Practice variation in diagnosis, monitoring and management of fetal growth restriction in the Netherlands.

OBJECTIVES: Fetal growth restriction (FGR) is a condition characterized by its complexity in diagnosis and management. There is a need for early accurate diagnosis, evidence-based monitoring and management of FGR to improve neonatal outcomes. This study evaluated differences and similarities in protocols of Dutch hospitals in the approach of (suspected) FGR in the context of the national guideline. STUDY DESIGN: FGR protocols were collected from Dutch hospitals between November 2019 and June 2020. Collected data were coded for further analysis and categorized in eight predetermined key domain

Update on Fetal Monitoring: Overview of Approaches and Management of Category II Tracings.

Electronic fetal monitoring (EFM) is widely used to assess fetal status in labor. Use of intrapartum continuous EFM is associated with a lower risk of neonatal seizures but a higher risk of cesarean or operative delivery. Category II fetal heart tracings (FHTs) are indeterminate in their ability to predict fetal acidemia. Certain patterns of decelerations and variability within this category may be predictive of neonatal morbidity. Adjunct tests of fetal well-being can be used during labor to further triage patients. Intrauterine resuscitation techniques should target the suspected etiology of

The role of continuous fetal monitoring in the management of preterm premature rupture of membranes.

OBJECTIVE: Continuous fetal monitoring (CFM) is often used in the management of preterm premature rupture of membranes (PPROM) but there is little evidence to support this approach. The objective of this study was to evaluate the clinical outcome of PPROM when managed by CFM. METHODS: A retrospective review was conducted of 129 cases PPROM outcomes for the period January 1, 1998 to December 31, 2003. All women underwent CFM. Delivery was carried out for non-reassuring fetal testing, vaginal bleeding, evidence of infection, positive vaginal pool phosphatidylglycerol when available, and spontan

A prospective comparison of selective and universal electronic fetal monitoring in 34,995 pregnancies.

We investigated the effects of using intrapartum electronic fetal monitoring in all pregnancies, as compared with using it only in cases in which the fetus is judged to be at high risk. Predominant risk factors included oxytocin stimulation of labor, dysfunctional labor, abnormal fetal heart rate, or meconium-stained amniotic fluid. This prospective alternate-month clinical trial took place over a 36-month period during which 34,995 women gave birth. In alternate months, either 7 (for "selective monitoring") or 19 (for "universal monitoring") fetal monitors were made available in the labor and

[Polymorphonuclear infiltration of the placenta in cases with internal fetal monitoring (author's transl)].

The infectious morbidity due to intra-amniotic fetal monitoring was studied morphologically in 100 spontaneous deliveries with intra-amniotic fetal monitoring. The control group consisted of 100 spontaneous deliveries with external fetal monitoring. It was shown that internal fetal monitoring does not increase the infectious morbidity with total labours of up to 12 hours. The occurrence of inflammatory cell infiltrates in the placenta, the membranes and the umbilical cord is not in each case due to a bacterial intra-uterine infection. The pathogenesis of these infiltrates is discussed comparin

Monitoring and timing of delivery in suspected late fetal growth restriction at term.

BACKGROUND: Infants affected by late fetal growth restriction are at risk of adverse perinatal outcomes, either due to placental insufficiency or the consequences of late iatrogenic prematurity. There is no randomized trial reporting the optimal timing of delivery, and recommending delivery at 37 weeks can cause harm. Antenatal ultrasound parameters can be used to risk-stratify these pregnancies into low-risk cases, which are associated with a lower likelihood of adverse outcomes and are thus suitable for full-term delivery, and high-risk cases, which may benefit from early-term delivery. Methods for Monitoring Risk of Hypoxic Damage in Fetal and Neonatal Brains: A Review.

Fetal, perinatal, and neonatal asphyxia are vital health issues for the most vulnerable groups in human beings, including fetuses, newborns, and infants. Severe reduction in oxygen and blood supply to the fetal brain can cause hypoxic-ischemic encephalopathy (HIE), leading to long-term neurological disorders, including mental impairment and cerebral palsy. Such neurological disorders are major healthcare concerns. Therefore, there has been a continuous effort to develop clinically useful diagnostic tools for accurately and quantitatively measuring and monitoring blood and oxygen supply to the

Fetal heart rate monitoring in nonobstetric surgery: a systematic review of the evidence.

OBJECTIVE: Concern for fetal well-being during maternal nonobstetric surgery may result in obstetricians and other maternity care providers being asked to perform intraoperative fetal heart rate (FHR) monitoring. We systematically reviewed the evidence regarding the use of FHR monitoring during nonobstetric surgery after potential fetal viability (>22 weeks gestational age), and examined the FHR patterns and outcomes reported. DATA SOURCES: A systematic review of the evidence was performed. Sources included databases (MEDLINE, EMBASE, Cochrane, and CENTRAL), hand searching, guidelines, confer

Diagnostic accuracy of fetal heart rate monitoring in the identification of neonatal encephalopathy.

OBJECTIVE: To estimate the diagnostic accuracy of electronic fetal heart rate abnormalities in the identification of neonates with encephalopathy treated with whole-body hypothermia. METHODS: Between January 1, 2007, and July 1, 2013, there were 39 neonates born at two hospitals within our system treated with whole-body hypothermia within 6 hours of birth. Neurologically normal control neonates were matched to each case by gestational age and mode of delivery in a two-to-one fashion. The last hour of electronic fetal heart rate monitoring before delivery was evaluated by three obstetricians b

Perinatal monitoring of fetal well-being in the presence of congenital heart block.

We present a case of congenital complete heart block associated with maternal autoantibodies in which a normal labor and delivery could safely be allowed to proceed despite the absence of the usual tool of electronic fetal heart rate monitoring for fetal distress, by the technique of rupturing membranes and using a fetal scalp electrode electrocardiographic tracing to assess the fetal atrial rate.

The incidence of fetal asphyxia in six hundred high-risk monitored pregnancies.

Six hundred high-risk monitored obstetric patients were reviewed for evidence of fetal asphyxia at delivery. The over-all incidence was 20 per cent, i.e., 8 times the incidence in a normal obstetric population. Highly significant indicators of risk for asphyxia were severe toxemia (79 per cent), prematurity with further medical or obstetric complications (36 per cent), and clinical fetal distress, particularly meconium staining with fetal heart rate abnormality (33 per cent). All obstetric, medical, or gestational complications in this review were associated with an increased risk for fetal as

Epidural analgesia, fetal monitoring and the condition of the baby at birth with breech presentation.

Between December 1970 and March 1973, 138 patients with a singleton fetus presenting by the breech after 36 weeks of pregnancy were deemed suitable for vaginal delivery under epidural analgesia; 130 were delivered vaginally, 10 of them by breech extraction. There was one stillbirth and no neonatal deaths. Epidural analgesia for vaginal breech delivery seemed beneficial. In 65 cases it was possible to compare the umbilical vein pH with the Apgar score at one minute. In 35 patients a continuous recording of the fetal heart rate was used to predict the Apgar score at one minute and the results ar

Perioperative Maternal-Fetal Outcomes in the Setting of Minimally Invasive Fetal Therapy for Complex Monochorionic Pregnancies with Monitored Anesthesia Care.

INTRODUCTION: Fetoscopic selective laser photocoagulation (FSLPC) and selective cord occlusion with radiofrequency ablation (RFA) can improve fetal outcomes when vascular anastomoses between fetuses cause twin-to-twin transfusion syndrome (TTTS) or selective fetal growth restriction (sFGR) in multiple gestation pregnancies with monochorionic placentation. This study analyzed perioperative maternal-fetal complications and anesthetic management in a high-volume fetal therapy center over a 4-year period. METHODS: Included patients received MAC for minimally invasive fetal procedures for complex

Low sensitivity of the new FIGO classification system for electronic fetal monitoring to identify fetal acidosis in the second stage of labor.

OBJECTIVE: In 2015, new FIGO guidelines for CTG interpretation were presented (FIGO-15). In 2017, the previous Swedish guidelines (SWE-09) were replaced with guidelines adapted to FIGOs (SWE-17). The performance of these three templates had not been scientifically evaluated before its clinical implementation. The objective of this study was to compare the sensitivity and specificity to detect fetal acidosis at birth using these three templates during the second stage of labor. STUDY DESIGN: This case-control study included 295 neonates with cord blood pH < 7.05 and 591 controls with pH ≥ 7.15

Fetal bladder sagittal length: a simple monitor to assess normal and enlarged fetal bladder size, and forecast clinical outcome.

PURPOSE: We examined if the parameter of fetal bladder sagittal length (FBSL) could serve as a monitor of normative and enlarged fetal bladder size. MATERIALS AND METHODS: There were 76 consecutive cases examined between 1984 and 2000 that included measurement of fetal bladder size as FBSL and postnatal urological followup. Fetal images used to assess normal bladder size were derived from cases in which the bladder was normal on prenatal imaging and postnatal testing. An enlarged bladder was categorized as being greater than the 95% CI for a given gestational age (GA). The presence and extent
