Factor VII Activating Protease (FSAP) is a plasma protease affecting both coagulation and fibrinolysis. Although a role in hemostasis is still unclear, the identification of additional physiologic substrates will help to elucidate its role in this context. FSAP has been reported to cleave fibrinogen, but the functional consequences of this are not known. We have therefore undertaken this study to determine the implications of this cleavage for fibrin-clot formation and its lysis. Treatment of human fibrinogen with FSAP released an N-terminal peptide from the Bβ chain (Bβ1-53) and subsequently …

OBJECTIVE: To provide mutation analysis and prenatal diagnosis for a family affected with congenital factor VII(FVII) deficiency.
METHODS: DNA was extracted from peripheral blood samples from the proband and his parents. All exons and flanking sequence of the FVII gene were amplified with PCR and subjected to direct sequencing. Prenatal diagnosis was performed by amniocentesis.
RESULTS: A homozygous mutation (NM_000131.3) c.572-1G>A was identified in the proband. Both parents of the fetus were carriers of the mutation.
CONCLUSION: A method for molecular diagnosis of congenital factor VII de…

Inherited factor VII (FVII) deficiency is a rare autosomal recessive hemorrhagic disorder. The major clinical symptoms include: bleeding from the oral cavity, epistaxis, menorrhagia, spontaneous hemarthros, bleeding to the gastrointestinal tract and central nervous system, and perioperative bleeding. The aim of this study was to present our experience in preventing bleeding and hemorrhagic disorders in surgical patients with inherited FVII deficiency by using recombinant activated FVIIa (rFVIIa), and with prothrombin complex concentrates (PCCs). In 2002-2011, 17 patients with inherited FVII de…

BACKGROUND: Factor VII-activating protease (FSAP) is a recently discovered plasma protease with a role in the regulation of hemostasis and vascular remodeling processes. Higher levels and activity of FSAP have been reported in patients with deep vein thrombosis, but there are no data on plasma FSAP in ischemic stroke (IS).
OBJECTIVE: To investigate whether FSAP antigen levels and activity are associated with IS and/or etiologic subtypes of IS.
PATIENTS AND METHODS: To assess the potential association between FSAP and IS, plasma FSAP antigen levels and activity were measured in 600 consecutiv…

INTRODUCTION: Cell death is a central event in the pathogenesis of sepsis and is reflected by circulating nucleosomes. Circulating nucleosomes were suggested to play an important role in inflammation and were demonstrated to correlate with severity and outcome in sepsis patients. We recently showed that plasma can release nucleosomes from late apoptotic cells. Factor VII-activating protease (FSAP) was identified to be the plasma serine protease responsible for nucleosome release. The aim of this study was to investigate FSAP activation in patients suffering from various inflammatory diseases o…

BACKGROUND: Empirical off-label use of recombinant activated factor VII (rFVIIa) has been reported to be effective in some cases of severe postpartum haemorrhage (PPH). Successful management of these patients has lead to more wide-spread use of rFVIIa in less severe cases without any evidence for the advantages of its administration.
METHODS: Until November 2006, we had administered rFVIIa to 38 parturients. Based on our initial experience with the first 12 patients, we prepared guidelines for the use of rFVIIa. During the existence of these guidelines, we made a retrospective comparison of t…

BACKGROUND: Recombinant activated factor VII (rfVIIa) is widely used as a prohemostatic agent for patients with congenital and acquired haemophilia. In the last few years it has been increasingly used 'off label' in medical and surgical patients to minimise or control severe or refractory bleeding, in particular when conventional therapy failed or was judged to be insufficient.
METHODS: A computer-assisted search of the MEDLINE, EMBASE, and CENTRAL (Cochrane Central Register of Controlled Trials) electronic databases up to July 2006 to identify published randomised controlled trials (RCTs) th…

Blood coagulation factor VII (fVII) is physiologically synthesized in the liver and released into the blood. Binding of fVII to tissue factor (TF) at sites of vascular injury triggers coagulation and hemostasis. TF/fVIIa complex formation on the surface of cancer cells plays important roles in cancer biology. Although fVII is synthesized by hepatocellular carcinoma, it remained unclear how TF/fVIIa complex formation and promigratory signaling can occur for most other cancers in extravascular locations. Here, we show by reverse transcription-PCR analysis that nonhepatic cancer cell lines consti…

Recombinant activated factor VII (NovoSeven), a novel hemostatic agent originally developed for the treatment of bleeding episodes in hemophilia A or B patients with inhibitors, has been recently employed with benefit for the management of hemorrhages in other nonhemophilic congenital and acquired hemostatic abnormalities. This review focuses on the use of this drug in acquired and congenital von Willebrand disease. The analysis of the literature data shows that recombinant activated factor VII is an effective agent for the treatment of refractory bleeding in von Willebrand disease patients an…

There is increasing interest in continuous infusion of recombinant activated factor VII (rFVIIa) as a convenient and safe alternative to intermittent bolus therapy. In the Australian patients reported in this paper, cost savings of up to 25% in the first 12 h of treatment with continuous infusion of rFVIIa have been achieved safely, suggesting that substantial overall savings are possible. However, in the Thai patient reported, a dose reduction of 35% in the first 12 h was associated with poor haemostatic control, suggesting that a dose reduction of >25% may be inadvisable. The indications for…

A simplified technique using DEAE-cellulose chromatography for the preparation of factor VII deficient substrate was developed in order to reduce the high cost of individual factor VII assay in the routine coagulation laboratory. The substrate prepared from cryo-removed human and bovine plasma had a high correlation (r = 0.9929) with two of the most popular imported commercial substrates available (DADE, Ortho). When compared several other imported commercial substrates of equal quality, the prepared substrate was 3,000 to 6,000 times cheaper. Using the prepared factor VII deficient substrate …

The congenital FVII deficiency (FVIID) is a rare haemorrhagic disorder with an autosomal recessive pattern of inheritance. Data on phenotype and the genotype from 717 subjects in Central Europe (six countries), Latin America (Costa Rica, Venezuela) and United States, enrolled in the Greifswald Registry of FVII Deficiency were analysed. We detected 131 different mutations in 73 homozygous, 145 compound heterozygous and 499 heterozygous subjects. Regional differences were observed in the mutation pattern and the clinical profile of the evaluated patients. Seventy-one per cent of homozygous and 5

Factor V deficiency is usually accompanied with recurrent epistaxis, menorrhagia and haemorrhages after trauma. So far, gastrointestinal bleeding has not been reported. We describe here the first case of severe cecal bleeding in a 28-year-old woman with homozygous factor V deficiency. As a reasonable alternative to large amounts of fresh frozen plasma, we indicated recombinant activated factor VII (rFVIIa), as supra-physiological concentrations directly activate factor X and prothrombin on the surface of activated platelets. With this regimen, the bleeding immediately stopped and the patient w…

BACKGROUND: Congenital factor VII (FVII) deficiency is characterised by a poor and inconsistent relationship between FVII activity (FVII:C) and bleeding phenotype, making perioperative haemostatic management challenging. We aimed to describe perioperative management strategies, identify factors associated with haemostatic treatment, and evaluate perioperative outcome.
METHODS: We conducted a retrospective cohort study of patients with congenital FVII deficiency (severe FVII:C < 10%, moderate 10-19%, mild ≥20% according to ISTH) undergoing surgical procedures or delivery at Charité - Universit…

Factor VII (FVII) Padua is a rare inherited disorder caused by a missense variant in the F7 gene (Arg364Gln), resulting in variable FVII activity depending on the thromboplastin source used in assays. Although often linked to a hypercoagulable state, critical gaps remain in understanding its diagnosis, clinical manifestations, and geographic distribution. Following PRISMA guidelines, a systematic search of PubMed, Scopus, and Web of Science was conducted through February 2026 to identify studies reporting FVII Padua. Data on demographics, laboratory findings, and bleeding or thrombotic events …

Acquired haemophilia A (AHA) is a rare autoimmune bleeding disorder caused by neutralising antibodies against factor VIII. We describe an older man with prostate cancer on active surveillance who presented with spontaneous ecchymoses, progressive anaemia and isolated prolongation of activated partial thromboplastin time. Factor VIII activity was severely reduced, and mixing studies and Bethesda assay confirmed a high-titre factor VIII inhibitor. He achieved initial haemostatic control with recombinant-activated factor VII and was transitioned to emicizumab using an accelerated AHA regimen. No …

Kidney biopsy is the standard of diagnosis for many kidney diseases and conditions. Severe bleeding following a kidney biopsy is a complication for which limited therapeutic interventions exist. The use of recombinant activated factor VII (rFVIIa) is described in pediatric patients to control severe intra-operative hemorrhage and has been used in at least one adult patient to manage post-kidney biopsy hemorrhage. We present a 10-year-old female who presented with acute kidney injury, thrombocytopenia, and hypertension, ultimately diagnosed with ANCA-vasculitis. Following a kidney biopsy, the p…

Coagulation factor (F) VII deficiency is the most frequent among the rare, inherited bleeding disorders and is predominantly caused by missense mutations in the F7 gene. The disease phenotype ranges from asymptomatic cases to extremely severe hemorrhagic forms, requiring prophylactic injections with plasma-derived or recombinant FVII concentrates. In response, we have developed an autologous cell-based approach that corrects the disease-causing mutation in patient- derived induced pluripotent stem cells (iPSC) and generates therapeutic, three-dimensional hepatic organoids (HO). We report the C
