Isolated Pauci-immune pulmonary capillaritis (IPIPC) is a rare form of small vessel vasculitis that affects only the lungs, causing inflammation of pulmonary capillaries and potentially leading to severe outcomes like alveolar haemorrhage. A 23-year-old woman with a prior diagnosis of rheumatoid arthritis presented with hemoptysis and respiratory distress, ultimately diagnosed with IPIPC. Severe cases of tuberculosis can cause vital functional disturbances depending on the stage of the disease and the organ manifestation. There is a particularly high mortality rate in intensive care patients with tuberculosis. Extrapulmonary organ manifestations of this disease in particular can be associated with misdiagnosis and fatal outcome. In this article the intensive care treatment of a patient who suffered from abdominal as well as pulmonary tuberculosis is reported.

Severe hyperleukocytosis caused by acute lymphoblastic leukaemia (ALL) is associated with an increased risk of early death due to the intracranial haemorrhage. We report on a boy who presented with ALL with an extremely high leukocyte count, who developed neurological deterioration due to multiple intracerebral haemorrhages. A case of multiple hornet stings is described with a rapidly fatal course due to the combination of massive haemolysis, coagulopathy, rhabdomyolysis, hyperkalaemia, acute renal failure, encephalopathy, hepatotoxicity and hyperglycaemia. These features of systemic envenomation can all be attributed to the toxic properties of Oriental hornet venom described in in vitro and in vivo experimental studies. Greater awareness of these features, aggressive treatment of hyperkalaemia and early institution of treatments such as peritoneal dialysis and plasma exchange may prevent fatalities in such cases.

A 2-month-old infant died of xanthogranulomatous pyelonephritis. Preoperatively pyonephrosis was suspected, because the child presented with a number of inflammatory, septic symptoms. Nephrectomy was performed and histopathology showed the kidney to be affected by xanthogranulomatous pyelonephritis. Postoperatively the child developed persistent attacks of fever and bronchopneumonia that led to his death with signs of pulmonary insufficiency.
