Postviral fatigue syndrome is associated with persistent infection by a virus. The patient with the condition has failed to eliminate the virus in the usual time. There is little evidence of a deficient immune response by the patient as the explanation for the viral persistence, and it must be assumed that most of the explanation lies in down-regulation of virus expression in infected cells. The general symptomatology of postinfectious syndromes may be mediated by cytokines liberated as part of the infection. Part of the syndrome may also be due to local effects of virus infection in muscles o…

In writing a history of any illness there is always a dilemma whether to attempt the story of the condition 'itself', the medical attempts t … in their actual context (as authors themselves used them), without defining either. Post-infectious fatigue syndrome (PIFS) will cover similar conditions when related to infective episodes. All will be used in a neutral fashion, to refer to changing realities as understood by doctors and historians. This chapter attempts both chronological description and …

The postviral fatigue syndrome overlaps with psychiatry at a number of points. First, there is the influence that some psychological states have on physiological processes, such as immunity. Second, psychological symptoms, particularly depression but also anxiety, are a major feature of the syndrome. Third, difficulties in the doctor-patient relationship are common. Each of these three areas are discussed in detail. Special attention is given to the possible mechanisms underlying the occurrence of psychological symptoms, which are sufficient to make a psychiatric diagnosis in at least two thir…

A model of hemorrhagic shock (arterial mean pressure = 40 mmHg) terminated by death in all cases was developed in 21 dogs. The study was addressed to investigate the central respiratory response, development of diaphragmatic fatigue (FD) and the influence of the suppression of spontaneous rhythmic respiratory muscle activity by mechanical ventilation during shock. We measured diaphragmatic force: transdiaphragmatic pressure (Pdi), spontaneous and stimulated electrical activity of the diaphragm (integrated and rectified electromyogram: Edi) tracheal occlusion pressure (P 0, 1) cardiac output (Q…

Fatigue is a frequent complaint from patients suffering from the hyperventilation syndrome. Fatigue was quantified in terms of the endurance time that a certain force can be generated in a sustained handgrip contraction, and in terms of the time course of changes in certain parameters of the EMG-power spectrum of the contracting muscles. This was done in a group of 25 normal subjects and a group of 30 patients suffering from the hyperventilation syndrome. No difference in endurance times was found. The EMG-fatiguability in the hyperventilation group developed even slower than in the normal gro…

STUDY OBJECTIVES: The diagnosis of myalgic encephalomyelitis/chronic fatigue syndrome (CFS) is based on a constellation of symptoms which center around fatigue. However, fatigue is commonly reported in the general population by people without CFS. Does the biology underlying fatigue in patients with CFS also drive fatigue experienced by individuals without CFS?
METHODS: We used UK Biobank actigraphy data to characterize differences in physical activity patterns and daily temperature rhythms between participants diagnosed with CFS compared to controls. We then tested if single nucleotide varia…

OBJECTIVE: Chronic fatigue syndrome (CFS) is often associated with significant levels of disability. Although fatigue and depression have been found to be independently related to severity of disability, it is not clear how these three factors are mutually related. The present study sought to address this issue by specifically testing a model of mediation whereby depression was hypothesized to influence relations between fatigue and disability.
METHODS: Participants included 90 individuals seeking treatment for CFS at a tertiary care facility. Each provided demographic information and complet…

We investigated whether practicing under physical fatigue induced by cardiovascular exercise influences motor performance and learning in a non-fatigued limb, and whether demographic and physiological factors relate to variability in these effects. Sixty participants were randomly assigned to a Fatigue group (FATIGUE), who performed the 20-m multistage shuttle run test to exhaustion before practice, or a Control group (CONTROL), who performed stretching exercises. Both groups practiced a visuomotor tracking task (VTT) and completed rested and fatigued retention tests. Group-level effects were …

BACKGROUND: Cancer-related fatigue (CRF) is a major issue facing breast cancer survivors (BCS) that can negatively impact their symptoms and quality of life.
OBJECTIVES: The aims of this study were to examine levels of fatigue, identify preferred types of fatigue self-management, and explore the relationship between fatigue levels and management choices by cancer stage.
METHODS: This cross-sectional descriptive study included 229 BCS recruited from 5 hospitals in Korea. The study inclusion criteria were limited to BCS between 20 and 69 years old in stages 1, 2, or 3 who were undergoing or ha…

BACKGROUND: Muscle fatigue of the lower limbs results in dynamic imbalance and gait instability, increasing the risk of falling. However, people might slow walk without physical muscle fatigue due to mental fatigue. Wearable inertial measurement units (IMU) and machine learning approaches have been well employed for recognizing human activities.
RESEARCH QUESTION: The study aims to use a machine learning technique to recognize the data collected from IMUs for physically fatigued or slow-walking gaits. Second, the study aims to reveal the location or the number of IMUs can have the best perfor…

BACKGROUND: Heart rate variability (HRV) is an objective, non-invasive tool to assessing autonomic dysfunction in chronic fatigue syndrome/myalgic encephalomyelitis (CFS/ME). People with CFS/ME tend to have lower HRV; however, in the literature there are only a few previous studies (most of them inconclusive) on their association with illness-related complaints. To address this issue, we assessed the value of different diurnal HRV parameters as potential biomarker in CFS/ME and also investigated the relationship between these HRV indices and self-reported symptoms in individuals with CFS/ME.

OBJECTIVE: Fatigue is known as one of the most common long-term sequelae of Q fever infections. The study aimed to determine the prevalence of fatigue symptoms, chronic fatigue, and chronic fatigue syndrome (CFS) in a sample of patients who were exposed to Q fever (Coxiella burnetii) infection compared to controls, and to contrast Q fever patients with and without fatigue symptoms related to somatoform symptoms, hypochondriacal worries and beliefs, psychosocial complaints, and social support.
METHODS: Cross-sectional study of 84 Q fever exposed patients from a specific region in Jena (Germany…

Case Presentation: A 65-year-old male veteran presented to the Veterans Affairs Boston Healthcare System (VABHS) emergency department with progressive fatigue, dyspnea on exertion, lightheadedness, and falls over the last month. New bilateral lower extremity numbness up to his knees developed in the week prior to admission and prompted him to seek care. Additional history included 2 episodes of transient loss of consciousness resulting in falls and a week of diarrhea, which had resolved. His medical history was notable for hypothyroidism secondary to Hashimoto thyroiditis, seizure disorder, vi…

INTRODUCTION: Sarcoidosis is a systemic granulomatous disease of unknown etiology, primarily affecting the lungs and thoracic lymph nodes. Fatigue is a frequent and disabling symptom in sarcoidosis with a significant impact on quality of life. In Denmark, the incidence of sarcoidosis has increased; however, the extent and risk factors of fatigue have not been investigated and no recent reports of the characteristics of patients with sarcoidosis in Denmark exist.
AIM: To assess the frequency of fatigue in patients with sarcoidosis in Denmark at diagnosis and to investigate if fatigue correlate…
