Health care in Canada is universal, accessible, transferable and publicly funded. Each of Canada's provinces has the responsibility for health care funding and delivery through its ministry of health, controlled by the governing provincial party and overseen by a Minister of Health. The Federal Government is responsible for ensuring the provinces conform to the spirit and regulations within the Canada Health Act and for broad programme funding, through the federal Minister of Health. As such, access to emergency health services is available to all Canadians free of direct charge. Some aspects

A key component in the dehospitalization of persons with chronic mental illnesses and their translocation to a wide range of settings has been the shift from state to federal funding encouraged by the Social Security Administration's restrictions on funding for institutions for mental diseases (IMDs), usually referred to as the IMD exclusion. The overall effect of the exclusion, which limits federal funding for mentally ill patients receiving care in many settings, including state psychiatric hospitals, has been to create incentives for states to move patients out of state hospitals, which has

Health and health service access in Zambian prisons are in a state of 'chronic emergency'. This study aimed to identify major structural barriers to strengthening the prison health systems. A case-based analysis drew on key informant interviews (n = 7), memos generated during workshops (n = 4) document review and investigator experience. Structural determinants were defined as national or macro-level contextual and material factors directly or indirectly influencing prison health services. The analysis revealed that despite an favourable legal framework, four major and intersecting structural

BACKGROUND: Hospital accreditation is a systematic evaluation program conducted by an independent external accreditation body. It is the most common health assessment approach for enhancing hospital quality and safety. However, its implementation in Afghanistan faces substantial governance challenges. This study examined accreditation governance challenges and proposed solutions in Afghanistan.
METHODS: A qualitative study using interpretative phenomenological analysis (IPA) conducted in 2024. Semi-structured interviews carried out with 75 key stakeholders including policymakers, accreditatio

OBJECTIVES: This study examines how digital finance, digital transformation, and governance quality jointly influence healthcare efficiency across emerging and high-income health systems. We assess whether digital innovations contribute to productivity improvements and whether institutional capacity conditions these effects.
STUDY DESIGN: We analyse a balanced panel of 13 countries over 2000-2023 using a multi-method empirical strategy. Baseline OLS and fixed- and random-effects models establish structural patterns, while the two-step system GMM corrects for endogeneity, persistence, and unob

Point-of-care testing for hepatitis C virus (HCV) offers multiple benefits to key populations and healthcare providers, but it has not achieved widespread implementation. This analysis investigates the impact of the health system on the sustainability of point-of-care HCV testing in Australia. Between September 2023 and January 2024, in-depth, semi-structured interviews were conducted with people involved in HCV policymaking in Australia. Data were coded using WHO's Health System Building Blocks framework (i.e., Health Workforce, Health System Financing, Medical Technologies, Leadership and Go

Uganda's 2017 Joint External Evaluation (JEE) revealed critical gaps in International Health Regulations (IHR) implementation, with limited progress despite ongoing efforts. Coordination mechanisms failed to address multisectoral implementation challenges. To accelerate progress, in 2018, Uganda established a dedicated National Action Plan for Health Security Acceleration (NAPHS) Team (A-Team)-a multidisciplinary unit embedded within government structures to facilitate cross-sector coordination and reduce bureaucratic barriers. We conducted a multimethod evaluation using outcome harvesting, ke

BACKGROUND: This study aimed to enhance insights into the key characteristics of maternal and neonatal mortality declines in Ethiopia, conducted as part of a seven-country study on Maternal and Newborn Health (MNH) Exemplars.
METHODS: We synthesised key indicators for 2000, 2010 and 2020 and contextualised those with typical country values in a global five-phase model for a maternal, stillbirth and neonatal mortality transition. We reviewed health system changes relevant to MNH over the period 2000-2020, focusing on governance, financing, workforce and infrastructure, and assessed trends in m

BACKGROUND: Low-income and middle-income countries continue to face challenges in financing health programmes due to budgetary constraints and decreased donor funding. Off-budget financing has become crucial for controlling diseases like malaria, HIV/AIDS and tuberculosis (TB). However, there is limited evidence on alternative domestic funding approaches to support implementation of healthcare programmes in Tanzania.
OBJECTIVE: The study explored stakeholder perspectives on alternative domestic funding approaches to support malaria, HIV/AIDS and TB services in Tanzania.
METHODS: A qualitativ

INTRODUCTION: Sustaining Field Epidemiology Training Programs (FETPs) is critical for long-term public health capacity. Institutionalization-embedding programs within national health systems-is a major step toward sustainability. This manuscript explores the experiences, perceived challenges, and strategies related to the institutionalization, sustainability, and funding of FETPs in the Eastern Mediterranean Region (EMR) and offers recommendations to strengthen their long-term integration within national health systems.
METHODS: A participatory regional workshop was held in Amman from May 18-

In 1993 a large outbreak of paralytic poliomyelitis occurred in Sudan as a result of an accumulation of large numbers of susceptible children that was accelerated by faltering immunization services. The extent of the outbreak led to the rapid rehabilitation of Sudan's Expanded Programme on Immunization (EPI); the government began financing vaccine purchase, operational aspects of EPI were decentralized, vaccine delivery was changed from a mobile to a fixed-site strategy, a solar cold chain network was installed, inservice training was resuscitated, and social mobilization was enhanced. BACKGROUND: Nepal has been undergoing demographic and epidemiological transitions, marked by an increasing burden of non-communicable diseases (NCDs) and injuries. These transitions have led to financial implications, including rising out-of-pocket (OOP) expenses. This study reviews and synthesizes evidence on the status, issues and challenges in health financing system, policies, and programs to achieve universal health coverage (UHC) in Nepal.
METHODS: We conducted a scoping review of literature on Nepal's health financing system, policies, and programs. A search strategy was developed usin

BACKGROUND: Pakistan has a high hepatitis burden for both hepatitis C virus (HCV) and hepatitis B virus (HBV). To achieve World Health Organization (WHO) 2030 targets for hepatitis elimination, there is a need to constitute progress in the country, find the barriers and strategies for HCV elimination, and take actions to address the gaps.
METHODS: We collected data from (1) WHO estimates in 2020, (2) midterm review questionnaire of the WHO regional action plan, and (3) WHO estimates on immunization. We analyzed these data to inform (1) the burden defined as prevalence and mortality and (2) re

Sierra Leone's health system remains highly dependent on external aid, with donor funding accounting for approximately 47% of total health expenditure between 2020 and 2024, compared with a government contribution of around 25-26%. Recent reductions in development assistance for health, including a sharp in-year contraction of USAID-supported health programming during 2024-25 and an approximately 15% reduction in Global Fund allocations, have exposed major structural vulnerabilities in the health financing system. We conducted a rapid mixed-methods study to examine the effects of recent and an

INTRODUCTION: In January 2025, the Trump Administration announced a 90-day freeze on all US government (USG) foreign aid, which subsequently became a permanent shutdown of the US Agency for International Development in July 2025. The sudden withdrawal of USG financing disrupted health and humanitarian systems worldwide, with disproportionate impacts on women, girls and other marginalised populations. This study documents impacts of the funding freeze during its early months (February-March 2025) on global health programmes.
METHODS: This rapid mixed-methods assessment combined a global online

Over the last decade, Singapore has grappled with substantial healthcare challenges, chiefly a bourgeoning aging population and a mounting burden of chronic diseases. The oral health landscape has also changed, with the Ministry of Health placing a greater focus on a life-course approach supported by policies that facilitate Singaporeans to receive dental care appropriately and affordably. A pivotal oral health policy is the National Dental Strategy, a comprehensive framework governing dental services in the public sector. This strategy encompasses aspects such as financing, workforce manageme
