Identification of the lateral femoral safe zone for drilling during LISS plate fixation of distal femur fractures.

BACKGROUND: The incidence of arterial injury associated with femoral fractures is approximately 1%. Lateral sub-muscular plate fixation is gaining popularity for the management of distal femoral fractures. The objective of this study was to assess the iatrogenic risk to the superficial femoral artery (SFA) during Less Invasive Stabilisation System (LISS) plate fixation of distal femoral fractures by analysing the range of distances and angles between LISS plate screws and the drilling line to the SFA. MATERIALS AND METHODS: We identified all patients who underwent LISS plate fixation of dista

Outcome analysis of retrograde nailing and less invasive stabilization system in distal femoral fractures: A retrospective analysis.

BACKGROUND: TWO MAJOR THERAPEUTIC PRINCIPLES CAN BE EMPLOYED FOR THE TREATMENT OF DISTAL FEMORAL FRACTURES: retrograde intramedullary (IM) nailing (RN) or less invasive stabilization on system (LISS). Both operative stabilizing systems follow the principle of biological osteosynthesis. IM nailing protects the soft-tissue envelope due to its minimally invasive approach and closed reduction techniques better than distal femoral locked plating. The purpose of this study was to evaluate and compare outcome of distal femur fracture stabilization using RN or LISS techniques. MATERIALS AND METHODS:

Treatment of Bilateral Distal Femoral Shaft Fractures in a Paraplegic Patient with Intramedullary Nailing: A Case Report.

INTRODUCTION: Lower extremity fractures are a frequent complication in patients with chronic spinal cord injury due to significant bone mineral density loss and altered bone metabolism. While non-operative management has traditionally been favored, advances in surgical techniques have expanded treatment options, including intramedullary nailing and plate fixation. However, the optimal approach remains controversial, especially in paraplegic patients with distal femoral fractures. This case report highlights the challenges and complications associated with surgical management in this unique pat

Evaluation of pediatric distal femoral physeal fractures and the factors impacting poor outcome requiring further corrective surgery.

Fractures of the distal femur carry a significant risk of physeal arrest and resulting growth complications which often require additional surgeries to correct the deformity. This study examines the risk of needing corrective procedures as a child approaches skeletal maturity. A retrospective analysis of patients treated at a single institution for distal femoral physeal fractures from 2000 to 2015 was performed. Association between sex, age, Salter-Harris (SH) class, and fracture displacement with the risk of physeal arrest were examined. Association between years of growth remaining to skele

Contradictory working length effects in locked plating of the distal and middle femoral fractures-a biomechanical study.

BACKGROUND: Working length have been reported to affect the plate stress and fixation stiffness. However, the results of previous studies have been controversial. The present study was to determine working length effects on different locations of femoral bone gap. METHODS: Five composite femurs with wide bone gaps at five levels (G1, 2, 3, 5, and 7), were fixed with locking plates. G1-3, G5 and G7 represented gaps at distal femur, distal-middle femur and middle femur respectively. Strain gauges were applied near the screw holes. The plate-bone constructs were loaded through a hemicylinder on

[Fractures involving the distal femoral epiphysis. Long-term outcome after completion of growth in primary surgical management].

During a period of 12 years (1983-1994) we operated 13 patients with fractures involving the distal epiphyseal cartilage of the femur. All patients could be followed up in clinical and radiological examination following the completion of growth. By examination no patients had any complaints. The clinical examination revealed no limitation in range of motion of the hip or knee joint compared to the other side in any of the patients. 5 patients out of 13 (38%) were found to have a correct axial alignment with no axis deviation, shortening or lengthening of the leg. The other 8 patients (62%) pre

Is the compressive nail method capable of accelerating bone union in patients with femoral distal diaphysis fracture?

OBJECTIVES: This study was conducted to evaluate clinically and radiologically the patients for whom compressive and non-compressive intramedullary nails were applied and to analyse the compression effectiveness in adult femur distal diaphysis fractures. HYPOTHESIS: The compressive locked nails shortened the union period in the distal diaphysis fractures, and it was found to be an effective method, as the pseudoarthrosis rate was lower. MATERIAL AND METHODS: A retrospective study was conducted among 59 patients for whom the compressive and non-compressive IMNs were applied to treat femur dis

Osteosarcoma in the distal femur two years after an ipsilateral femoral shaft fracture: a case report.

INTRODUCTION: The duration of symptoms preceding a definitive diagnosis of osteosarcoma is quite long. Pathological radiological signs are often evident by the time of diagnosis. Although several case reports have been published on osteosarcoma of the femur, to the best of our knowledge this report is the first one with such an unusual clinical course. CASE PRESENTATION: We describe the case of a 58-year-old Caucasian man who presented with a femoral shaft fracture. Two years post-trauma osteosarcoma in the ipsilateral distal femur was diagnosed. Was it coincidence? We think that the history

The Use of the Masquelet Technique in the Treatment of Pathological Distal Third Femoral Fracture Secondary to Chronic Osteomyelitis.

INTRODUCTION: Chronic osteomyelitis is a persistent infection of the bone that poses significant challenges, particularly when associated with pathological fractures and extensive bone defects. This case report highlights the application of Masquelet's induced membrane technique (MIMT) in managing a complex distal femur defect in a 50-year-old male with a long-standing history of chronic osteomyelitis. The patient presented with a non-union fracture, severe osseous destruction, and infection, requiring a multidisciplinary approach. CASE REPORT: The first stage involved radical debridement of

A spontaneously healed distal femoral fracture in an infant born preterm: A case report and review of the literature.

Distal femoral fractures are uncommon in the newborn period, and conventional radiographs can miss the diagnosis in infants born prematurely due to delayed ossification. Hence, modalities like ultrasound may have a role in diagnosis. Herein, we report a male infant born preterm with a swollen knee due to a distal femoral fracture after a non-traumatic Caesarean section. The fracture was not present on the conventional radiograph. Instead, it was detected on a repeat sonogram and healed spontaneously with no complications. This case describes the importance of musculoskeletal radiographic exper

Distal Femur Valgus Deformity After Rigid Intramedullary Nailing of Adolescent Femoral Shaft Fracture.

A 12-year-old girl developed a distal femoral shaft fracture treated with lateral trochanteric entry intramedullary nail fixation. The nail was retained after union because of a persistent nonossifying fibroma at the previous fracture site. At 16 months after surgery, marked valgus deformity was noted at the distal femur, with signs of implant haloing and loosening, suggesting repetitive motion and stress concentration of forces at the distal femur. Owing to recognition before skeletal maturity, the valgus was corrected with hemiepiphysiodesis. This finding illustrates the importance of follow

Bilateral proximal tibia stress fracture following bilateral distal femoral hemiepiphysiodesis - A rare complication.

Correction of lower limb deformities by hemiepiphyseodesis with 8-plates is gaining popularity among deformity surgeons. Complications including metal failure, recurrence of deformity have been reported but stress fracture has not been reported. We have encountered bilateral proximal metaphyseal tibia stress fractures following correction of valgus deformity of both knees by hemiepiphyseodesis of distal femur following inadvertent overcorrection to a varus deformity following loss of follow up. The varus deformity has been reversed by guided growth of distal femoral physis resulting in fractur

The effects of different cortical impingement scenarios on the biomechanical features of retrograde femoral nails in the management of distal femoral fractures: A finite element analysis.

OBJECTIVE:: This study aimed to assess the effects of different cortical impingement scenarios on the biomechanical features of retrograde femoral nails in managing distal femoral fractures. METHODS:: A mesh femur model was constructed using computed tomography (CT) images. Five different cortical impingement (CI) scenarios were designed: central model (CM), lateral CI (LCI), medial CI (MCI), anterior CI (ACI), and posterior CI (PCI). The fixation stability was evaluated by femoral head movement, stress, and elastic strain at the fracture site. The maximum stress on the femoral cortex and the

Early Outcome of Distal Femur Fracture Fixation in Adults: A Comparative Study Between Dynamic Condylar Screw and Distal Femoral Locked Plate.

BACKGROUND: Distal femoral fractures are difficult to treat due to their proximity to the knee joint, and different modalities have been developed over the years to overcome these challenges. Some studies have shown variable results of different operative modalities. However, there is little evidence to show the superiority of one over the other. OBJECTIVE: To compare the functional outcome of operative treatment between dynamic condylar screw (DCS) with a side plate and distal femoral locked compression plate (DFLCP) using the Schatzker and Lambert criteria. The mean time to union and compli

The impact of medial-first dual plating for reduction of distal femoral fractures: a retrospective comparative cohort study.

In dual-plate fixation of distal femoral fractures (DFF), reducing the relatively spared medial side first and using this metaphyseal-medial condylar segment as a template to reduce the lateral side may effectively facilitate reduction. This study evaluated the effectiveness of the medial-first dual-plate fixation in DFF. A retrospective study was conducted on 77 patients who underwent open reduction and internal fixation (ORIF) for DFF at a single institution between April 2012 and 2021. The patients were divided into a group in which medial-side ORIF was first performed followed by a lateral

Risk Factors for Nonunion Following Lateral Locked Plating of Distal Femoral Fractures: A Bayesian Analysis of 560 Patients.

BACKGROUND: Lateral locked plating remains an important treatment strategy for distal femoral fractures but has been associated with nonunion rates ranging from 6% to 20%. The objective of this study was to identify factors associated with nonunion following lateral locked plating of distal femoral fractures with use of a Bayesian analysis. METHODS: All consecutive patients ≥18 years of age who were treated with lateral locked plating for a distal femoral fracture at 2 Level-I trauma centers between 2006 and 2024 and who had ≥3 months of follow-up were included. Retrograde intramedullary nailing or locked plating for stabilisation of distal femoral fractures? A comparative study of 193 patients.

PURPOSE: The aim of this study was to evaluate the results of distal femoral fracture fixation of two different methods, lateral locking plate (LP) or an Intra-medullary nail (IMN), in patients managed in our institution. More specifically, to assess: (a) if there was a difference in functional outcomes between the LP and IMN groups; (b) whether the rate of complications was different between the two groups. METHODS: Between January 2009 and December 2018 adult patients with distal femoral fractures managed in our unit with either LP or IMN for extra and intra-articular fractures were eligibl
