[Juvenile myoclonic epilepsy].

INTRODUCTION: In this paper we review the history of juvenile myoclonic epilepsy (JME) since the description by Herpin in 1852 and 1857 of the seizures of some epileptic patients which he called impulsions . Credit is due to Janz and Christian who in 1957 gave a detailed description of the condition which they called impulsive Petit mal and we know today as JME. Delgado-Escueta and his group in Los Angeles discovered the genetic basis of JME in some patients. DEVELOPMENT AND CONCLUSIONS: The homogeneity of the clinical and neurophysiological characteristics of series of patients with JME repo…

[Juvenile myoclonic epilepsy].

CONCLUSION: We conclude that despite inevitable variability the clinical picture of JME is characteristic. It is easy to diagnose JME if one thinks of it while the history should be thoroughly analyzed. An EEG recording during sleep confirms the diagnosis. An early diagnosis of JME permits adequate prognosis of the subsequent course of epilepsy, and adequate therapy brings remission in most of the patients. If treatment starts following the large number of severe GTC seizures, the response to therapy is incomplete. The persistency of the illness throughout the life, the need for continuous med…

Myoclonic-astatic epilepsy.

Myoclonic-astatic epilepsy (MAE) belongs to the group of epilepsies with primarily generalized seizures as absence epilepsies, and juvenile myoclonic epilepsy, as well as infantile and juvenile idiopathic epilepsy with generalized tonic-clonic seizures. Like these types of epilepsy, MAE is polygenically determined with little non-genetic variability. The disease is characterized by the following criteria: genetic predisposition (high incidence of seizures and/or genetic EEG patterns in relatives); mostly normal development and no neurological deficits before onset; primarily generalized myoclo…

Increased Expression of MST1 in Patients With Epilepsy and in a Rat Model of Epilepsy.

Mammalian sterile20-like kinase 1 (MST1), a serine/threonine kinase frequently expressed, has emerged as pivotal modulator of multiple physiological and pathological conditions such as cellular growth, programmed cell death, oxidative stress, neurodegeneration, inflammation, and synaptic plasticity in the central nervous system. Various neurological diseases are associated with the activation of MST1. Epilepsy is a severe neurological disorder characterized by abrupt abnormal electrical activity in the brain and recurring spontaneous seizures. The most common pathological discoveries in patien…

A Rare Form of Reflex Epilepsy: Eating Epilepsy - A Case Report.

INTRODUCTION: Reflex epilepsies are rare syndromes where seizures are triggered by particular stimuli or activities that may be motor, sensory, or cognitive in nature. Eating-induced seizures are an uncommon presentation of reflex epilepsy. Many etiologies, including genetics, ethnicity, and specific foods and eating behaviors, are linked to eating epilepsy. CASE PRESENTATION: In this case report, a 55-year-old man who was admitted to the neurology clinic with epileptic seizures that occurred many times after spicy meals is presented. His neurological examination and metabolic values were nor…

[Immunity and epilepsy. Autoimmune encephalitis and epilepsy linked to autoimmune disorders].

A relationship between epilepsy an autoimmune disorder has been demonstrated by multiple studies. This association is stronger, when looking at specific disorders, such as systemic lupus erythematosus and diabetes mellitus type 1. Seizures could also represent a manifestation of immunological disorders with antibodies direct towards neuronal targets. This article focus on the diseases with more convincing evidence about this association or the disorders most frequently observed in pediatrics.

Diagnosis and prognosis of serum Fut8 for epilepsy and refractory epilepsy in children.

With adequate serum concentration of antiepileptic drugs, the epilepsy symptoms in many patients still cannot be controlled well. The alteration of glycosyltransferase has obvious influence on the pathogenesis of epilepsy. In this study, we focus on the diagnostic and prognostic value of fucosyltransferase 8 (Fut8) on epilepsy and refractory epilepsy. Serum samples of 199 patients with epilepsy, 59 patients with refractory epilepsy and 22 healthy controls who were diagnosed in Shenzhen Children's hospital from August 2018 to August 2019 were collected. The level of lectins was further analyzed…

Obesity and metabolic syndrome in patients with epilepsy, their relation with epilepsy control.

Obesity and metabolic syndrome (MetS) are commonly observed in patients with epilepsy (PWE). Obesity and MetS are not only affecting the physical fitness and quality of life of these patients, rather antiepileptic drugs (AEDs) compliance and seizure control have also been affected. The objective of this review is to search the published literature regarding the prevalence of obesity and MetS in PWE and their relation to the response to AEDs. A comprehensive search using PubMed, Cochrane Databases, and Google Scholar was performed. A supplementary citation search was also conducted by analyzing…

Family Burden and Epilepsy Surgery in Children with Drug-Resistant Epilepsy.

INTRODUCTION: Family burden (FB) in pediatric patients with drug-resistant epilepsy (DRE) is significantly higher than that in children with non-DRE. Epilepsy surgery is an established approach to treat DRE, and this study examines the impact of pediatric epilepsy surgery on FB. METHODS: We retrospectively analyzed data of families and pediatric patients with focal structural DRE treated with epilepsy surgery at our epilepsy center from April 2018 to November 2021. We examined the relationship between cognitive, behavioral, and epilepsy-specific data and the FB measured with the German versio…

Epilepsy surgery in patient with monogenic epilepsy related to SCN8A mutation.

Epilepsy surgery is superior to prolonged medical therapy in patients with drug-resistant focal epilepsy, but reports on epilepsy surgery outcomes for patients with a genetic etiology are limited, especially in adults. This is the first documented report of a stereoelectroencephalography (SEEG) evaluation and resective surgery outcome in an adult patient with epilepsy related to SCN8A mutation. We describe a patient with epilepsy related to SCN8A mutation which was reported as a variant of uncertain significance at time of his pre-surgical evaluation and reclassified as likely pathogenic about…

Sudden unexpected death in epilepsy: Incidence at a Spanish epilepsy unit.

INTRODUCTION: Sudden unexpected death in epilepsy (SUDEP) is the leading cause of death in patients with epilepsy. Most studies concerning this issue have been conducted in central and northern European countries and the United States. We conducted an epidemiologic study of SUDEP at our hospital's epilepsy unit. METHODS: This retrospective cohort study included all epileptic patients aged ≥14 years, regardless of epilepsy severity, who were treated at the outpatient epilepsy unit of our hospital between 2000 and 2013. The study included 2,309 patients. Deceased patients were identified using …

Epilepsies among twins born in families of Indian probands with epilepsy.

Twins have a high frequency of adverse perinatal events and have been reported to have an increased risk of seizures. Contrary to popular belief, a recent study reported that twins do not have an increased risk of seizures. We studied the relationship of twinning and epilepsy in India. The frequency of twinning in families of probands with epilepsy was compared with the frequency of twinning among hospital births in the same region of India. We also compared consecutively ascertained probands with epilepsy for their twin status and the occurrence of epilepsies in their twin and non-twin relati…

Epilepsy surgery in pediatric patients with complex COL4A1/2-associated epilepsy.

OBJECTIVE: Pathogenic variants in COL4A1/2 compromise vascular basement membrane integrity, causing intrauterine hemorrhage and infarction leading to extensive structural brain lesions. Vascular disruptions during critical windows of neuronal migration can result in malformations of cortical development (MCD), which may drive epileptogenicity. While surgery is established for drug-resistant focal epilepsy, COL4A1/2 patients with widespread or bilateral MRI abnormalities are frequently deemed ineligible. In selected patients, this necessitates a refined surgical rationale, shifting from strictl…

Epilepsy related mortality: more than sudden unexpected death in epilepsy (SUDEP).

BACKGROUND: The incidence of sudden unexpected death in epilepsy (SUDEP) in Ireland has previously been studied. The aim of this work is to examine how frequently epilepsy related deaths, other than SUDEP, occur in the Irish population. METHODS: All deaths referred to the coroner in 2019 were examined. Those with a history of possible epilepsy were noted and subjected to a more detailed assessment. Cases of SUDEP were excluded and other epilepsy related deaths were divided into those directly related to epilepsy and those indirectly related to epilepsy. RESULTS: We identified 54 non SUDEP ep…
