Patient-initiated second medical opinions: their necessity and economic cost.

PURPOSE: To assess the value of patient-initiated second medical opinions (SMO). METHODS: The authors prospectively collected demographic data from 100 consecutive patients. The authors recorded major changes in the patients' care, such as inappropriate surgery recommended, inadequate treatment performed, and appropriate treatment not recommended. The authors also recorded costs incurred or saved by the patients and the third-party payers. RESULTS: In nearly 15 of the cases, the authors had major disagreements with the initial diagnosis or management. Surgery had been recommended in 28 cases

Costs of Academic Engagement in Organized Plastic and Reconstructive Surgery in the United States.

Academic engagement in plastic and reconstructive surgery (PRS) has grown substantially, marked by an increasing number of professional societies, annual meetings, and peer-reviewed journals. While this growth has facilitated innovation, collaboration, and knowledge sharing, it has also introduced financial burdens. These costs may limit access to academic involvement, especially for early-career surgeons and those without institutional funding. Despite the implications these financial barriers pose for equity, academic engagement, and innovation within the field, the cumulative cost of academ

Oropharyngeal Dysphagia in patients with obstructive sleep apnea syndrome.

Although previous studies demonstrated that patients with obstructive sleep apnea syndrome (OSAS) may present subclinical manifestations of dysphagia, in not one were different textures and volumes systematically studied. The aim of this study was to analyze the signs and symptoms of oropharyngeal dysphagia using fiberoptic endoscopic evaluation of swallowing (FEES) with boluses of different textures and volumes in a large cohort of patients with OSAS. A total of 72 OSAS patients without symptoms of dysphagia were enrolled. The cohort was divided in two groups: 30 patients with moderate OSAS a

Oropharyngeal dysphagia in Parkinson's disease: potential utility of speech acoustic analysis in detection and evaluation of swallowing impairment progression.

AIM OF THE STUDY: This study aimed to evaluate the utility of speech acoustic analysis as a tool for diagnosing dysphagia in Parkinson's disease. CLINICAL RATIONALE FOR THE STUDY: Swallowing impairment is a common and potentially life-threatening symptom of Parkinson's disease (PD). Fiberoptic endoscopic examination of swallowing (FEES), considered the gold standard for dysphagia diagnosis, is often inaccessible in everyday clinical practice. Many studies highlight the link between speech and swallowing impairment in PD patients. Evaluating possible correlations between speech acoustic parame

Which medicines do our patients want from us?

BACKGROUND: The prescribing of medication, whether for infections or injuries, has come under the scrutiny of health maintenance organizations (HMOs). Our goal was to examine patient beliefs about the usefulness of certain classes of medications. METHODS: We surveyed 244 consecutive adult patients who presented to an urban private family medicine practice in Georgia. RESULTS: Regardless of who paid for prescriptions (an HMO or the patient), most people in the survey would be satisfied with over-the-counter medications and reassurance (84% for upper-respiratory infection, 72% for muscle strai

Identifying the causal effects of long-term exposure to PM2.5 and ground surface ozone on individual medical costs in China-evidence from a representative longitudinal nationwide cohort.

BACKGROUND: There is little evidence on whether PM2.5 and ground surface ozone have consistent effects on increased individual medical costs, and there is a lack of evidence on causality in developing countries. METHODS: This study utilized balanced panel data from 2014, 2016, and 2018 waves of the Chinese Family Panel Study. The Tobit model was developed within a counterfactual causal inference framework, combined with a correlated random effects and control function approach (Tobit-CRE-CF), to explore the causal relationship between long-term exposure to air pollution and medical costs. High-tech and low-tech orthopaedic surgery in Sub-Saharan Africa.

BACKGROUND: Zambia's governmental health system suffers from shortage of surgical supplies and poor management skills for the sparse resources at hand. The situation has been worsened by the dual epidemics of HIV disease and tuberculosis. On the other hand the private medical sector has benefited greatly from less bureaucracy under the goverment of the Movement for Multi-party Democracy. DISCUSSION: The Zambian-Italian Orthopaedic Hospital in Lusaka is a well organized small unit providing free treatment of physically disabled children. The running costs are met from the fees charged for priva

The nature and content of telephone prescribing habits in a community practice.

Prescribing medicine over the telephone was studied during a two-month period in a two-physician fee-for-service private family practice. Of 1,264 telephone calls, 392 (31.0%) resulted in a medication prescription. Of these calls, 176 (44.9%) were for refills of previously prescribed medicines. The most common drugs for new telephone prescriptions were decongestants/antihistamines/antitussives, antibiotics, and pain medications. Medicines were more likely to be prescribed via telephone for the diagnoses of upper respiratory infections, headache, low-back pain, or bronchitis. A patient was more

Contraceptive equity: the Birth Control Center of the International Workers Order.

The Equity in Prescription Insurance and Contraceptive Coverage Act, introduced in Congress in 1997 and still unpassed, seeks to redress health insurers' failure to pay for birth control as they pay for other prescription drugs, most paradoxically Viagra. In 1936 the International Workers Order (IWO), a fraternal society, became the first insurer to include contraception in its benefits package. A forerunner in the movement for prepaid medical care, the IWO offered its members primary care and contraceptive services for annual flat fees. Founded at a time when the legal status of contraception

Trend analysis of medical expenses under DRG payment reform in China.

The impact and evolving trends of Diagnosis-Related Group (DRG) payment reform on inpatient medical expenses have drawn substantial academic and societal attention. However, there remains a notable gap in research focusing on the cost-control efficacy and its dynamic trends under this reform. Employing Mann-Whitney U testing, structural change analysis, and interrupted time-series methods, this study quantifies pre-post DRG reform shifts in inpatient expenditures, insurance compensation patterns, and expenditure trajectories. The results showed that per capita hospitalization expenses decrease

Prevalence and Medical Resource of Patients with Diabetic Foot Ulcer: A Nationwide Population-Based Retrospective Cohort Study for 2001-2015 in Taiwan.

Diabetic foot ulcer (DFU) is one of the common complications of diabetes. DFU can cause a huge medical and financial burden due to infections, compromise the quality of life, and increase the mortality rate in patients. However, the consumption of medical resources for DFU is rarely mentioned. A retrospective cohort study was performed. Data were obtained from the National Health Insurance Research Database of Taiwan, and the prevalence and medical utilization data for DFU in 2001-2015 were extracted, followed by the analysis for high-risk populations. Between 2001 and 2015, there were 7511 ne

Temporal Trends of Adults Gaining or Losing Enrolment in Primary Care Medical Homes in Ontario, Canada: A Retrospective Serial Cohort Analysis.

Introduction Patient attachment to a primary care medical home has emerged as a key strategy to strengthen population health in high-income countries, including the U.S., Canada, New Zealand, the United Kingdom, and other European countries. In Canada, Ontario introduced primary care medical homes beginning in 2002, with capitation-based and interdisciplinary team-based models becoming increasingly prominent over time. Citing cost concerns, however, various government policies were subsequently introduced to limit physician entry into these types of medical homes. Entry into medical homes, rem

Thyroid function in neonates conceived after hysterosalpingography with iodinated contrast.

STUDY QUESTION: Does exposure to preconceptional hysterosalpingography (HSG) with iodinated oil-based contrast affect neonatal thyroid function as compared to iodinated water-based contrast? SUMMARY ANSWER: Preconceptional HSG with iodinated contrast did not influence the neonatal thyroid function. WHAT IS KNOWN ALREADY: HSG is a commonly applied tubal patency test during fertility work-up in which either oil- or water-based contrast is used. Oil-based contrast contains more iodine compared to water-based contrast. A previous study in an East Asian population found an increased risk of conge
