The use of gelatin capsules for ingestion of formula in dietary treatment of maternal phenylketonuria.

Maternal phenylketonuria (PKU) represents a high risk for birth defects, including mental retardation, in offspring. Thus, it could cancel gains represented by the prevention of PKU-induced mental retardation in the current generation. Effective dietary treatment of maternal PKU pregnancies could avoid this potentially tragic occurrence. However, dietary compliance is often difficult because a necessary component of the diet, medical nutritional formulas, often have an unpleasant taste and odour. We treated the second pregnancy of a phenylketonuric women who had required extended hospitalizati

Diarrhea in tube-fed patients: feeding formula not necessarily the cause.

PURPOSE: This study of diarrhea in tube-fed patients was undertaken to determine the proportion of cases in which feeding formula is not responsible for the diarrhea, the causes other than the feeding formula, and the diagnostic approach to diarrhea in tube-fed patients. PATIENTS AND METHODS: Inpatients at the Truman Memorial Veterans Hospital who received nasoenteric feeding during the time period from October 1986 through May 1988 were eligible for this study. Of 123 patients who received nasoenteric feeding, 32 patients had documented diarrhea (greater than 500 mL per day for at least two

Experimental methods of ethanol administration.

Techniques are reviewed for the experimental feeding of alcohol, including a liquid diet procedure invented 25 years ago. This technique results in much higher ethanol intake than with other approaches. As a consequence, various complications observed in alcoholics can be reproduced in animal models. These include fatty liver, hyperlipemia, various metabolic and endocrine disorders, tolerance to ethanol and other drugs, physical dependence and withdrawal and, in the baboon, liver fibrosis and cirrhosis. Variations of the liquid diet formulation are compared, and adequacy of nutrition in terms

Nutritional management of inflammatory bowel disease.

Nutritional deficits commonly occur in patients of all ages suffering from chronic inflammatory bowel diseases. Growth failure represents a common, serious complication unique to the pediatric age group. Although the etiology of nutritional problems and growth failure is multifactorial, malnutrition owing to inadequate nutrient intake is the primary cause. Nutritional supplementation via the enteral or parenteral route restores normal body composition and can reverse linear and ponderal growth failure if administered effectively and prior to bone maturation. Both parenteral and enteral nutriti

Evaluation of infant feeding in acute gastroenteritis.

Sixty-eight bottle-fed babies under 9 months of age with mild acute gastroenteritis were observed to evaluate current feeding regimens following acute gastroenteritis in infancy. All babies were fed for 24 h with a glucose-electrolyte mixture (GEM) and then randomly assigned to either a gradual reintroduction to their normal milk, i.e., slow regrade; immediate return to full-strength formula; or a rapid regrade to a hypoallergenic whey hydrolysate formula. All groups were well matched for age, sex, ethnic origin, nutritional state, and degree of hydration. There was no significance difference

Elemental diets in the prophylaxis and therapy for intestinal lesions: an update.

The recognition of potentially noxious physiologic substances in the intestinal milieu prompted the use of an "elemental" semihydrolyzed formula diet in the prophylaxis of experimental acute ischemic enteropathy. Elemental diets have been used in the management of a variety of digestive diseases. An elemental diet protects the intestinal mucosa of rodents from radiation injury and facilitates mucosal healing. Clinical trials have shown the benefits of this form of treatment in the prevention of acute radiation enteropathy and in the therapy for delayed radiation enteropathy and Crohn's disease

Evolving trends in enteral alimentation.

Nutrition has become a cornerstone of surgical patient care. With better understanding of metabolic and compositional requirements, great advances have been made in the area of total parenteral nutrition. Recent attention to full utilization of alimentary tract function has prompted a resurgence of interest in dietary formulas and methods of delivery. Three new approaches to the alimentary tract provide better access for feeding. Needle catheter jejunostomy allows early alimentary tract utilization following operations, while percutaneous endoscopic gastrostomy and jejunostomy provide long-ter

Medication bezoar: intestinal obstruction by an isocal bezoar. Case report and review of the literature.

Medications may occasionally obstruct the gastrointestinal tract by virtue of their physical mass. Obturative obstruction of the alimentary tract is reportedly caused by an increasing number of medications, including hydroscopic bulk laxatives, cholestyramine, nonabsorbable antacids, and vitamin C tablets. Inspissated Isocal tube feedings caused jejunal obstruction in a postoperative patient. Medication bezoars are a rare cause of intestinal obstruction that may result in significant patient morbidity, including bowel necrosis, perforation, and peritonitis. The radiographic appearance may mimi

Preventive lipid-based nutrient supplements given with complementary foods to infants and young children 6 to 23 months of age for health, nutrition, and developmental outcomes.

BACKGROUND: One nutritional intervention advocated to prevent malnutrition among children is lipid-based nutrient supplements (LNS). LNS provide a range of vitamins and minerals, but unlike most other micronutrient supplements, LNS also provide energy, protein and essential fatty acids. Alternative recipes and formulations to LNS include fortified blended foods (FBF), which are foods fortified with vitamins and minerals, and micronutrient powders (MNP), which are a combination of vitamins and minerals, OBJECTIVES: To assess the effects and safety of preventive LNS given with complementary food

Food intolerance and Crohn's disease.

It has been claimed that prolonged remissions of Crohn's disease can be achieved after enteral or parenteral nutrition, by identifying and excluding foods that exacerbate a patient's symptoms. The occurrence of food intolerances were assessed after induction of remission with elemental diet in 42 eligible patients to whom single foods were introduced over five days. Suspect foods were reinvestigated with open and if possible, double blind rechallenge. Fourteen patients (33%) dropped out of the study because of relapse of disease unrelated to food (n = 8) or because of difficulties in complying

Dietary fructose in the management of intractable diarrhea of infancy.

Carbohydrate digestion/absorption was evaluated in 11 infants with intractable diarrhea while they were receiving a carbohydrate-free soy-isolate formula. Seven patients were fed within 48 h of admission. Enteral feedings were initiated in the remainder after they had gained 1 kg while receiving parenteral nutrition. All feedings were initially administered by continuous nasogastric infusion. Nine patients were initially fed formula with polymeric glucose; two received fructose as the carbohydrate source, based on a documented history of polymeric glucose intolerance. Five of the nine develope

Identification of causative foods in children with eosinophilic esophagitis treated with an elimination diet.

BACKGROUND: Eosinophilic esophagitis (EoE) is a chronic inflammatory disease with isolated eosinophils in the esophagus predominantly triggered by foods. The optimal testing to identify inciting foods remains unclear. OBJECTIVES: We sought to determine the effectiveness of allergy testing-directed diets in patients with EoE. METHODS: A retrospective analysis of all children with EoE seen at the Children's Hospital of Philadelphia between 2000 and 2011 identified 941 patients with EoE. Skin prick tests (SPTs) and atopy patch tests (APTs) were conducted, and predictive values were calculated.

Microbial contamination of enteral feeding formulas and diarrhea.

Twenty-five medical and surgical patients receiving liquid ready-to-use sterile enteral formulas were evaluated prospectively to investigate the relation of diarrhea to serum albumin level, total lymphocyte count, delayed hypersensitivity to purified protein derivative, antibiotic therapy, administration rate and site of enteral formula, and microbial contamination of enteral feeds. Formulas were administered to 6 patients with hang times of up to 6 h by pump-assisted continuous drip and to 19 patients with hang times of up to 3 h as a bolus feeding. Samples of formulas for microbial culture w

Postdischarge formula consumption in infants born preterm.

In 31 infants born preterm and formula fed ad libitum, all milk intake was weighed from hospital discharge to nine months post-term. Mean daily milk intake was high, reaching 230 g/kg before four weeks post-term and was still over 150 g/kg beyond six months. Five of the 31 infants (16%) consumed 300-350 g/kg; 50% 'voluntarily' consumed more than upper recommended limits for energy intake and 35% did so for protein intake.

Could the use of saliva substitutes improve food oral processing in individuals with xerostomia? A systematic review.

Xersotomia is associated with food avoidance and low nutritional assessment. This review seeks to document whether products called "saliva substitutes" or "artificial saliva" can really replace saliva in food oral processing. Pubmed and Science Direct were searched for articles using the keywords "saliva substitutes" and "artificial saliva." An advanced search was applied using the terms "xerostomia" and/or "food oral processing" and/or "eating" and/or "mastication" and/or "chewing" and/or "swallowing." The analysis methods and the inclusion criteria were documented in a protocol published in

Nutritional Profile and Functional Properties of Orange-Fleshed Sweet Potato, Bambara Groundnut, and Brown Rice Blended Complementary Food.

A sweet potato-based complementary food was formulated to enhance the use of locally available ingredients to address micronutrient deficiencies, particularly vitamin A deficiency in Ghana. Five complementary food blends were formulated using orange-fleshed sweet potato (OFSP), Bambara groundnut, and brown rice flours with the possibility to meet the nutritional requirements of infants aged 6-23 months in alignment with the CAC standards. A sensory evaluation was conducted, consisting of breastfeeding and nonbreastfeeding mothers, to identify the most suitable blend, after which the nutritiona
