Atypical longitudinal self-inflicted incised wounds as a cause of death in an ecstasy user.

A 32-year-old chronic drug abuser was found dead at the entrance to his home, covered in blood. Pools of blood, bloody footprints and bloodstains were found across the apartment. A double-edged razor was recovered from the scene. Autopsy revealed multiple incised wounds: two on the palmar aspect of both forearms (37 cm and 33 cm long, oriented longitudinally). The cuts extended into the subcutaneous adipose tissue, muscles and blood vessels. There was an additional 30 cm cut extending across the middle of the torso anteriorly and two more cuts on the anteromedial aspect of the lower legs, set

Penile carcinoma - a rare cause of sudden death.

Penile carcinoma is an uncommon but potentially mutilating disease with a heterogeneous etiology. We present a case in which a man died due to severe blood loss from an amputated penis. It was initially regarded as a violent death with an external cause. Autopsy found irregular defects with smooth edges in the external genital area and on both sides of the groin, with yellowish-gray callus-like structures and dried blood at the base of where the penis would have been located. Histological examination revealed the presence of squamous cell carcinoma with signs of keratinization. Subsequent inve

Sudden death by rupture of a varicose vein: Case report and review of literature.

Venous disease, including varicose veins and chronic venous insufficiency, is one of the most common chronic medical conditions. Haemorrhage from rupture of varicose veins in the legs is rare and can lead to sudden death. Nevertheless, this condition is not included among the causes of sudden death. In this case, an 88-year-old man was found dead in a large pool of blood at home. Initially, investigators mistakenly assumed that it was a case of homicide. Bloodstain pattern analysis was performed. An external examination of the victim showed an ulcer on the left foot and evident varicose veins

[Bumping into a knife--exculpatory statement or (tragic) accident?].

Assessing stab wounds is a common task in forensic medicine. Quite often an accused claims that the victim bumped into the knife. Taking into account all the facts ascertained in the investigations conducted, such a statement can be rapidly disproved in most cases. The present case of a 14-year-old boy fatally wounded by his mother with a kitchen knife examines the aspect of walking into a knife. It is an example that this statement cannot always be regarded as implausible and that every single case has to be reconstructed in detail on the basis of all the investigation results.

Unattended fatal haemorrhage due to spontaneous peripheral varicose vein rupture--two case reports.

Venous haemorrhage is a rare complication of varicose veins of the legs. Varicosis is found in 15-50% of the population. Haemorrhage from ruptured varicose veins of the legs can occur spontaneously or after a minor trauma. More frequent complications of varicosis include peripheral oedema of the ankles, skin pigmentation and skin ulcers. In case of venous haemorrhage simple possibilities of first aid can be applied, therefore fatal haemorrhage is a rarity. Two cases of fatal varicose vein rupture are presented. A 58-year-old man and a 76-year-old woman, both living alone, were found dead in th

A Comparison of the Effects of Intraosseous and Intravenous 5% Albumin on Infusion Time and Hemodynamic Measures in a Swine Model of Hemorrhagic Shock.

UNLABELLED: Introduction Obtaining intravenous (IV) access in patients in hemorrhagic shock is often difficult and prolonged. Failed IV attempts delay life-saving treatment. Intraosseous (IO) access may often be obtained faster than IV access. Albumin (5%) is an option for prehospital volume expansion because of the absence of interference with coagulation and platelet function. Hypothesis/Problem There are limited data comparing the performance of IO and IV administered 5% albumin. The aims of this study were to compare the effects of tibial IO (TIO) and IV administration of 500 mL of 5% albu

[Cooperation between a surgeon and a gastroenterologist in the management of vascular complications of the liver cirrhosis].

AIM: The aim of this work was to confirm indications for surgical procedures in cases of relapsing bleeding from oesophageal varices as a consequence of the liver cirrhosis. METHODOLOGY: Based on the retrospective study of the endoscopical examinations results and the patient records, the criteria of the patient selection for the surgical procedure, the type of the surgical procedure, the postoperative complications and the long-term assessment, were revised. RESULTS: During the six-year period, 7625 gastrofibroscopies were conducted. In this group, the oesophageal varices were diagnosed in

Arteriotracheal fistula in patients treated for lymphoma.

Exsanguinating hemorrhage is an unusual cause of death in patients who are being treated for neoplasms. We report two cases of adolescents with mediastinal non-Hodgkin's lymphoma who each developed a fistula between the right brachiocephalic trunk and the trachea following mediastinal irradiation and systemic chemotherapy. During the hospital course, the patients had each suffered a respiratory arrest and undergone difficult endotracheal intubation. The tracheal ulcerations at the fistula sites were unsuspected prior to autopsy.

Case report: Zone 3 gunshot wound to the left neck treated with combined Foley catheter balloon tamponade and interventional radiology stent placement.

In the trauma and military setting, catheter balloon tamponade has been successfully used as an effective tool for the management of exsanguination secondary to penetrating injuries. This maneuver may improve outcomes in patients with uncontrolled bleeding, particularly in penetrating neck injuries (PNI), thorax, axillae, and groin. We describe a case of a stab wound to the left neck that led to surgical left neck exploration utilizing Foley catheter balloon tamponade (FCBT) for initial control of massive hemorrhage and a subsequent interventional radiology (IR) endovascular stent placement fo

Extraabdominal Rupture of an Inflammatory Abdominal Aortic Aneurysm in a Patient With Aortodecubital Fistula: A Case Report.

Aortodecubital fistula is a pathologic communication between aorta and a decubitus ulcer. It is very rarely encountered vascular condition in abdominal aortic aneurysms (AAA), with difficult diagnostics and high mortality. Patients often present with systemic and local infection and are at risk for hemorrhage. We present a paraplegic patient with fistulous communication between an inflamed abdominal aortic aneurysm and a sacral decubitus ulcer, leading to intermittent bleeding episodes and finally to exsanguination. While extremely rare, this case emphasizes the need for early, accurate diagno

[Epistaxis--a stepwise plan for diagnosis and therapy].

Although nosebleed is usually an uncomplicated condition, in extreme cases it can certainly cause complications and even lead to exsanguination. In principle, local causes of bleeding, for example due to changes within the nose itself, need to be differentiated from symptomatic causes (e.g. infectious diseases, hypertension, coagulation disorders, malignant systemic diseases). The present article deals not so much with the details of nosebleed, but rather with the suitable therapeutic measures required. The author has developed a stepped approach which is intended to provide orientational aid

Transabdominal chorionic villus sampling in the second trimester of pregnancy: feto-maternal transfusions in relation to pregnancy outcome.

Volumes of feto-maternal transfusions (FMTs) in transabdominal chorionic villus sampling (TACVS) in the second trimester of pregnancy were calculated from the difference between maternal alpha-fetoprotein (AFP) concentrations before and 1 h after TACVS. In 50 pregnancies, there existed no correlation between FMT volume and the amount of villi collected or the number of TACVS attempts. The expected risk of fetal exsanguination due to very voluminous FMT could not be substantiated. In one case, immunization could have been the cause of hydrops fetalis, although only a volume of 0.15 ml could be

Aortoesophageal fistula. Review of clinical, radiographic, and endoscopic features.

Primary aortoesophageal fistula is a rare cause of severe and often fatal gastrointestinal bleeding. The classic diagnostic triad consists of midthoracic pain and sentinel hemorrhage, followed by fatal exsanguination. A prompt, definitive diagnosis at the time of the initial bleeding episode is essential for timely, life-saving surgery. Any combination of low-grade, intermittent hematemesis, with midthoracic chest pain, dysphagia, or a mediastinal mass, should alert the physician to this diagnosis, and an aggressive diagnostic and therapeutic approach. In this article, we report a case of aort

Phenylephrine ameliorates cerebral cytotoxic edema and reduces cerebral infarction volume in a rat model of complete unilateral carotid artery occlusion with severe hypotension.

BACKGROUND: Phenylephrine is a selective alpha(1) adrenergic receptor agonist that increases arterial blood pressure by peripheral vasoconstriction. However, whether phenylephrine improves the outcome of cerebral ischemia in patients with internal carotid artery disease during hemorrhagic shock is unclear. METHODS: (Experiment 1) Twenty-one adult male Sprague-Dawley rats were anesthetized with isoflurane and their lungs mechanically ventilated. After the right common carotid artery was ligated, arterial blood was withdrawn until mean arterial blood pressure (MAP) reached 30 mm Hg to induce ce

Causes and associations with mortality in patients with pelvic ring injuries with haemorrhagic shock.

BACKGROUND: High energy pelvic ring injuries are associated with significant morbidity and mortality and can be accompanied by haemorrhagic shock following associated vascular injury. This study evaluated the causes and predictors of mortality in haemodynamically unstable pelvic fractures. METHODS: This retrospective observational study at a Major Trauma Centre reviewed 938 consecutive adult patients (≥ 18yrs) with pelvic ring injuries between December 2014 and November 2018. Patients with features of haemorrhagic shock were included, defined as: arrival Systolic BP < 90 mmHg, Base Deficit ≥

The first resection of an aortic aneurysm.

In 1951 the report from Paris that an abdominal aortic aneurysm had been successfully resected greatly influenced surgeons throughout the world who, until then, had regarded such an operation as being outside the bounds of surgery. Indeed, as a young surgeon I could hardly believe such a procedure would be possible, having seen an unsuccessful attempt at producing thrombosis of the aneurysm by introducing coils of wire into the sac, (Colt's operation) and, in other cases, merely standing by helplessly as the patient exsanguinated from rupture of the aneurysm.

Endovascular treatment of aortobronchial fistula secondary to coronary artery bypass graft (CABG).

Aortobronchial fistula (ABF) is a rare and late complication of cardiac surgery. If untreated, mortality rate is approximately 100% secondary to exsanguinations haemoptysis. Early diagnosis and treatment are essential for successful management. Open surgical repair is associated with high morbidity and mortality rate, ranging from 25% to 41%. Endovascular treatments of ABF is a less invasive treatment modality and have become an important alternative to open surgical intervention in aortic pathologies. We present a case of ABF that successfully is managed by endovascular approach.

Multifactorial mortality in bongos and other wild ungulates in the north of the Congo Republic.

Wildlife mortality involving bongos, Tragelaphus eurycerus, and other ungulates was investigated in the north of the Congo Republic in 1997. Four bongos, one forest buffalo, Syncerus caffer nanus, and one domestic sheep were examined and sampled. Although an outbreak of rinderpest had been suspected, it was found that the animals, which had been weakened by an Elaeophora sagitta infection and possibly also by adverse climatic conditions, had been exsanguinated and driven to exhaustion by an unusual plague of Stomoxys omega.

Benckiser's hemorrhage: about an uncommon case report.

BACKGROUND: Benckiser's hemorrhage is a serious obstetric emergency caused by rupture of one or more umbilical vessels of velamentous insertion, putting the fetus in distress and leading to rapid fetal death through exsanguination in utero. It is an uncommon condition associated with a neonatal mortality rate of 75-100%. This hemorrhage of fetal origin happens most often when the membranes rupture, whether spontaneously or artificially. This is why prenatal diagnosis via ultrasound can only be beneficial and make it possible to schedule a prophylactic caesarean section before the onset of feta

Hepatic trauma in Auckland.

A retrospective review of 102 cases of hepatic trauma in the Auckland area between 1979 and 1985 is presented. Particular attention has been focused on those cases where there was massive bleeding, and the prognostic factors that govern outcome have been determined. Mechanism and multiplicity of injury, and the presence of severe hypotension (systolic blood pressure less than .80 mmHg) either at presentation or following induction of anaesthesia were the four most important determinants of prognosis. Blunt trauma caused 82% of these cases, with the mortality in this group being 27%, compared w
