trauma and large-scale terrorist attacks disproportionately affect clinically vulnerable adolescents, exacerbating self-harm and suicidality while burdening under-resourced public mental health systems. This study evaluated the feasibility and preliminary efficacy of a parent-only Dialectical Behavior Therapy (DBT) skills group within routine care. We hypothesized that the parent-only DBT skills group would be associated with improved parental emotion regulation, decrease helplessness and family conflict, and indirectly reduce adolescent risk behavior

BACKGROUND: Posttraumatic stress disorder (PTSD) affects up to 25% of trauma-exposed adolescents; yet, access to evidence-based treatment remains limited in rural regions. Trauma-focused cognitive behavioral therapy (TF-CBT) is the first-line intervention, but structural barriers such as long travel distances and therapist shortages hinder implementation. Digital and blended formats may improve accessibility, but evidence for adolescents with PTSD is limited. Shame and self-criticism are common following interpersonal trauma and can reduce engagement; compassion-focused strategies target these

Clinicians' own internal resources for understanding relationships--that is, their attachment organizations--have been found to influence the process and outcome of treatment. The current study addressed whether the attachment organizations of novice couple and family therapists were associated with couples' experiences of their therapists, therapeutic alliance, session impact, and emotionally focused couple therapy (EFT) fidelity (i.e., especially as related to targeting and working with attachment needs and overt and underlying emotions). Novice couple and family therapists delivered EFT, an

OBJECTIVE: Randomised Control Trials play an important role in psychotherapy research and have a defining impact on mental health policy. However, relatively little research has been published investigating the experiences of therapists participating in such trials. The present paper reports on a study investigating the experiences of therapists trained to deliver both protocols within an RCT.
METHOD: Semi-structured interviews were conducted with 9 therapists who participated in an RCT comparing Emotion-Focused Therapy (EFT) and Cognitive Behaviour Therapy (CBT) as treatments for Generalized…
