Recent factor analytic studies have emphasized the importance of both somatic and situational fears in agoraphobia. Franklin (1980) proposed a developmental model of agoraphobic fears in which somatic changes (interoceptive fears), and feared consequences such as panic (outcome fears), produce situational fears through misattribution. Five hypotheses derived from this model were tested on 60 agoraphobics who rated their original and current fears. In all subjects interoceptive and outcome fears relating to panic attacks developed before situational fear and avoidance. Initially outcome fears w…

U.S. Air Force fliers are all volunteers who undergo rigorous training for their profession. Their motivation may be deeprooted and emotional, or may arise from a conscious decision made in early adulthood. Some motivation is flawed and fails early. Other motivation may be eroded by a single dangerous event, by an accumulation of "close calls" (one's own or others'), or by a growing interest in nonaviation elements. Since Air Force fliers may "quit" only at some personal cost to pride or pocketbook, they may develop a fear of flying. This may be truly phobic, a situational reaction, or an awar…

From protozoa to mammals, organisms have been selectively bred for genetic differences in defensive behaviour which are accompanied by differences in brain and other biological functions. Studies of twins indicate some genetic control of normal human fear from infancy onwards, of anxiety as a symptom and as a syndrome, and of phobic and obsessive-compulsive phenomena. Anxiety disorders are more common among the relatives of affected probands than of controls, especially among female and first-degree relatives; alcoholism and secondary depression may also be over-represented. Familial influence…

All human beings are biologic, psychological, and sociologic creatures. The treatment program for agoraphobia at the University of Kansas Medical Center School of Medicine, Kansas City, is designed to deal effectively with problems in these three areas. The biologic area is dealt with by medication; tricyclic antidepressants are used to block panic attacks, and benzodiazepines are given, as needed, for anticipatory anxiety. Group therapy is particularly effective in the areas of individual psychopathology and social anxiety. We have not found agoraphobia difficult to treat and have demonstrate

The purpose of this paper is to elucidate methodological problems in the study of fears and to make suggestions that will facilitate future research. The most basic problem begins with a definition of "fear". Other considerations include methods of assessment, classification, and the instruments used.

Fear-potentiated startle (FPS) has been widely used to study fear processing in humans and rodents. Human studies showed higher startle amplitudes and exaggerated fear reactivity to unpredictable vs. predictable threats in individuals suffering from post-traumatic stress disorder (PTSD). Although human FPS studies use both sexes, a surprisingly limited number of rodent FPS studies use females. Here we investigate the effects of signal-threat contingency, signal-threat order and threat predictability on FPS in both sexes. We use a classic fear-conditioning protocol (100% contingency of cue and …

Exposure to childhood trauma is extremely common (>60 %) and is a leading risk factor for fear-based disorders, including anxiety and posttraumatic stress disorder. These disorders are characterized by deficits in fear extinction and dysfunction in underlying neural circuitry. Given the strong and pervasive link between childhood trauma and the development of psychopathology, fear extinction may be a key mechanism. The present study tests the impact of childhood trauma exposure on fear extinction and underlying neural circuitry. Children (N = 44, 45 % trauma-exposed; 6-11 yrs) completed a nove…

Fear, which can be expressed innately or after conditioning, is triggered when a danger or a stimulus predicting immediate danger is perceived. Its role is to prepare the body to face this danger. However, dysfunction in fear processing can lead to psychiatric disorders in which fear outweighs the danger or possibility of harm. Although recognized as highly debilitating, pathological fear remains insufficiently treated, indicating the importance of research on fear processing. The neurobiological basis of normal and pathological fear reactions is reviewed in this article. Innate and learned fe…

The numbers needed to treat is six 95% CI (3 to 36). Participants with scores <26 report their fear of heights is very much improved. There was a 4.5-point difference in the HIQ score at eight weeks (p = 0.055) on the multiple regression analysis. Conclusions This is the first randomized trial of this novel imaginal intervention which is probably …

Falls are the leading cause of injury-related morbidity and mortality among older adults. In addition to the resulting physical injury and potential disability after a fall, there are also important psychological consequences, including depression, anxiety, activity restriction, and fear of falling. Fear of falling affects 20 to 43% of community-dwelling older adults and is not limited to those who have previously experienced a fall. About half of older adults who experience fear of falling subsequently restrict their physical and everyday activities, which can lead to functional decline, depr…

After a review of the anatomy of the cavernous sinuses (CS), this work presents the clinical picture and imaging protocol of lesions which occur in this area. It outlines extension and imaging features of these lesions. It emphasises MRI appearance, such as T1, T2 and diffusion signal, type of contrast medium uptake. A complementary CT scan is performed if an associated abnormality of the base of the skull is suspected on MRI (lysis, condensation). This paper proposes a straightforward classification system depending on imaging and sets out the principal symptoms of the main aetiologies of CS

The symbolic representation of a disease is related to personal perceptions and cultural background. In the present study, the authors evaluate the population knowledge and fears related to skin and other prevalent or severe diseases. This survey was based on a semi-structured form to investigate demographic aspects, dermatologic consultations, fears and knowledge of 19 dermatoses and 11 prevalent or severe diseases. We interviewed 302 people, of which 54% were women and the mean age was 39 years. Some fears of dermatoses surpass those of severe diseases. Skin cancer and total alopecia disclos…

Social fearfulness is expressed on a continuum of severity from moderate distress to incapacitating fear. The present article focuses on the brain states associated with this broad dimension of social anxiety in humans. In total, 70 published studies are summarized documenting the neural correlates of social anxiety during states of rest, threat-related cognitive-affective activation, and acute symptom provocation. Neural exaggeration in limbic (amygdala) and paralimbic (insula) regions appears to be associated with functional outcomes involving increased attention for and processing of social…
